Antimicrobial stewardship programs in sepsis treatment: principles, impact, and future directions.

Sepsis continues to be a serious global health concern with increasing mortality rates owing to antimicrobial resistance (AMR). Antimicrobial stewardship programs (ASPs) have emerged as critical tools for optimizing antibiotic use and reducing AMR. This review examines the applications of ASPs in sepsis treatment, highlights their role in improving patient outcomes, and addresses the challenges posed by multidrug-resistant organisms. The review begins by exploring the global AMR crisis and its implications for sepsis therapy and then evaluates the Surviving Sepsis Campaign's guidelines and the

International expert consensus statement on the diagnosis and management of congenital nephrogenic diabetes insipidus (arginine vasopressin resistance).

Congenital nephrogenic diabetes insipidus (NDI; also known as arginine vasopressin resistance) is a rare inherited disorder of water homeostasis, caused by insensitivity of the distal nephron to arginine vasopressin. Consequently, the kidney loses its ability to concentrate urine, which leads to polyuria, polydipsia and the risk of hypertonic dehydration. The diagnosis and management of NDI are very challenging and require an integrated, multidisciplinary approach. Here, we present 36 recommendations for diagnosis, treatment and follow-up in both children and adults, as well as emergency manag

Inequalities in treatment planning for children with intellectual disabilities: A questionnaire study of dentists in Europe.

BACKGROUND: Children with intellectual disabilities experience major inequality in the field of oral health, including a higher number of extracted teeth. The literature explains this difference in terms of higher levels of dental disease but does not mention the possibility of inequality in the treatment options offered these children. AIM: The aim is to investigate whether the same treatment options are offered by practitioners to children with and without intellectual disability in equivalent clinical circumstances. DESIGN: A scenario involving a clinical dental situation was developed, w

Screening and treatment of chagas disease in organ transplant recipients in the United States: recommendations from the chagas in transplant working group.

Donor-derived transmission of Trypanosoma cruzi, the etiologic agent of Chagas disease, has emerged as an issue in the United States over the past 10 years. Acute T. cruzi infection causes substantial morbidity and mortality in the posttransplant setting if not recognized and treated early. We assembled a working group of transplant infectious disease specialists, laboratory medicine specialists, organ procurement organization representatives and epidemiologists with expertise in Chagas disease. Based on review of published and unpublished data, the working group prepared evidence-based recomm

Cystic echinococcal liver disease: new insights into an old disease and an algorithm for therapy planning.

Human cystic echinococcosis (CE) continues to be a major health problem in developing countries. A review of current literature discloses four alternatives for the management of active CE, consisting of surgery, percutaneous treatment (PT), chemotherapy, and follow-up without intervention, but no clear guidelines for directing patients to the different management options. Palliation of symptoms or prevention of complications is the main rationale for the treatment of CE. Surgery has long been considered as the gold standard treatment. However, a meta-analysis comparing the clinical outcomes of

Results of an intervention in an academic Internal Medicine Clinic to continue, step-down, or discontinue proton pump inhibitor therapy related to a tennessee medicaid formulary change.

BACKGROUND: In July 2005, the State of Tennessee Medicaid Program (TennCare) announced formulary changes for proton pump inhibitors (PPIs) to be implemented in August 2005. Prior to these changes, pantoprazole was the only preferred PPI, and there were no restrictions to its use. The revised formulary included 3 preferred PPIs (esomeprazole, lansoprazole, and omeprazole OTC), all of which required prior authorization (PA). In order to obtain an approved PA for a PPI, the patient was required to have either (a) a diagnosis of erosive esophagitis, Barrett's esophagus, Schatzki's ring, a patholog

USED FOR WARFARE: Bioterrorism is a perfectly foreseeable eventuality. It is defined by the international use, or menace of use, of living organisms whatever their nature, or of substances derived from these organisms, aimed at provoking a disease or the death of human beings, animals or plants. UPDATED COUNTERACTING STRATEGIES: More than 180 infectious agents could be used for terrorist ends. The recent events in the dawn of this twenty-first century have reactivated and notably updated the strategies to counteract such an event. Clinical and therapeutic guidelines have been circulated in man

Reducing Laboratory Testing and Blood Draws in the Newborn Nursery to Increase High-value Care.

INTRODUCTION: Newborn infants undergo multiple blood tests before hospital discharge, some of which are unnecessary due to outdated hospital policies and provider variability. Reducing nonevidence-based laboratory testing can improve healthcare value, and clustering of blood sampling can reduce pain for newborns. METHODS: This quality improvement project aimed to reduce the weekly number of laboratory tests and blood draws per patient-day for newborns by 10% within 8 months. A multidisciplinary team developed and addressed key drivers with Plan-Do-Study-Act cycles. Interventions included high

Counselling regarding a planned provision of medical aids is currently neither reimbursed nor regulated in Germany. It is offered by a range of providers with employees with varying levels of qualification and may be provided in a personal conversation, by phone, or remotely. In many cases, counselling is omitted entirely between the prescription and trial use or delivery of the product. The quality of medical aids counselling, which plays a key role in temporary, supportive or palliative care for urinary incontinence, is therefore considered problematic. In 2019, the working group "Geriatric U

Country data on AMR in Brazil in the context of community-acquired respiratory tract infections: links between antibiotic susceptibility, local and international antibiotic prescribing guidelines, access to medicine and clinical outcome.

BACKGROUND: Antimicrobial resistance (AMR) is one of the biggest threats to global public health. Selection of resistant bacteria is driven by inappropriate use of antibiotics, amongst other factors. COVID-19 may have exacerbated AMR due to unnecessary antibiotic prescribing. Country-level knowledge is needed to understand options for action. OBJECTIVES: To review the situation with respect to AMR in Brazil and initiatives addressing it. Identifying areas where more information is required will provide a call to action to minimize any further rises in AMR within Brazil and to improve patient

The APHEA project as a turning point in modern environmental epidemiology: Current and future relevance.

In the late 80s and early 90s, it was unclear whether the observed levels of air pollution were related to health effects, with fragmented evidence based on a few single-city studies. The Air Pollution and Health: a European Approach (APHEA)-funded projects started in 1993 and continued until 2009 and, later, within the context of the APHENA collaboration with researchers from the United States and Canada. APHEA was among the first to introduce the multicity/multicountry approach, using standardized methods and specifically developed protocols, including examining exposure-response functions a
