A case of an anomalous duodenum, pancreas and biliary tract, with gallstone perforation.

A case of an elongated, anomalous duodenum with intestinal rotation, annular pancreas and disease of the biliary tract with acute jaundice caused by gallstone perforation is presented. The ERCP (endoscopic retrograde cholangiopancreatography) was of great value in the preoperative investigations. Surgical relief of the obstruction of the biliary duct made the patient free of symptoms.

[An unusual cause of jaundice].

Hepatic impairment is common during hyperthyroidism. It is most often asymptomatic. Hyperthyroidism revealed by jaundice has been rarely described in the literature. We here report the case of a 52-year old patient in Dakar (Senegal) presenting with jaundice associated with pruritus. Laboratory tests showed elevated alanine aminotransferases (1.1 N), aspartate aminotransferase(1.5 N), alkaline phosphatases (3 N), gamma glutamyl transferases (1.3 N) and bilirubinemia (22 N). Abdominal ultrasound was normal. A toxic or drug-related cause, bile duct obstruction, viral or autoimmune hepatitis as w

An impressive choledochal cyst and its surgical resection.

INTRODUCTION: Choledochal cysts are rare congenital dilations of the biliary tree that can present with non-specific symptoms such as abdominal pain, jaundice, cholelithiasis and pancreatitis. Although most commonly identified in children, they can be found in the adult population. However, because of the non-specific symptoms, this diagnosis may be difficult to make in the adult. A physician therefore must keep this diagnosis within their differential, as it may arise in an unexpected patient population who may present with a convoluted work up. CASE PRESENTATION: In this report, we present

The current status of preoperative biliary drainage for patients who receive pancreaticoduodenectomy for periampullary carcinoma: a comprehensive review.

BACKGROUND: Surgery on patients with malignant obstructive jaundice carries increased risks of postoperative morbidity and mortality. Preoperative biliary drainage has been developed to reduce this procedure-related risks, but its role in patients who are going to receive pancreaticoduodenectomy for periampullary carcinoma is still controversial. METHODS: This article aimed at reviewing the current status of preoperative biliary drainage for patients with peri-ampullary tumors who were candidates for pancreaticoduodenectomy. A MEDLINE and PubMed database search from 1980 to 2013 was performed

Hepatic fascioliasis presenting with bile duct obstruction: a case report.

Fascioliasis is a zoonotic infection caused by a liver trematode: fasciola hepatica; which commonly affects cattle and sheep, humans are accidental hosts. Several cases have been reported in the literature worldwide with a large geographical distribution. We present a case of bile duct obstruction due to a hepatic fascioliasis, successfully treated with both a combined surgical and medical approaches. A high index of suspicion should be kept in mind for all cases of obstructive jaundice, especially in areas in which human fascioliasis infection is repeatedly reported.

Percutaneous management of bile-plug syndrome: a case report.

Bile-plug syndrome is defined as an obstruction of the common bile duct by bile sludge in full-term infants. It is a correctable cause of obstructive jaundice in infants and is generally treated surgically. Here, we present a case of a 5-month-old infant with bile-plug syndrome, which was treated by percutaneous biliary interventions. To the best of our knowledge, percutaneous treatment of bile-plug syndrome is reported for the first time in an infant. This minimal invasive treatment modality may be a useful alternative to surgery in infants with bile-plug syndrome.

Novel approach to iatrogenic bile peritonitis.

Bile peritonitis after injury to the biliary tree is a serious complication that requires exploratory laparotomy. Our patient had an obstructing ampullary carcinoma, and generalized bile peritonitis developed from attempted percutaneous transhepatic cholangiography. The patient's condition was managed by peritoneal lavage and endoscopic transampullary stenting, with immediate relief of pain and toxicity. Exploratory laparotomy was avoided, and an eventual pylorus-sparing Whipple resection was the definitive treatment. We believe this to be the first report of successful nonoperative treatment

Portal hypertension as an early manifestation of carcinoma of the pancreas.

Massive hematemesis followed by 48 hours the onset of obstructive jaundice in a previously asymptomatic 38-year-old male patient. Splenomegaly was the only abnormal physical finding. Esophagoscopy visualized bleeding varices. The liver scan was normal. Dilated intra- and extrahepatic bile ducts and a possible retroperitoneal mass were demonstrated by ultrasonography. Laparotomy disclosed the presence of a carcinoma of the head and body of the pancreas that obstructed the common bile duct and the portal vein. This case report should draw attention to the vast spectrum of rare presentations of p

[Endoscopic papillotomy (EPT) (author's transl)].

From August 1974 to December 1977 a total of 155 endoscopic papillotomies were performed. Endoscopic papillotomy (EPT) has proven to be an acceptable alternative to surgical reintervention at the biliary tree especially in aged and cholecystectomized patients where obstructive jaundice is caused by a concrement. The rate of all complications (9%) is comparatively low. In our own series 3 patients died after endoscopic papillotomy. EPT may be justified in jaundiced patients without previous cholecystectomy when there is a high risk for laparotomy. Preoperative drainage of massive hyperbilirubin

An ampullary adenoma presenting with jaundice caused by duodenal intussusception: a case report.

BACKGROUND: Ampullary adenomas are premalignant lesions. However, biliary obstruction causing jaundice is rare. Duodenal intussusception secondary to an ampullary adenoma rarely occurs because of the fixed position of the duodenum in the retroperitoneum. Herein, we have described a rare case of ampullary adenoma with jaundice caused by duodenal intussusception. CASE PRESENTATION: A 40-year-old woman presenting with vomiting and yellowish discoloration of the skin was admitted to another hospital. The patient had experienced recurrent epigastric pain and vomiting for the past 18 months. Blood

Computer aided diagnosis of jaundice. A comparison of two data bases.

A computer system for probabilistic diagnosis of jaundice was tested on a patient sample from a geographical area different from that for which it was first constructed. 144 consecutive patients with jaundice seen in two Stockholm hospitals were interviewed and examined to record a total of 82 indicants from history, demographic details, physical findings and laboratory tests. Data were compared with those of 319 jaundiced patients previously interviewed and examined at different London hospitals. It was found that disease incidences were different in the two patient samples. There were more p

Acute severe cholecystitis with empyema presenting as a gallbladder mass, jaundice and Mirizzi Syndrome: A case report.

INTRODUCTION: Acute severe cholecystitis with empyema presenting as a gallbladder mass, jaundice and Mirizzi Syndrome (MS) is a complex surgical problem both diagnostically and in terms of management as it mimics both xanthogranulomatous cholecystitis (XGC) and gallbladder carcinoma. PRESENTATION OF CASE: A 48-year-old gentleman was referred to us with biliary colic and weight loss with ultrasound findings of gallstones. At subsequent follow-up he became deeply jaundiced with deranged liver function and a CT showing a gallbladder mass and dilated biliary tree. Follow-up MRCP suggested XGC and

Biliary obstruction due to ruptured hydatid cyst.

Intrabiliary rupture of a hydatid cyst of the liver is a rare complication. It can give rise to jaundice due to cholangitis, and common bile duct obstruction caused by hydatid membranes and daughter cysts. A case of intrabiliary ruptured hydatid cyst presenting as abscess of the liver is reported with a brief review of relevant literature.

Bronchobiliary fistulas in adults.

Reported herein are 16 cases of nontraumatic bronchobiliary fistula in adults observed over 32 years. The fistulas were due to either biliary obstruction or infectious diseases involving the liver. All patients had biloptysis and other respiratory symptoms, and most had cholangitis, jaundice, and/or sepsis. The diagnosis was established by fistulography or cholangiography. All patients except one were treated surgically by relief of biliary obstruction and drainage of intra-abdominal abscesses. All 15 surgically treated patients had a successful outcome. Rapid diagnosis and early treatment of

Hepatobiliary scanning in cholecystitis.

To validate the use of 99mTc-PIPIDA in the diagnosis of biliary tract disease, 117 patients with clinical symptoms of acute cholecystitis were prospectively scanned as an initial screening test. The accuracy of the test was evaluated on the basis of surgical pathology in 59 patients. Three groups were defined: cystic duct obstruction, common duct obstruction, and normal scan. The diagnostic accuracy of the test for acute cholecystitis was 97%. Complete common duct obstruction was confirmed at surgery in 12 patients (100%). A normal scan helped to rule out acute cholecystitis. Jaundice did not

Autoimmune hepatitis following long-term denosumab therapy in a patient with Hashimoto thyroiditis.

Denosumab, a receptor activator of nuclear factor-κB ligand (RANKL) inhibitor, is widely used to treat osteoporosis. Its role as a trigger for autoimmune hepatitis (AIH) remains uncertain. We report a 68-year-old woman with osteoporosis and Hashimoto thyroiditis who developed jaundice and severe hepatocellular liver injury after 3 years of denosumab therapy. Alternative viral, obstructive, and metabolic causes were excluded. Liver biopsy demonstrated interface hepatitis with a lymphoplasmacytic portal infiltrate consistent with AIH, and autoimmune serologies were positive. Hemophagocytic lymph

Double trouble: synchronous extrahepatic cholangiocarcinoma and gallbladder cancer in a Caucasian woman with no pancreaticobiliary maljunction.

Synchronous malignancies of the bile duct and the gallbladder are rare. These cases are often associated with pancreaticobiliary maljunction which is characterized by a long common shared pancreatobiliary channel leading to the Sphincter of Oddi. This predisposes the biliary epithelium to pancreatic enzyme reflux and makes the development of neoplasia more likely. We describe the case of a 64-year-old Caucasian female who presented with new jaundice and severe cholecystitis secondary to an impacted gallstone which was seen on ultrasound. Magnetic resonance cholangiopancreatography was organize

EBV Infection Unmasking a Choledochal Cyst in an Infant.

Hepatic involvement is common in acute Epstein-Barr virus (EBV) infection in children. It usually manifests as a transitory elevation of transaminases in up to 80% to 90% of patients, and they normalize by 2 to 6 weeks. A cholestatic pattern with elevated gamma-glutamyl transferase (γGT) and alkaline phosphatase (ALP) is common, in up to 60% in young adults. However, jaundice is very rare occurring in only 5% of pediatric patients. We report here an 8-month-old female with EBV infection who developed obstructive jaundice 2 weeks after the initial infection. Radiologic investigations were compa

[An autopsy case report of adenosquamous cell carcinoma of the duodenal papilla with multiple liver and lung metastases].

A 63-year-old man was admitted to Saitama-Kyodo Hospital for the management of obstructive jaundice. Gastrointestinal endoscopy revealed a tumor of the duodenal papilla, and tumor biopsy suggested adenosquamous cell carcinoma. Computed tomography revealed multiple tumors in both the liver and lung, and these were diagnosed as metastases using bronchoscopy-guided lung biopsy and ultrasound-guided liver biopsy, respectively. The patient was treated with gemcitabine therapy after successful management of the jaundice by percutaneous transhepatic cholangiodrainage. However, he died three months af
