Total anomalous pulmonary venous drainage.

A case of a Nigerian full-term infant with complete anomalous pulmonary venous drainage into the coronary sinus is described. Presentation was from birth but the cardiac defect was clinically suspected at the age of 51 hours. Diagnosis was confirmed by two-dimensional echocardiography and cardiac catheterization. Despite a high operative mortality associated with this condition, the infant had a remarkably smooth post operative course and an excellent result from his repair on the 5th day of his life.

Caesarean delivery and outcome in very low birthweight infants.

OBJECTIVE: To determine the relationship between mode of delivery, intraventricular haemorrhage (IVH), and mortality in very low birthweight (VLBW) infants. STUDY DESIGN: A historical cohort study of infants admitted to a single level III neonatal intensive care unit during a five-year period. Infants < 1500 g born by caesarean delivery (n = 400) were compared to those born by vaginal delivery (n = 305). RESULTS: After controlling for potential confounding variables including: gestational age, fetal presentation, and multiple birth, caesarean delivery was not associated with a decreased odds

Congenital defects of the gastrointestinal tract and abdominal wall. A three-year review.

One hundred forty-four infants with anomalies of the gastrointestinal tract and abdominal wall were treated at Children's Memorial Hospital between July 1970 and July 1973. The overall mortality, including unoperated infants and those dying from other causes up to six months later, was 17%. Associated defects, particularly of the heart and CNS, were the most common causes for death. Many infants were premature; however, this factor alone did not contribute to mortality. Differences in surgical technique did not affect mortality in patients with esophageal atresia, but one baby with total agang

Outcomes of early-phase ampicillin plus aminoglycosides versus ampicillin plus cefotaxime for preterm infants.

BACKGROUND: When used as early empirical antibiotics for preterm infants, ampicillin + aminoglycosides may reduce mortality more effectively than ampicillin + cefotaxime. We compared in-hospital outcomes between early-phase ampicillin + aminoglycoside and ampicillin + cefotaxime therapy. METHODS: Preterm infants (<35 weeks' gestation) hospitalized between July 2010 and March 2022 were identified from a nationwide Japanese database. Patients received ampicillin + aminoglycoside (n = 33,204) or ampicillin + cefotaxime (n = 12,158) as early empirical therapy. We performed 2:1 propensity score-ma

Hepatic Arteriovenous Malformation in a Preterm Infant With High-Output Cardiac Failure.

Congenital hepatic arteriovenous malformations (HAVM) are rare and serious with a known high mortality rate. They are mostly described in the term population and, if symptomatic, present with early-onset congestive heart failure. To our knowledge, there have been no published cases of an affected very preterm infant. Prenatal diagnosis and preprocedural planning are paramount in these critically ill infants. Strategies for management of HAVM differ depending on the size of both the infant and HAVM as well as severity of symptoms. Management may include supportive pharmacologic therapy, transca

Preterm infant retinal OCT markers of perinatal health and retinopathy of prematurity.

The increasing survival of preterm infants has led to the importance of improving long-term outcomes associated with preterm birth. Antenatal and perinatal insults not only impact mortality, but also long-term disability. While in the intensive care nursery, preterm infants are also exposed to various stressors that lead to long-term cognitive deficits. It is therefore critical to identify early, low-stress, non-invasive biomarkers for preterm infant health. Optical coherence tomography (OCT) is a powerful imaging modality that has recently been adapted to the infant population and provides no

Perinatal echovirus infection: insights from a literature review of 61 cases of serious infection and 16 outbreaks in nurseries.

A review of literature published before June 1985 revealed 61 reported cases of neonatal echovirus infection at a nonmucosal site, including 43 cases (70%) due to echovirus 11. Onset of disease occurred between the third and fifth days of life in 63% of cases, indicating that most infections are acquired in the immediate perinatal period rather than in utero. Mortality was higher in infants with severe hepatitis (83%) than in infants with infection of the central nervous system (19%). Acute illness occurred within one week before delivery in 68% of the mothers of nonnosocomially infected infan

Differentiating cause-of-death terminology for deaths coded as sudden infant death syndrome, accidental suffocation, and unknown cause: an investigation using US death certificates, 2003-2004.

We compared written text on infant death certificates for deaths coded as sudden infant death syndrome (R95), unknown cause (R99), and accidental suffocation (W75). Using US mortality files supplemented with the death certifiers' written text for all infant deaths with International Classification of Diseases (ICD)-10 assigned codes R95, R99, and W75, we formed cause-of-death subcategories from common themes identified from the written text. Among all infant deaths in 2003-2004, the underlying cause of death was listed as R99 for 2128 deaths, R95 for 4408 deaths, and W75 for 931 deaths. Among

The Impact of Hospital Delivery Volumes of Newborns Born Very Preterm on Mortality and Morbidity.

OBJECTIVE: To examine if the annual patient volume of infants born very preterm (VPT, gestational age <32 weeks) at a hospital is associated with neonatal mortality and morbidity. STUDY DESIGN: We performed an observational, secondary data analysis using a 20-year panel of birth certificates linked to hospital discharge abstracts, including transfers in California, Michigan, Missouri, Oregon, Pennsylvania, and South Carolina from 1996 through 2015. The study included all in-hospital VPT deliveries (n = 208 261). Study outcomes were in-hospital mortality or serious morbidity (intraventricular

Risk of mortality subsequent to umbilical cord infection among newborns of southern Nepal: cord infection and mortality.

BACKGROUND: Neonatal sepsis may stem from local umbilical cord infections. Signs of cord infection are common in low-resource settings, yet the risk of mortality subsequent to these signs has not been quantified in either developed or developing countries. We compared the risk of mortality between infants with and without signs of umbilical cord infection during a community-based trial of chlorhexidine interventions in southern Nepal. METHODS: Newborns were evaluated for signs of umbilical cord infection (pus, redness, swelling). A nested case-control approach was used to estimate the odds of

A verbal and social autopsy study to improve estimates of mortality determinants in Pakistan: study design and methods.

BACKGROUND: Reducing newborn, child, adolescent, and maternal mortality is a global priority, yet Pakistan continues to bear a high burden of preventable deaths. Routine civil registration does not capture causes or social determinants for most of these deaths. The verbal and social autopsy (VASA) methodology fills this gap by combining symptom-based identification of the cause of death with structured assessment of social and care-seeking factors. Here we describe the methodological framework of a national VASA study conducted using the Pakistan National Nutrition Survey 2018 sampling frame,

Why do maternal and newborn deaths continue to occur?

According to estimates in 2015, there were 303,000 maternal deaths, 2.7 million newborn deaths and 2.6 million stillbirths. A wide range of factors, from health system dynamics to social determinants of health and underlying health conditions, contribute to this outcome. The highest mortality risk for mothers and their babies is on the day of birth, and most of these deaths are preventable. The largest burden of deaths occurs in low-income countries, particularly in sub-Saharan Africa and South Asia, due to their young population and high fertility. Substantial reductions in maternal and newbo

Registered deaths from the vital statistics registration system of Lagos City, a system that was judged to be 60% complete, were analysed fo … ountries, a high proportion of the deaths in Lagos City being those of children aged under 5 years. Infant mortality is dominated by perinatal causes which constituted a huge 38.4% of deaths of infants under 1 year, the other important causes being dysentery and diarrhoea, pneumonia and tetanus. Among adults, death from motor vehicles accidents is the most importa …

Survival Benefit of Empirical Therapy for Staphylococcus aureus Bloodstream Infections in Infants.

BACKGROUND: The impact of early adequate empirical antibiotic therapy on outcomes of infants in the neonatal intensive care unit (NICU) who develop Staphylococcus aureus bloodstream infections (BSI) is unknown. METHODS: Infants with S. aureus BSI discharged in 1997-2012 from 348 NICUs managed by the Pediatrix Medical Group were identified. Early adequate empirical antibiotic therapy was defined as exposure to ≥1 antibiotic with anti-staphylococcal activity on the day the first positive blood culture was obtained. All other cases were defined as inadequate empirical antibiotic therapy. We eval

Traditional birth attendant training for improving health behaviours and pregnancy outcomes.

BACKGROUND: Between the 1970s and 1990s, the World Health Organization promoted traditional birth attendant (TBA) training as one strategy to reduce maternal and neonatal mortality. To date, evidence in support of TBA training is limited but promising for some mortality outcomes. OBJECTIVES: To assess the effects of TBA training on health behaviours and pregnancy outcomes. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (18 June 2012), citation alerts from our work and reference lists of studies identified in the search. [Treatment of PPHN with iNO in an Infant with Trombophilia and Thrombus in PDA. Clinical Case.].

PPHN is common in 1-2/1000 newborn infants. The morbidity and mortality accompanying the disease are extremely high. The treatment is frequently ineffective. The therapy with iNO is the fir l strategy based therapy of PPHN. We present the clinical case of an infant with congenital pneumonia and secondary PPHN treated with iNO. During the clinical observation of the infant congenital trombophilia was diagnosed, due to the presence of a thrombus in PDA. The outcome was favorable.

Maternal Anti-Dengue IgG Fucosylation Predicts Susceptibility to Dengue Disease in Infants.

Infant mortality from dengue disease is a devastating global health burden that could be minimized with the ability to identify susceptibility for severe disease prior to infection. Although most primary infant dengue infections are asymptomatic, maternally derived anti-dengue immunoglobulin G (IgGs) present during infection can trigger progression to severe disease through antibody-dependent enhancement mechanisms. Importantly, specific characteristics of maternal IgGs that herald progression to severe infant dengue are unknown. Here, we define ≥10% afucosylation of maternal anti-dengue IgGs …

Risk factors associated with acute kidney injury in extremely low birth weight (ELBW) infants.

The aim of this study was to determine the incidence, risk factors, and outcome of acute kidney injury (AKI) in extremely low birth weight (ELBW) infants. In a case-control study, medical records of all ELBW infants who were admitted to our Neonatal Intensive Care Unit (NICU) between 1 January 2000 and 31 January 2008 were reviewed. During the study period, 12.5% (59/472) of all ELBW infants developed AKI. Forty-six infants with available medical records were matched to 46 controls. The mean gestational age and birth weight of infants with AKI and their controls were 24.7 ± 1.8 vs. 24.9 ± 1.9

Lower perinatal mortality in preterm born twins than in singletons: a nationwide study from The Netherlands.

BACKGROUND: Twin pregnancies are at increased risk for perinatal morbidity and death because of many factors that include a high incidence of preterm delivery. Compared with singleton pregnancies, overall perinatal risk of death is higher in twin pregnancies; however, for the preterm period, the perinatal mortality rate has been reported to be lower in twins. OBJECTIVE: The purpose of this study was to compare perinatal mortality rates in relation to gestational age at birth between singleton and twin pregnancies, taking into account socioeconomic status, fetal sex, and parity.
