[Patella dislocation in athletes].

Most dislocations of the patella occur during sports activities. The entities instability, maltracking and loss of tracking should be defined by patient history, clinical examination and radiological evaluation including magnetic resonance imaging (MRI). Based on these criteria a new classification of patella dislocations (5 types) was established which allows a standardized treatment algorithm. Type 1 is a simple (traumatic) dislocation without maltracking and without instability. Type 2 has a high redislocation risk (defined as instability) without maltracking. An isolated stabilizing surgic

Medial patellofemoral ligament injury. Location-based rate of recurrent patellar dislocation after non-operative treatment.

OBJECTIVES: The role of the medial patellofemoral ligament (MPFL) as a patellofemoral joint stabilizing structure is undisputable. After traumatic patellar dislocation, MPFL injury, together with bone edema in the medial patellar facet and lateral femoral condyle, is a pathognomonic finding in magnetic resonance imaging (MRI). MPFL injury in the femoral insertion has been reported to most likely predict recurrent dislocations. The objective of this study was to detect if any MPFL injury location predicts the earliest onset of the patellar re-dislocation. METHODS: In total, 64 eligible patient

Groove-deepening trochleoplasty reduces lateral patellar maltracking and increases patellofemoral contact pressures: Dynamic simulation.

Groove-deepening trochleoplasty is performed to restore patellar stability by increasing the lateral constraint applied to the patella by the trochlear groove. Multibody dynamic simulation of knee function was used to characterize the influence of groove-deepening trochleoplasty on patellar tracking and patellofemoral contact pressures. Computational models were created to represent seven knees with trochlear dysplasia, indicated by a flat trochlear groove and supratrochlear spur. The models were manipulated to remove the spur and deepen the trochlear groove to represent the average shape foll

A pelvic support osteotomy and femoral lengthening with monolateral fixator.

UNLABELLED: Pelvic support osteotomies for chronically dislocated hips improve stability but result in limb-length discrepancy and valgus deformity of the knee. We prospectively evaluated and followed up 16 patients (14 females, two males) to ascertain whether a monolateral external fixator could achieve a pelvic support osteotomy and eliminate the limb-length discrepancy without disturbing knee motion. The patients had a mean age of 25.3 years at the time of surgery. The Harris hip score, knee range of motion, and Trendelenburg sign were evaluated preoperatively and at followup. The average t

The Miller-Galante knee prosthesis for the treatment of osteoarthrosis. A comparison of the results of partial fixation with cement and fixation without any cement.

In a prospective, non-randomized study of 344 patients who had 392 primary total knee replacements with a Miller-Galante I prosthesis for the treatment of osteoarthrosis, the results of partial fixation with cement (insertion of the tibial and patellar components with cement and of the femoral component without cement) were compared with those of fixation without any cement. Of the 392 knees, 183 (163 patients) had fixation without cement (Group I) and 209 (181 patients), with and without cement (Group II). The average duration of follow-up was three years (range, two to five years). Nine pati

A modification of the Bereiter trochleoplasty: indications, technique and outcomes.

INTRODUCTION: The Bereiter technique is one recognised method of trochleoplasty that was described using a lateral para-patellar approach. We present our surgical technique and outcomes of this procedure using a medial para-patellar approach allowing for accurate soft tissue balancing of the patella. METHODS: In total, 27 consecutive patients underwent a Bereiter trochleoplasty using a medial approach. Patients completed pre- and post-operative Kujala scores. All patients' medical records and imaging were reviewed to identify pre-operative indications, complications and re-dislocations. RESU

[Reconstruction of quadriceps femoris muscle function with muscle transfer].

Femoral nerve palsy, mostly of iatrogen cause, leads to paresis of quadriceps muscle with complete loss of knee extension. Therapeutical options include neurolysis, nerve reconstruction or functional muscle transplantations. Another concept is the transfer of hamstring muscles as described in post polio surgery. We describe our experience of biceps femoris and semitendinosus muscle transfer for reconstruction of knee extension. From 2003 to 2007 seven patients (mean age 43) with complete loss of knee extension after femoral nerve lesion were treated. Nerve palsy was caused by direct lesion, tr

A novel frameshift TBX4 variant in a family with ischio-coxo-podo-patellar syndrome and variable severity.

BACKGROUND: Congenital anomalies of the knee are a spectrum of rare disorders with wide clinical and genetic variability, which are mainly due to the complex processes underlying knee development. Despite progresses in understanding pathomechanisms and associated genes, many patients remain undiagnosed. OBJECTIVE: To uncover the genetic bases of a congenital patellar dislocation affecting multiple family members with variable severity. METHODS: We performed ES in the proband and his father, both showing bilateral patellar dislocation, his sister with a milder similar condition, and his unaff
