Management of sepsis following injury.

Sepsis is a significant cause of late morbidity and mortality in the severely injured patient. In addition to the risk factors of shock, multiple transfusions, and contamination, the trauma patient may have the additional factor of severe immunologic depression. The prevention of sepsis should be an early consideration. Invasive diagnostic and therapeutic maneuvers should be limited to those that are absolutely necessary, since the incidence of nosocomial infection is high. Prophylactic antibiotics should not be misused, as these may increase the risk of serious, resistant infections. Frequent

Gene Therapy in Hemophilia: Clinical Advances, Immunological Challenges, and Emerging Therapeutic Perspectives.

Gene therapy is reshaping the therapeutic paradigm in hemophilia by enabling sustained endogenous clotting factor production after a single administration. This approach moves disease management beyond lifelong replacement therapy. While clinical trials have demonstrated marked reductions in bleeding rates and treatment burden, real-world implementation has revealed emerging complexities. These include interindividual variability in transgene expression reflected by a progressive reduction in circulating FVIII or FIX activity over time, uncertainty regarding the long-term durability of express

Prolonged COVID-19 infection in a patient with B-cell acute lymphoblastic leukemia maintained on convalescent plasma until recovery with monoclonal antibodies.

INTRODUCTION: Patients with hematologic malignancies who undergo immunosuppressive therapies such as chimeric antigen receptor T-cell (CAR-T) therapy are at high risk of prolonged SARS-CoV-2 infection due to impaired humoral immunity. Treatment options remain limited, with variable efficacy, in such settings. METHODS: We describe a 21-year-old man with Down syndrome and B-cell acute lymphoblastic leukemia complicated by B-cell aplasia following CD19-directed CAR-T therapy. The patient developed COVID-19 and experienced persistent symptomatic infection, with high viral load and prolonged rever

Anti-CD20 Disrupts Meningeal B-Cell Aggregates in a Model of Secondary Progressive Multiple Sclerosis.

OBJECTIVE: Therapies targeting B cells have been used in the clinic for multiple sclerosis (MS). In patients with relapsing MS, anti-CD20 therapy often suppresses relapse activity; yet, their effect on disease progression has been disappointing. Most anti-CD20 therapeutic antibodies are type I, but within the unique microenvironment of the brain, type II antibodies may be more beneficial, as type II antibodies exhibit reduced complement-dependent cytotoxicity and they have an increased capacity to induce direct cell death that is independent of the host immune response. METHODS: We compared t

Persistent Pyelonephritis and Renal Abscess due to Mycoplasma hominis in an Immunocompromised Patient: A Case Report.

BACKGROUND: Mycoplasma hominis is a fastidious, cell wall-deficient bacterium commonly colonising the lower genitourinary tract. While usually commensal, it can cause opportunistic infections in immunocompromised patients. As it can be difficult to culture on standard mediums and is resistant to many first-line antimicrobials, diagnosis and treatment are often delayed. CASE PRESENTATION: We report the case of a 31-year-old woman with Stage IVA extranodal marginal zone lymphoma previously treated with bendamustine-rituximab, who presented with fever, suprapubic pain and haematuria in the setti

Microbiome-urothelium crosstalk in bladder cancer: From dysbiosis to clinical translation.

Bladder cancer (BCa) ranks among the most frequently diagnosed malignancies worldwide. The advent of high-throughput sequencing technologies has revealed distinct urinary microbial signatures that differentiate healthy individuals from BCa patients. While growing evidence implicates microbiota dysbiosis in the pathogenesis, progression, and therapeutic response of BCa, establishing a direct causal relationship remains challenging due to methodological heterogeneity across studies. This review summarizes the current knowledge of the human urinary microbiome (urobiome) in both health and disease

Metabolic reprogramming tailors T cell immunity in sepsis.

Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection, characterized by persistently high morbidity and mortality. Current treatment strategies have limitations, particularly the persistence of an immunosuppressed state. Recent studies have revealed that sepsis not only causes immune system dysregulation but also leads to metabolic disturbances, specifically metabolic reprogramming in T cells-a field still in its early stages. This review systematically explores the mechanisms of T-cell metabolic reprogramming in sepsis, including enhanced glycolysi

Mitochondrial transfer in sepsis: Dual role, mechanistic networks, and translational strategies.

Sepsis is a life-threatening clinical syndrome characterized by dysregulated host response, metabolic disturbance, and multiple organ dysfunction. Mitochondrial damage and bioenergetic failure are core pathological events driving sepsis progression. As a critical intercellular communication mechanism, mitochondrial transfer (MT) participates in mitochondrial quality control, energy homeostasis, and inflammatory regulation under septic stress. This review systematically summarizes the structural and functional mitochondrial injury in sepsis and endogenous quality control pathways. We focus on t

Breaking the barrier: from biosynthetic inhibition to multidimensional modulation of the mycobacterial cell wall in tuberculosis therapy.

Tuberculosis remains a leading cause of global mortality, with the rise of multidrug resistant and extensively drug-resistant Mycobacterium tuberculosis strains highlighting an urgent need for novel therapeutics. The mycobacterial cell wall has long served as a validated drug target, owing to its unique architecture comprising arabinogalactan, peptidoglycan, and mycolic acids. This review provides a comprehensive overview of recent advances in cell wall targeting anti tuberculosis drug discovery, encompassing both established and emerging targets within the major biosynthetic pathways. Macrophages in Septic Cardiomyopathy: Heterogeneity, Function, and Therapy.

Sepsis is a life-threatening condition characterized by multiple organ dysfunction resulting from a dysregulated host immune response to infection. Septic cardiomyopathy (SCM), one of the most severe complications of sepsis, manifests as myocardial tissue injury and functional impairment and serves as a primary contributor to mortality in septic patients. Macrophages, pivotal innate immune cells in the heart, are classified into distinct subsets based on their developmental origins and microenvironmental cues. The dual roles of macrophage subsets, mediating both injury and protection, during t

ATG5 suppresses type I IFN-dependent neutrophil effector functions during Mycobacterium tuberculosis infection in mice.

Inflammation is critical for controlling infections but can cause disease when unchecked. During Mycobacterium tuberculosis (Mtb) infection, neutrophil-dominated inflammation is associated with exacerbated disease. ATG5 expression by neutrophils mediates autophagy-independent control of infection but mechanistic understanding of how this regulates protective neutrophil function is lacking. Using genetic mouse models along with in vivo and in vitro infection systems, we report herein that ATG5 is required in neutrophils to suppress type I interferon-induced PAD4-mediated histone citrullination

Mycoplasma agalactiae Vaccines: Current Status, Hurdles, and Opportunities Due to Advances in Pathogenicity Studies.

Contagious agalactia (CA) is a serious multietiological disease whose classic etiological agent is Mycoplasma agalactiae and which causes high morbidity and mortality rates in infected herds. CA is classified as a notifiable disease by the World Organization for Animal Health due to its significant worldwide economic impact on livestock, primarily involving goat and sheep farms. The emergence of atypical symptoms and strains of M. agalactiae in wildlife ungulates reestablishes its highly plastic genome and is also of great epidemiological significance. Antimicrobial therapy is the main form of

Current and future treatments for tuberculosis.

Guidelines on the treatment of tuberculosis (TB) have essentially remained the same for the past 35 years, but are now starting to change. Ongoing clinical trials will hopefully transform the landscape for treatment of drug sensitive TB, drug resistant TB, and latent TB infection. Multiple trials are evaluating novel agents, repurposed agents, adjunctive host directed therapies, and novel treatment strategies that will increase the probability of success of future clinical trials. Guidelines for HIV-TB co-infection treatment continue to be updated and drug resistance testing has been revolutio

Zika Virus: Immune Evasion Mechanisms, Currently Available Therapeutic Regimens, and Vaccines.

The sudden emergence of infectious pathogens such as Zika virus (ZIKV) holds global health concerns. Recent dissemination of ZIKV from Pacific to Americas with an upsurge of congenital anomalies and Guillain Barre Syndrome (GBS) in adults has created an alarming situation. High-throughput studies are in progress to understand ZIKV's mode of pathogenesis and mechanism of immune escape, yet the pathogenesis remains obscure. Mainly ZIKV's envelope (E) protein and nonstructural proteins (mainly NS1 and NS5) manipulate host cell to support viral immune escape by modulation of the interferon pathway

Pharmacogenetics: a practical role in predicting antiretroviral drug toxicity?

Research into the toxic effects of antiretroviral therapy has made tremendous progress, particularly over the 5 years since the earliest descriptions of the lipodystrophy syndrome. In particular, the contribution of specific antiretroviral drugs to these toxicity syndromes is becoming clearer, along with their pathophysiological mechanisms. This knowledge can now direct research on host genetic factors that may significantly influence the risk of both short-term and long-term toxicities. At present, the genetic association with abacavir hypersensitivity appears sufficiently strong to consider

Physical and cDNA mapping in the DBH region of human chromosome 9q34.

Staphylococcus comprises up to two-thirds of all pathogens in orthopedic implant infections and they are the principal causative agents of two major types of infection affecting bone: septic arthritis and osteomyelitis, which involve the inflammatory destruction of joint and bone. Bacterial adhesion is the first and most important step in implant infection. It is a complex process influenced by environmental factors, bacterial properties, material surface properties and by the presence of serum or tissue proteins. Properties of the substrate, such as chemical composition of the material, surfa

Klebsiella pneumoniae in inflammatory bowel disease and rectal cancer: from gut inflammation to a potential driver of carcinogenesis.

The gut microbiota is a critical modulator of intestinal inflammation and colorectal cancer (CRC), yet the specific microbial drivers that bridge inflammatory bowel disease (IBD) to rectal carcinogenesis remain poorly defined. Klebsiella pneumoniae (K. pneumoniae) has recently emerged as a compelling candidate at this nexus; however, several critical questions remain unresolved: (i) whether K. pneumoniae colonization is a cause or a consequence of rectal carcinogenesis; (ii) how strain level heterogeneity determines pathogenic versus commensal outcomes; and (iii) which rectal microenvironmenta

Low-dose docetaxel reprograms CAR T cells for enhanced solid tumor immunity.

BACKGROUND: Chimeric antigen receptor (CAR) T cell therapy in solid tumors is hampered by dense stromal barriers, rapid functional exhaustion, and limited persistence. Here, we report that low-dose docetaxel acts as an immunomodulatory primer that reprograms CAR T cells to address these barriers. Retrospective analysis of breast cancer patients identifies host lymphocyte reserve as a key determinant of docetaxel treatment benefit, implying an immunological component beyond direct tumor cell killing. METHODS: Sub-cytotoxic docetaxel concentrations were evaluated in primary human T cells and CA

LC3-associated phagocytosis in macrophage responses to Paracoccidioides spp.

BACKGROUND: Paracoccidioidomycosis (PCM) is a systemic infection that is endemic to Latin America, caused by thermodimorphic fungi from the Paracoccidioides genus. These fungi are facultative intracellular parasites of macrophages. LC3-associated phagocytosis (LAP), a non-canonical form of autophagy, plays a critical role in the response of these phagocytes to similar pathogens. OBJECTIVES: In this study, we investigated the role of LAP in the macrophage responses to Paracoccidioides spp. METHODS: We detected LAP in macrophages infected with Paracoccidioides spp by immunofluorescence microsc

Schistosomal extracellular vesicles: Mechanisms of immune modulation and emerging clinical applications.

Schistosomiasis remains a persistent global health challenge and a leading cause of parasitic morbidity worldwide. The complex, enduring relationship between Schistosoma parasites and their human hosts is underpinned by a sophisticated molecular dialogue, much of which is mediated by extracellular vesicles (EVs). These parasite-derived vesicles, which carry proteins, lipids, and nucleic acids, act as a double-edged sword in host-parasite interactions. On one hand, they facilitate immune evasion by promoting Th2 polarization, expanding regulatory T cells, and driving alternative macrophage acti
