[A new ECG marker of anterior acute myocardial infarction].

ST-segment elevation is the hallmark of acute transmural myocardial ischemia caused by acute occlusion of a coronary artery. ST-segment elevation is the major criterion for the patients with chest pain to immediate reperfusion therapy. Despite its clinical importance, the mechanism of ST-elevation remains unclear. Two patients are reported with proximal left anterior descending coronary occlusion but without ST-segment elevation. The distinct ECG patterns were tall, with symmetrical T-waves and upsloping and digoxin-like ST-segment depression. Patients with these ECG patterns need immediate co

Is isolation of all four pulmonary veins necessary in patients with paroxysmal atrial fibrillation?

The upper pulmonary veins (PVs) are responsible for the majority of atrial fibrillation (AF) triggering foci, whereas the inferior PVs are more difficult to ablate and prone to postablation ostial stenosis. Most procedure failures can be attributed to incomplete isolation or recurrent PV left atrial reconnection rather than to identification of another focus. Furthermore, in certain patients AF triggers can be detected outside the PVs, and local denervation of the ganglionic plexus around the superior PV left atrial junctions following the ablation procedure may also play a role in eliminating

Electrocardiographic manifestations: bundle branch blocks and fascicular blocks.

Intraventricular conduction block is the general name given to a varied group of electrocardiographic entities. All share a common finding of some degree of delay in ventricular activation; recognition of these blocks hinges upon analysis of the QRS complex, as well as the ST-T changes associated with them. Bundle branch block (right or left), and fascicular block (left anterior or left posterior) are all examples of intraventricular conduction block. Causation of intraventricular conduction block may be cardiac or noncardiac; early recognition of the etiology may be of clinical importance. Th

[Immediate and long-term results of radiofrequency ablation of accessory atrioventricular pathways].

The aim of this study was to report the authors' experience of radiofrequency ablation of accessory atrioventricular pathways over a 10 year period (01-91 to 10-00), and the effect of the "learning curve" on the results. The data of 400 patients admitted to primo-ablation of a bundle of Kent was analysed retrospectively. A total of 481 ablations were performed (1.20 per patient). The cumulative global success in the 414 accessory pathways treated was 90.6%. The primary success rate increased from the 1st to the 4th quartile from 68 to 97% (p = 0.0001). The mean duration of fluoroscopy and numb

[Sudden infant death syndrome (SIDS): a case with accessory pathways and resorptive degeneration].

We report the case of a 71 day-old female infant victim of SIDS. Histological observations were focused on the cardiac conduction system. We observed accessory pathways of Mahaim type with areas of resorptive degeneration. These findings, associated with particular conditions and/or neurovegetative stimuli, could cause potentially malignant arrhythmias.

[Radiofrequency ablation of incessant supraventricular tachycardia due to anomalous pathway of retrograde decremental conduction].

Woman, 46 years-old with incessant supraventricular tachycardia and tachycardiomyopathy. The diagnosis of a concealed retrograde long conduction time accessory pathway was obtained with the delay of the next atrial activation by delivering a ventricular premature beat during His bundle refratoriness. During electrophysiologic investigation the earliest atrial activation was found to be within the coronary sinus ostium. Two 25 watts applications of radiofrequency were followed by the interruption of the incessant supraventricular tachycardia. The patient has remained symptom free without recurr

Proarrhythmic effect of adenosine in a patient with atrial flutter.

Adenosine has been proposed as a useful diagnostic agent in patients with narrow complex regular tachycardia of uncertain origin. Its effects are usually transient owing to its extremely short plasma half life and, as a consequence, it is thought to be safer than other drugs used in the acute treatment of such arrhythmias. However, adenosine had a proarrhythmic effect when administered to a patient in order to confirm the diagnosis of atrial flutter. As expected, a transient increase in atrioventricular block was seen but this was followed by a doubling of the ventricular rate and haemodynamic

Propagation model using the DiFrancesco-Noble equations. Comparison to reported experimental results.

Propagation, re-entry and the effects of stimuli within the conduction system can be studied effectively with computer models when the pertinent membrane properties can be represented accurately in mathematical form. To date, no membrane models have been shown to be accurate representations during repolarisation and recovery of excitability, although for the Purkinje membrane the DiFrancesco-Noble (DN) model has become a possibility. The paper examines the DN model, restates its equations and compares simulated waveforms in a number of propagation contexts to experimental measurements reported

Neonatal refractory supraventricular tachycardia: successful treatment with amiodarone.

A 3-week-old neonate with supraventricular tachycardia unresponsive to traditional therapy was treated successfully with amiodarone. An electrophysiologic study suggested the presence of a concealed left-sided accessory atrioventricular pathway. Because of its significant side effects, amiodarone should be used only as a last resort in the treatment of neonatal supraventricular tachycardia.

Intracardiac electrophysiologic studies in the medical evaluation of aviators.

Ten aviators with cardiac symptoms or electrocardiographic abnormalities underwent electrophysiologic testing. Four patients were studied because of symptoms including palpitations, nearsyncope, and sudden cardiac death. Six patients were studied because of electrocardiographic abnormalities including AV block, right bundle branch block, sinus bradycardia, ventricular tachycardia, and questionable Wolff-Parkinson-White syndrome. Three patients with bradycardia and/or AV block were found to have increased vagal tone. A fourth patient had nearsyncope and intra-Hisian block. Of four patients eval

Ventricular pre-excitation and prolonged Q-T interval syndromes in a patient with mitral valve prolapse.

A 34-year-old woman with mitral valve prolapse, frequent ventricular dysrhythmias, and chest pain was studied. Surface electrocardiograms demonstrated short PR and prolonged Q-T intervals. A-V nodal conduction times during atrial pacing were characteristic of A-V nodal bypass tract function. The case is thus interpreted to be one of overlap between three syndromes known to predispose to cardiac dysrhythmias, i.e., mitral valve prolapse, Lown-Ganong-Levine syndrome, and prolonged Q-T interval syndrome.

[Repetitive supraventricular tachycardia. Case report and electrophysiologic bases].

A typical case of repetitive supraventricular tachycardia is reported. Periods of normal heart rate are interrupted by short bursts of supraventricular tachycardia. In the case described this phenomenon can be explained by coexistence of reentry and an automatic rhythm from a subsidiary pacemaker. This pacemaker was found to be in the bundle of His; its automaticity showed a rate somewhat faster than that of the sinus node. Careful analysis of the surface electrocardiograms allowed a definition of the electrophysiologic events starting a run of tachycardia. The circus movement entertaining the
