Between February 1985 and January 1987, sixty cases of pulmonary infection in immunocompromised host were seen at the Department of Medicine, Ramathibodi hospital. The underlying causes were: thirty-four cases with hematologic malignancies, nine with autoimmune diseases, seven with aplastic anemia, three with non-hematologic malignancies, and seven following corticosteroid therapy. Infectious agents were identified in thirty-two cases; nineteen bacterial, three fungal, four nocardial, three strongyloidiasis and three tuberculosis. In nineteen cases, the infectious agents were classified as "pr

Hospital-acquired infection with strains of methicillin-resistant Staphylococcus aureus (MRSA) have considerably increased in recent years. In addition to being resistant to methicillin, these strains are resistant to practically all forms beta-lactams, aminoglycosides and many other antibiotics. There appears no cost-effective control and preventive measures for this common but also potentially life-threatening disease. Although not clearly presented, the overall cost for the treatment of patients infected with MRSA should be enormous. Can laboratory medicine (or clinical microbiology) contri

Neutropenia is the most important predisposing factor for bacterial infection in immunocompromised hosts. It is mostly associated with mucosal damage as a consequence of chemotherapy. Nearly 50 p. 100 of infections occurring in neutropenic patients are acquired in the hospital. Colonization precedes infection in 80 p. 100 of the cases. The sources of colonization are ubiquitous and include water, air, food and medical personnel while gut colonization is more often responsible for infection than in normal patients. Fungal infection also emerges as an increasing problem in haematology. The use o…

In this article the three different manifestations of pulmonary aspergillosis are compared with each other: bronchopulmonary aspergillosis, aspergilloma and invasive pulmonary aspergillosis, and their different clinical and radiological manifestations. In particular, invasive pulmonary aspergillosis--being a severe complication in immunosuppressed patients whose defenses are weak--often presents diagnostic problems due to its initially mostly unspecific clinical and radiological signs. The late signs of a pulmonary aspergillus infection, such as formation of a cavern or the manifestation of th

PURPOSE: Bacteremia in the elderly is associated with a different clinical course and a higher mortality rate when compared with that in younger age groups. In order to examine these issues in the aged, we reviewed the clinical course and factors involved in the outcome of 100 episodes of bloodstream infections in patients over 65 years of age.
PATIENTS AND METHODS: The hospital records of all patients over 65 years of age at The Mount Sinai Hospital with a positive blood culture result during the period October 1984 to October 1986 were reviewed. Place of residence before hospital admission,…

Infection is a common problem in elderly patients, the most common infections being those in the urinary tract and the respiratory tract. In geriatric practice, infection is most frequently seen in combination with many other problems. The geriatric presentation is described in which patients may present with an increased dependency upon care providers due to the development of fresh problems like falls, immobility, confusion, incontinence of urine, and fecal impaction with incontinence. It is important to recognize this presentation and not to miss the diagnosis. It is well recognized that pn…

Cryptococcus neoformans, Histoplasma capsulatum, and Coccidioides immitis are the three fungi that regularly cause disseminated, life-threatening disease in patients with AIDS. Cryptococcosis is the fourth most common opportunistic infection in patients with AIDS and results in meningitis or pneumonia or both, in most cases. In addition, there have been unusual focal infections described and even unexplained fever alone. In any patient at risk for HIV infection, routine screening tests should include serum cryptococcal antigens, and this test should be repeated whenever new symptoms or signs a…

The diagnosis of pulmonary fungal infections is reviewed. An overview of the major endemic and opportunistic fungal infections is followed by a brief discussion of the available diagnostic tools. These include: (1) direct microscopic examination of secretions or body fluids; (2) histopathologic examination of tissue biopsies; (3) cultures of secretions, body fluids, or ground-up tissue; and (4) immunological tests ("fungal serologies"). In the final section, a current diagnostic approach is given that identifies the diagnostic tests more likely to be helpful for each infection.

Bacterial infections cause a substantial burden of disease in the HIV-infected population. Like the opportunistic infections associated with AIDS, they are often usually severe. Unlike many AIDS-related opportunistic infections, however, many can be eradicated by therapy with the appropriate antibiotic, although some, like Salmonella sepsis, require prolonged suppressive therapy to prevent recurrences. Most importantly, the possibility of a bacterial infection should be kept in mind when evaluating any acutely ill HIV-infected patient, as therapy of bacterial disease is often curative while un…

In a systematic screening programme for neonatal infections involving 16,008 births, Haemophilus influenzae was isolated in 14 mother-infant couples (0.8 to 1,000 births). Comparisons with other series published in the U.S.A. showed similar circumstances of occurrence and initial clinical manifestations. However, the course of the infection was different, since there was no septicaemia or meningitis in our series. This raises the question of whether the strains responsible for the disease in the U.S.A. have a particular aggressiveness that is unidentifiable by serotype or biotype. In view of t…

Diabetic foot infections are frequently much worse than the initial clinical appearance would indicate. Altered immune response, peripheral vascular disease, and neuropathy are key factors in the production of infection. Treatment must be based upon careful bacteriologic analysis and individualized antibiotic therapy. Surgical debridement, when indicated, is the mainstay of treatment. Partial foot amputation and salvage are always preferred over higher amputation when possible.
