Two cases of retroperitoneal fibrosis with asynchronous bilateral hydronephrosis are reported. One case was managed initially by unilateral ureterolysis. Subsequent contralateral disease was treated with a 6-week course of oral steroids alone with resolution of the bilateral hydronephrosis for 10 years. The other case was treated with unilateral renal exploration. Subsequent contralateral and residual ipsilateral disease was treated similarly with steroids and 7 months later there was resolution of bilateral hydronephrosis.

AIMS: The current study aims to assess the role of ultrasound (US) parameters in explaining the variance of hydronephrosis grades in reno-urinary anomalies, as well as their performance in quantifying the incidence of patients with uropathies.
METHODS: In this study 196 patients were prospectively included. All of them had renal/urinary tract anomalies and were recruited from 5992 US examined hospitalized patients. Computed tomography (CT) after abdominoperineal (AP) resection for rectal carcinoma is a routine procedure for the detection of recurrent tumor and distal metastases. To assess the significance of the ultrasonographic (US) demonstration of grade 1 hydronephrosis, the abdominal or renal sonograms of 370 patients were retrospectively reviewed. In 80 patients, there was a clinical suspicion of renal obstruction. Grade 1 hydronephrosis was observed in 33 of the 80 patients, and obstruction was confirmed in 17 of those 33 patients (51.5%). We reported a case of ureteral polyp in a 12-year-old boy whose chief complaint was asymptomatic gross hematuria. An excretory urogram revealed severe right hydronephrosis with a filling defect of the upper ureter. Our diagnosis was right hydronephrosis due to the ureteral polyp. A partial ureterectomy with an end-to-end anastomosis was performed. A polyp was found in the excised ureter and the histological diagnosis was a fibroepithelial polyp. Six months after the operation, the right hydronephrosis showed remission.

Intermittent hydronephrosis may appear as acute abdominal pain. Between episodes of pain, the patient may be asymptomatic, and the intravenous urogram usually will be normal. The condition is diagnosed from intravenous urograms taken either during an episode of pain or after hydronephrosis has been precipitated by hydration. There are many causes of intermittent hydronephrosis; however, the closed renal pelvis and nondistensible ureteropelvic junction are important factors in nearly all cases. Pyeloplasty is the best treatment and is usually curative.

The incidence of urological complications after aorto-iliac graft surgery was prospectively studied in 49 patients and retrospectively in 35 other patients operated on more than five years before. Acquired hydronephrosis following cystectomy and ileal conduit urinary diversion for bladder cancer is most commonly caused by stricture of the ureter or the ureteroenteric anastomosis. Nevertheless, malignant obstruction due to ureteral tumor recurrence must be ruled out. Neuroendocrine tumors of the ureter are extremely rare and an unlikely cause of hydronephrosis in this setting. We present the first reported case of a patient with a history of bladder cancer and an ileal conduit presenting with hydronephrosis secondary to an obstructing carcinoid tumor of the ureter.
