[Sinus node syndrome].

Recently disorders of sinus node function have found increasing interest in clinical medicine thanks to new diagnostic and therapeutic developments. This paper represents a comprehensive review of these conditions, combined under the name "Sick Sinus Syndrome" (SSS). Besides a detailed analysis of 63 cases seen at our institution, the results of other groups are compared and extensively discussed. The clinical picture of the SSS is characterized by a wide variety of bradycardiac and tachycardic atrial arrhythmias, occurring separately or in combination. These can be classified in three subgrou

Function and modulation of premotor brainstem parasympathetic cardiac neurons that control heart rate by hypoxia-, sleep-, and sleep-related diseases including obstructive sleep apnea.

Parasympathetic cardiac vagal neurons (CVNs) in the brainstem dominate the control of heart rate. Previous work has determined that these neurons are inherently silent, and their activity is largely determined by synaptic inputs to CVNs that include four major types of synapses that release glutamate, GABA, glycine, or serotonin. Whereas prior reviews have focused on glutamatergic, GABAergic and glycinergic pathways, and the receptors in CVNs activated by these neurotransmitters, this review focuses on the alterations in CVN activity with hypoxia-, sleep-, and sleep-related cardiovascular dise

Video-tracking and high-speed bright field microscopy allow the determination of swimming and cardiac effects of D. magna exposed to local anaesthetics.

Local anaesthetics are among the most used drugs in clinical practice, but once they are released to the environment, the effects on the aquatic fauna remain uncertain. This study evaluates, for the first time, the impact of tetracaine, lidocaine and bupivacaine on the survival rate and physiological effects of cladocera Daphnia magna. Video-tracking and image processing allowed us to obtain changes in behaviour parameters like swimming average velocity and mean square displacement. We found that tetracaine shows the most severe effect. A high-speed microscopy system was also used to determine

The Relationship Between Admission Insulin Resistance Index (AIRI) and In-Hospital Outcome in Non-Diabetic Acute Coronary Syndrome.

BACKGROUND: Acute coronary syndrome (ACS) is a major cardiovascular problem due to its high hospitalization and mortality rates. One of the risk factors for atherosclerosis that leads to ACS is insulin resistance (IR) which plays a role in the pathogenesis and development of cardiovascular events. This study aims to determine the relationship between IR and in-hospital outcomes in non-diabetic patients with ACS. METHODOLOGY: This was a cohort study conducted from January-June 2021. Insulin resistance was assessed using the Admission insulin resistance index (AIRI). This measurement was perfor

Effect of Beta-Blocker Therapy, Maximal Heart Rate, and Exercise Capacity During Stress Testing on Long-Term Survival (from The Henry Ford Exercise Testing Project).

Whether lower heart rate thresholds (defined as the percentage of age-predicted maximal heart rate achieved, or ppMHR) should be used to determine chronotropic incompetence in patients on beta-blocker therapy (BBT) remains unclear. In this retrospective cohort study, we analyzed 64,549 adults without congestive heart failure or atrial fibrillation (54 ± 13 years old, 46% women, 29% black) who underwent clinician-referred exercise stress testing at a single health care system in Detroit, Michigan from 1991 to 2009, with median follow-up of 10.6 years for all-cause mortality (interquartile range

Supernormal conduction in the anomalous bundles of the Wolff-Parkinson-White syndrome: an overlooked electrophysiologic property with potential clinical implications.

The anterograde refractory period (RP) of the accessory pathway (AP) is the main determinant factor of ventricular rate during atrial fibrillation in the Wolff-Parkinson-White (WPW) syndrome. We describe 3 examples of anterograde supernormal conduction (SNC) and 1 of retrograde SNC in APs. The paradoxical early recovery of propagation due to SNC, well inside a prolonged anterograde RP in the AP, may play a relevant role to determine the rate of ventricular response during atrial fibrillation, eventually leading to extremely fast ventricular rates, syncope, and even ventricular fibrillation in

Cardiotoxicity in magnesium-deficient rats fed cadmium.

We studied the cardiotoxicity in magnesium (Mg)-deficient male rats fed 50 micrograms/g Cd for 45 consecutive days. Cd at a low concentration (0.28 ppm) in the heart induced cardiotoxic effects manifested by a decrease of the heart rate and weight and histopathological changes in the presence of Mg deficiency through Cd supplementation to a normal diet did not induce any cardiotoxic effect. Cardiac output (CO) did not increase in response to the decrease in the total peripheral resistance (TPR) in the Mg-deficient rats fed Cd, suggesting that supplementation of Cd to the Mg-deficient diet may

Energy expenditure of below-knee amputees during harness-supported treadmill ambulation.

Traditional rehabilitation of amputees is primarily aimed at strengthening remaining musculature necessary for prosthetic use and gait training. Available gait training time, however, is often limited by pain, residual limb skin tolerance, and the patient's cardiovascular endurance. Harness-supported treadmill ambulation is a rehabilitation technique being used by physical therapists to decrease an individual's body weight by a given percentage during exercise. This, theoretically, allows an amputee to ambulate on a prosthesis at a lower energy cost. The purpose of this study was to compare th

Electrocardiographic repolarization during stress from awakening on alarm call.

OBJECTIVES: The present study aimed to characterize the electrocardiographic features of cardiac repolarization during an arousal reaction in healthy subjects. BACKGROUND: Electrocardiographic ST segments and T waves may indicate the activity of cardiac autonomic nervous control. Abnormal dynamics of repolarization are considered to reveal susceptibility to cardiac arrhythmias. Responses in normal subjects may help to understand the effects on patients' arrhythmias. METHODS: Ambulatory electrocardiography was performed in 30 healthy physicians during emergency calls while they were on duty i

Automated echocardiographic analysis of systemic ventricular performance in hypoplastic left heart syndrome.

In patients with hypoplastic left heart syndrome, the right ventricle is positioned as the systemic ventricle. Monitoring its function is important but difficult because of its unusual position and geometry. The purposes of this study were to determine the feasibility of applying automatic echocardiographic measurements to the evaluation of right ventricular function in patients with hypoplastic left heart syndrome and to compare their function with that of normal subjects. Eleven patients with hypoplastic left heart syndrome (mean age 1.6 years) were evaluated with automatic border detection.

Validation in the General Population of the iHealth Track Blood Pressure Monitor for Self-Measurement According to the European Society of Hypertension International Protocol Revision 2010: Descriptive Investigation.

BACKGROUND: High blood pressure is one of the most common reasons why patients seek assistance in daily clinical practice. Screening for hypertension is fundamental and, because hypertension is identified only when blood pressure is measured, accurate measurements are key to the diagnosis and management of this disease. The European Society of Hypertension International Protocol revision 2010 (ESH-IP2) was developed to assess the validity of automatic blood pressure measuring devices that are increasingly being used to replace mercury sphygmomanometers. OBJECTIVE: We sought to determine wheth

Measuring real-time blood viscosity with a ventricular assist device.

Ventricular assist devices assist in blood circulation and form a crucial component of artificial hearts. While it is important to measure parameters such as the flow rate, pressure head and viscosity of the blood, implanting additional devices to do such measurements is inadvisable. To this end, we demonstrate the adaptation of a ventricular assist device for the purpose of measuring blood viscosity. Such an approach eliminates the need for additional dedicated viscometers in artificial hearts. In the proposed method, the blood viscosity is measured by applying radial vibrational excitation t
