Understanding the Independent Risk Factors of Anterior Shoulder Dislocation Using MRI.

BACKGROUND: Anterior shoulder dislocation is the most common dislocation in the musculoskeletal system, frequently occurring in traumatic ev … sion, coracohumeral distance, humeral containing angle, glenoid height, depth and width (diameter), humeral head diameter, glenoid width-to-depth ratio, glenoid height-to-diameter ratio, and humeral diameter-to-glenoid width ratio. RESULTS: Each group consisted of 49 MRI series from patients aged 17-75 years. The independent parameters associated with anterio …

Operative treatment of 2-part surgical neck fractures of the proximal humerus (AO 11-A3) in the elderly: Cement augmented locking plate Philos™ vs. proximal humerus nail MultiLoc®.

BACKGROUND: Proximal humeral fractures are with an incidence of 4-5 % the third most common fractures in the elderly. In 20 % of humeral fractures there is an indication for surgical treatment according to the modified Neer-Criteria. A secondary varus dislocation of the head fragment and cutting-out are the most common complications of angle stable locking plates in AO11-A3 fractures of the elderly. One possibility to increase the stability of the screw-bone-interface is the cement augmentation of the screw tips. A second is the use of a multiplanar angle stablentramedullary nail that might pr

Bony increased-offset reversed shoulder arthroplasty: minimizing scapular impingement while maximizing glenoid fixation.

BACKGROUND: Scapular notching, prosthetic instability, limited shoulder rotation and loss of shoulder contour are associated with convention … t reverse shoulder arthroplasty. A cylinder of autologous cancellous bone graft, harvested from the humeral head, was placed between the reamed glenoid surface and baseplate. Graft and baseplate fixation was achieved using a lengthened central peg (25 mm) and four screws. Patients underwent clinical, radiographic, and CT assessment at a minimum of 2 years afte …

Humeral hemiarthroplasty with biologic resurfacing of the glenoid for glenohumeral arthritis. Surgical technique.

BACKGROUND: Biologic glenoid resurfacing was developed in 1988 as an alternative to total shoulder arthroplasty in selected (usually younger) patients with primary, posttraumatic, or postreconstructive glenohumeral arthritis. A variety of biologic surfaces, including anterior capsule, autogenous fascia lata, and Achilles tendon allograft, have been combined with a humeral hemiarthroplasty. METHODS: From November 1988 to November 2003, thirty-four patients (thirty-six shoulders) who were managed with biologic glenoid resurfacing and humeral head replacement either with cement (ten shoulders) o…

Posttraumatic Avascular Necrosis After Proximal Femur, Proximal Humerus, Talar Neck, and Scaphoid Fractures.

Posttraumatic avascular necrosis (AVN) is osteonecrosis from vascular disruption, commonly encountered after fractures of the femoral neck, … lapse are frequently symptomatic, and total hip arthroplasty is the most reliable treatment. In the humeral head, certain fracture patterns correlate with avascularity at the time of injury, but most do not go on to develop AVN due to head revascularization. Additionally, newer surgical approaches and improved construct stability appear to lessen the risk of A …

Transhumeral head plasty for large Hill-Sachs lesions.

Large, engaging Hill-Sachs lesions can cause recurrent glenohumeral instability following Bankart repair of torn anterior capsulolabral structures. We offer a novel technique for correcting the posterolateral humeral head defect without significantly altering normal anatomic structures. The glenohumeral joint is exposed via a deltopectoral approach. After the defect geometry is appreciated by direct palpation and visualization, the tip of an anterior cruciate ligament tibial guide is centered in the defect. The drill sleeve is approximated to the anterior lesser tuberosity starting hole, and a…

Arthroscopic Bone Graft Remplissage Using Flexible Bone Allograft for Treatment of Hill-Sachs Lesions in Anterior Shoulder Instability.

Hill-Sachs lesions are defects in the posterior humeral head from anterior glenoid impaction during dislocation. Left untreated, they can contribute to failure after arthroscopic Bankart repair. Arthroscopic remplissage, involving capsulotenodesis of the posterior capsule and infraspinatus tendon, is the most frequently performed approach to address this lesion. In this technical note, we describe a bone graft remplissage that augments the remplissage with the arthroscopic insertion of a flexible bone graft between the capsule and the sutures to fill the Hill-Sachs defect. Once secured, the gr…

The methylene tetrahydrofolate reductase (C677T) mutation as a potential risk factor for avascular necrosis in sickle cell disease.

Avascular necrosis (AVN) of the humeral and femoral heads is a frequent and debilitating complication of sickle cell disease. Some of the risk factors for AVN are alpha-thalassemia and age. Recently, newly discovered thrombophilia mutations have been associated with AVN in patients without sickle cell disease. We studied the frequency of the thermolabile methylene tetrahydrofolate reductase (MTHFR) variant (C677T) in adult sickle cell patients with and without AVN. The frequency of the MTHFR mutation was 35.6% in patients with AVN and 12.9% in those without AVN (p = 0.006). These data suggest

[Analgesic-free self-reduction of acute shoulder dislocation].

Incidence and severity of shoulder lesions are increasing especially in alpine skiing accidents. In case of a shoulder dislocation there is always the question of a safe and atraumatic reduction technique if possible without analgetics or even general anesthesia. We present a new reduction technique with the help of the patient's own body weight under muscular relaxation. Even in the presence of displaced fractures of the greater tuberosity or a depression fracture of the humeral head successful reductions were obtained. After the reduction, the arm is immobilized in a Gilet. The patient is ab…

Transient amnesia following prehospital low-dose ketamine administration.

Low doses of ketamine have been shown to be safe and effective for pain relief. Adverse effects are generally mild and transient. A 69-year-old woman suffered a witnessed ground-level fall without report of head injury, loss of consciousness, or intoxication. She was in severe pain despite 10 mg of intravenous morphine and paramedics provided intravenous ketamine 16 mg (0.19 mg/kg). Upon arrival to the ED, she was alert and oriented. An X-ray demonstrated an acute comminuted nondisplaced right humeral head and neck fracture. Her pain improved after an additional 4 mg of morphine and placement …

Classification of proximal humerus fractures according to pattern recognition is associated with high intraobserver and interobserver agreement.

BACKGROUND: The Mayo-Fundación Jiménez Díaz (FJD) classification for proximal humerus fractures aims to identify specific fracture patterns and apply displacement criteria to each pattern. The classification includes 7 common fracture patterns: isolated fractures of the greater or lesser tuberosity, fractures of the surgical neck, impacted fractures involving head rotation in a varus and posteromedial direction or in valgus, and fractures where the humeral head is dislocated (head dislocation), split (head splitting), or depressed (head impaction). The purpose of this study was to evaluate the…

The treatment of subcapital humerus fracture with dynamic helix wire and the risk of concommitant lesion of the axillary nerve.

Displaced subcapital fractures of the humerus represent a therapeutic challenge for the surgeon, all the more so because of their high incidence among osteoporotic patients. Open surgical repair involves increased risk of avascular necrosis of the humeral head, while percutaneous reduction and fixation often fails as a result of loosening pins. As a possible solution, a minimally invasive technique was developed in which a dynamic titanium wire helix is inserted into the medullary space of the diaphysis of the humerus and then rotated proximally into the head fragment. This new approach for os…

Failed ulnar nerve surgery due to persistent unrecognized snapping triceps from cubitus varus due to a distal humeral malunion in an adult: illustrative case.

BACKGROUND: Snapping triceps is a dynamic condition in which a portion of the medial head of the triceps dislocates over the medial epicondyle during flexion or extension. Pushed by the triceps, the ulnar nerve typically also dislocates over the medial epicondyle, causing neuropathy. Posttraumatic cubitus varus deformities resulting from pediatric supracondylar fractures have been associated with snapping triceps. This is the first case of snapping triceps associated with cubitus varus due to distal humeral malunion, which occurred in an adult. OBSERVATIONS: A 23-year-old man sustained a left

Contralateral C7 Nerve Transfer with Direct Coaptation to Restore Lower Trunk Function After Traumatic Brachial Plexus Avulsion Injuries: Surgical Technique.

INTRODUCTION: We describe a new technique for treating traumatic brachial plexus avulsion injury with a contralateral C7 nerve transfer with direct coaptation that shortens the time to muscle reinnervation. STEP 1 EXPLORE THE INJURED BRACHIAL PLEXUS: Explore the brachial plexus carefully and confirm the nerve-root avulsion injuries from C7 to T1. STEP 2 HARVEST THE CONTRALATERAL C7 NERVE: Dissect the divisions of the contralateral C7 nerve root, divide the nerve at the junction between the divisions and cords, and mobilize it proximally.
