Support for Older People With Vision Impairment: A Rapid Realist Review.

BACKGROUND: Vision impairment among older adults is a significant public health issue impacting their physical and mental health and overall quality of life. However, access and navigation of support services are inconsistent and lacking. Recognising the need to gain insight to improve access to services in this population, this review aimed to understand how, why, for whom, to what extent and in what context the provisions for care and support for older people with vision impairment worked. METHODS: A rapid Realist review was undertaken. Six databases (Cinahl, Cochrane Library, PubMed, Socia

Artificial Placenta and Partial Ectogenesis for Extremely Preterm Infants: A Narrative Review.

Extremely preterm births at the edge of viability are associated with high mortality and significant long-term morbidity. This is largely due to the sudden and involuntary shift from fetal placental support to neonatal lung breathing, accompanied by systemic organ injury. Between 22 and 24 weeks of gestation, the fetal lungs, brain, and metabolic regulatory systems are not biologically prepared for extrauterine life. As a result, extremely preterm infants are highly vulnerable to ventilator-induced lung injury, oxygen toxicity, hemodynamic instability, and inflammatory cascades that contribute

User-Centered Design of an Electronic COPD Patient Questionnaire for Clinical Decision Support.

PURPOSE: Effective COPD management depends on structured symptom and risk assessment, individualized therapy, and action plan provision, yet these are inconsistently delivered in routine care. Electronic health record (EHR)-integrated clinical decision support systems (CDSSs) can help close these gaps but require accurate, structured patient data to drive their recommendations. Digital pre-visit questionnaires are increasingly used to collect such data, but patient preferences for their content and design remain poorly understood, particularly in COPD. PATIENTS AND METHODS: We used a user-cen

Predictors of depression among caregivers of children with malnutrition in Gedeo zone public hospitals, Southern Ethiopia: case-control study.

BACKGROUND: Depression is a leading cause of disability worldwide, significantly impacting global health, particularly for women. It accounts for 4.3% of the total disability-adjusted life years (DALYs), making it a major contributor to global disability. The high rate of depression in developing nations, the caregiving responsibilities of caregivers, their susceptibility to depression, and maternal mental health in these nations may significantly impact childhood development. Therefore, the study aimed to identify the predictors of depression among caregivers of children with malnutrition in

Exploring the lived experiences of parents caring for infants with gastroschisis in Rwanda: The untold story.

Pediatric surgery is a crucial pillar of health equity but is often not prioritized in the global health agenda, especially in low-and middle-income countries. Gastroschisis (GS) is a type of structural congenital anomaly that can be treated through surgical interventions. In Rwanda, neonatal surgical care is only available in one hospital. The experience of parents of children born with gastroschisis has not been previously studied in Rwanda. The objective of this study was to explore the lived experiences of parents of children diagnosed with GS in Rwanda. A qualitative study using a semi-st

Preventing Child Sexual Abuse and the Use of Child Sexual Abuse Materials: Following up on the German Prevention Project Dunkelfeld.

Sexual interest in minors (i.e., pedophilia and hebephilia) is considered a risk factor for Child Sexual Abuse (CSA) and the use of Child Sexual Abuse Material (CSAM). This study examined the long-term development of CSA, CSAM use, associated cognitions, and quality of life among self-referred, help-seeking individuals diagnosed with pedophilic or hebephilic disorder (PHD) outside the judicial system. Of the 110 eligible men who had undergone therapy for PHD due to psychological distress or risk of offending, 56 were available for follow-up assessment 1-11 years after treatment. Behavioral man

A RARE CAUSE OF NEONATAL SALT WASTING: CLINICAL MANAGEMENT OF A CASE WITH PSEUDOHYPOALDOSTERONISM DETECTED WITH A NOVEL HOMOZYGOUS VARIANT IN THE SCNN1A GENE.

INTRODUCTION: Pseudohypoaldosteronism (PHA) is a rare condition that can lead to life-threatening hyperkalemia, cardiac arrest and death if not rapidly recognised and treated. Systemic PHA results from the inactivation of variants in genes encoding subunits of the epithelial sodium channel (ENaC). Frequent dose revision is required in oral replacement therapy in patients with systemic PHA. This condition requires a lifelong close follow-up and treatment process in patients. CASE DESCRIPTION: In this study, we present the clinical follow-up of a newborn diagnosed with PHA who presented at 9 da

Patient Navigation for the Care Needs of Cancer Survivors: A Scoping Review.

BACKGROUND: Cancer care increasingly emphasizes holistic, patient-centered support across the disease trajectory; however, many health systems lack sufficient resources to address patients' physical, psychosocial, and logistical care needs, particularly in under-resourced settings. Patient navigation, delivered by trained non-clinician, peer, or community-based navigators, has demonstrated effectiveness in improving cancer screening and treatment initiation, yet its role in addressing the ongoing health and related care needs of people living with cancer remains less well characterized. Lunar magnetism impairs wheat seedling photosynthesis: A simulated environment study.

Plants evolved under Earth's stable geomagnetic field (GMF), a condition sharply contrasting with the near-absence of a global magnetic field on the Moon. However, the effects of this stark magnetic disparity on fundamental plant processes like photosynthesis remain underexplored, particularly in the context of future lunar agriculture. This study rigorously investigated the physiological and biochemical mechanisms underpinning the photosynthetic response of wheat seedlings - a staple crop selected for its centrality in closed-loop life support - to a simulated lunar weak magnetic field (WMF,

Endocannabinoid signals in the control of emotion.

The appropriate control of emotional responses evoked by environmental stimuli is an important innate mechanism for ensuring quality of life and even for survival. Inappropriate responses and decreased abilities to adjust to changed environmental situations can lead to psychiatric disorders, such as posttraumatic stress disorders, phobia and depression. Endocannabinoid signalling has emerged as one of the regulatory systems of the brain supporting appropriate emotional responses. As various components of the endocannabinoid system have become therapeutic targets, understanding the endocannabin

Study of the cement implantation syndrome: A review.

Bone cement implantation syndrome (BCIS) is a critical and potentially life-threatening condition that manifests during implantation. Characterized by a constellation of symptoms, including hypoxemia, hypotension, cardiac arrhythmias, elevated pulmonary vascular resistance, and occasionally cardiac arrest, BCIS typically ensues shortly after cement introduction, albeit with rare instances of delayed onset. Primarily attributed to the exothermic reaction of bone cement implantation, this syndrome is caused by local tissue damage, histamine and prostaglandin release, and microemboli formation, u

Why and When Are Evidence-Based Interventions Adopted in Paediatric Supportive Care? A Qualitative Exploration of the Determinants of Photobiomodulation Implementation.

BACKGROUND: Oral mucositis is a common and debilitating side effect of childhood cancer and stem cell transplant treatments. It affects the quality of life of children and young people (CYP) and places a strain on services. Photobiomodulation is recommended for oral mucositis prevention in international guidance but is poorly implemented in UK Principal Treatment Centres (PTCs). Our study explored the key barriers and facilitators to implementing photobiomodulation into standard care. METHODS: CYP, their parents, and healthcare professionals (HCPs) were purposively sampled for diversity in th

The Challenges of Afghanistan and Iraq Veterans' Transition from Military to Civilian Life and Approaches to Reconnection.

Afghanistan and Iraq veterans experienced traumas during deployment, and disrupted connections with friends and family. In this context, it is critical to understand the nature of veterans' transition to civilian life, the challenges navigated, and approaches to reconnection. We investigated these issues in a qualitative study, framed by homecoming theory, that comprised in-depth interviews with 24 veterans. Using an inductive thematic analysis approach, we developed three overarching themes. Military as family explored how many veterans experienced the military environment as a "family" that

Beyond the Bedside: Addressing Structural Gaps in the Care of Undocumented, Seriously Ill Patients.

Undocumented patients face structural constraints to accessing life-sustaining treatments in the subacute setting due to gaps in health care coverage. We present the case of a 38-year-old Spanish-speaking woman with short bowel syndrome, dependent on prolonged parenteral nutrition after critical illness, who faced additional challenges as an undocumented and uninsured patient. Despite medical stabilization and a consistent desire to pursue life-prolonging care, discharge planning was complicated by a health care system not designed to support individuals without legal status or insurance.
