Although nonpuerperal galactorrhea in youth may be a sign of pituitary prolactinoma, the etiology may be benign and extensive neurologic or endocrinologic evaluation and treatment may be unnecessary. The case history of a 16 years old boy with delayed puberty and the sudden onset of galactorrhea is presented. Prolactin analyses and X-ray studies of the sella turcica lead to the diagnosis of a pituitary tumor. A prolactin producing microadenoma was selectively excised via the transsphenoidal route. Cerebrospinal fluid enzyme levels of creatine kinase (CK), lactate dehydrogenase (LDH), glutamate oxaloacetate transaminase (GOT) and angiotensin converting enzyme (ACE) were studied in 40 acute stroke patients comprising 20 lacunar strokes and 20 cortical strokes. A marked elevation of at least one of the enzymes CK, GOT or LDH was seen in 80% of cases of cortical strokes. No elevation was seen in lacunar stroke with CK, GOT or ACE and only a slight elevation with LDH. Amenorrhea and galactorrhea developed in a female patient with tuberous sclerosis. There was no evidence of a pituitary tumor; she had an abnormal EEG, and computed tomographic scan showed multiple intracerebral calcifications but no lesions in the pituitary gland or hypothalamus. She had fixed hyperprolactinemia that was unresponsive to protirelin, chlorpromazine, levodopa, bromocriptine mesylate, or estrogen. The circulating prolactin may be of pituitary origin or may possibly be secreted ectopically by a hamartoma.

In migraine patients a considerable metabolic disturbance characterised by a decrease in bicarbonate and an increase in lactate was demonstrated during the migrainous state. Measurements of the cerebral circulation were taken by means of radioactive isotope and showed a distinct decrease in blood supply to the entire brain during the migraine attack. Following forced hyperventilation during the attack-free interval a pathological reactivity with decreased blood flow was found in the hemisphere which had been indicated as the usual site of the migrainous pain.
