Paralytic instability of the hip in myelomengoceles.

The results of a Colonna type arthroplasty with several modifications were evaluated in 29 hips of 22 myelomeningocele children with subluxations or dislocations. The operations were performed only in children with high neurosegmental levels where no muscle transfers were possible or when previous surgical procedures failed to maintain hip stability. Improvement in radiographic and clinical stability was demonstrated in 79 per cent of the hips. Eighty-three per cent of the hips lost motion in the flexion-extension arch with a mean loss of 30 degrees. The arthroplasty improved hip stability but…

Roentgenography of congenital dislocation of the hip.

The chronically dislocated hip of a 15-month-old girl was studied postmortem. The cartilaginous components of the pelvis (both true and false acetabula) and proximal femur were demonstrated roentgenographically and correlated with gross anatomical observations. These findings should be useful in the interpretation of radiographs in congenital hip dysplasia in children.

Dislocation of the hip in birth defects.

Several heritable disorders--although rare--are reviewed that are directly or indirectly related to dislocation of the hip. With the ever increasing list of clinical syndromes and our growing understanding of genetics, the physician who first sees these patients should broaden his views to identify these conditions and establish the diagnosis. The effectiveness of treatment often depends upon such knowledge.

Overdiagnosis of congenital dislocation of the hip.

In a retrospective study of 548 cases of CDH diagnosed and treated immediately after birth in the city of Malmo, 90 per cent may not have developed manifest dislocation even without treatment when the expected incidence is taken into account. It could, however, be demonstrated from variables such as sex ratio, birth rank, fetal presentation and the size of the capital epiphyses that the entire group indeed had characteristics relating them to CDH. These characteristics were more obvious in coses with a firmly established diagnosis. Cases in whom the diagnosis could not be confirmed radiologica

Suppurative arthritis of the hip in children.

Of thirty-seven children treated at the Mayo Clinic for suppurative arthritis of the hip and followed for at least one year (average, 8.3 years), nineteen had a satisfactory result and eighteen, an unsatisfactory result. An analysis of the records of the thirty-seven children revealed the following: (1) duration of symptoms was the most important prognostic feature, there being no unsatisfactory results if treatment was instituted less than four days after onset of symptoms; (2) fourteen of sixteen patients with associated osteomyelitis and six of eight with so-called penicillin-resistant Stap…

Reconstructive surgery in the myelomeningocele hip.

In 17 myelomeningocele patients with subluxating or dislocated hips, 31 posterolateral iliopsoas were treated by transfers, as described by Sharrard, in addition to complimentary procedures to balance muscle power and center the hip joint. Surgical goals are to stablize the hips within the acetabulum and prevent the occurrence of severe fixed flexion and adduction contractures of the hip and secondary lumbar lordosis. Surgery is performed at about 1 year of age with bilateral adductor transfer to the ischium, followed in 2 weeks by the posterolateral transfer of the iliopsoas muscle, first on …
