Peripheral ameloblastoma with cervical lymph node metastasis: a case report and review of the literature.

This report describes a case of peripheral ameloblastoma of the mandible with metastasis to a cervical lymph node. The clinicopathological characteristics of both the primary and metastatic tumours are presented. In addition, a comprehensive review of the literature pertaining to this rare phenomenon was performed, covering the past 50 years. A 76-year-old male, who had undergone successful surgical resection of a right mandibular peripheral ameloblastoma 10 years earlier, presented with metastatic lesions in a cervical lymph node. Both the primary lesion and the metastatic specimen were re-ex

Giant cell tumor of bone in the mandible presenting without typical histological features: a case report.

BACKGROUND: Giant cell tumor of bone is a locally aggressive bone tumor characterized by the proliferation of round-to-oval mononuclear cells and uniformly distributed osteoclast-type giant cells. Giant cell tumor of bone typically arises in long bones, whereas craniofacial involvement is rare. Atypical histological and clinical presentations can complicate diagnosis. This study presents a challenging case of giant cell tumor of bone in the mandible. CASE PRESENTATION: A Japanese man in his 70s presented with a slowly expanding radiolucent lesion in the left mandible, first noted a decade ago

Utility of pre-chemoradiotherapy Pan-Immune-Inflammation-Value for predicting the osteoradionecrosis rates in locally advanced nasopharyngeal cancers.

PURPOSE: The aim of this retrospective study was to explore whether pretreatment Pan-Immune-Inflammation-Value (PIV) measurements might predict the risk of mandibular osteoradionecrosis (ORN) in patients receiving concurrent chemoradiotherapy (CCRT) for locally advanced nasopharyngeal cancer (LA-NPC). METHODS: The platelet, monocyte, neutrophil, and lymphocyte counts acquired on the first day of CCRT were used to compute pretreatment PIV levels: PIV = (Platelets × Monocytes × Neutrophils) ÷ Lymphocytes. Receiver operating characteristic curve analysis was used to determine the association bet

Does childhood chemotherapy affect mandibular bone structures in a lifetime?

BACKGROUND: Chemotherapy, one of the most important treatment modalities for treating childhood cancers, is a major cause of bone loss in patients and survivors. OBJECTIVES: This study aimed to evaluate mandibular bone structures in childhood cancer survivors (CCSs) by means of fractal dimension (FD) analysis and the Klemetti index (KI), and to compare them with regard to the control group. MATERIAL AND METHODS: In this retrospective study, the panoramic radiographs of 49 CCSs were included as the study group and the panoramic radiographs of 49 cancer-free volunteers were included as the con

Osteosarcoma in a Patient With Pseudohypoparathyroidism Type 1b Due to Paternal Uniparental Disomy of Chromosome 20q.

It is assumed that a persistent high level of parathyroid hormone (PTH) might have a relation with bone malignancy. However, there has been no report of osteosarcoma associated with pseudohypoparathyroidism type 1b (PHP1b), which is accompanied by high PTH. PHP1b is the result of resistance to PTH in certain end-organ tissues, especially the kidney; the response in bone is unaffected because it normally expresses stimulatory G protein equally from both parental alleles. A 21-year-old male, presenting with gum swelling at the right mandible, was referred to a dental clinic. A curative surgical

Unusual Adenomatoid Odontogenic Tumor.

The adenomatoid odontogenic tumor is a rare benign neoplasm. It can, however, have locally aggressive behavior. This is a case of an adenomatoid odontogenic tumor of unusual location and behavior in a 15-year-old female patient. A panoramic radiograph revealed a large radiolucent lesion involving the retained tooth 33. Teeth involved in this lesion were displaced and with apparent root resorption. A prototype of the mandible showed a marked expansion of cortical bone, fenestration points in the lingual cortex, and fragility of the base of the mandible. Therefore, because of the risk of postope

[Transoral coronoidectomy: Technical note].

INTRODUCTION: Among the skeletal causes of limited mouth opening, uni- or bilateral coronoid process hypertrophy, or Langenbeck disease, is the most frequent. It can be associated with an osteochondroma or a coronoid-malar bone conflict and is then called Jacob disease, an unilateral pathology. Treatment rests on coronoidectomy in both cases. This technique is illustrated via two cases, one Langenbeck and one Jacob disease. TECHNICAL NOTE: A transoral approach was performed. After subperiosteal dissection, the coronoid process was cleared. The process was than severed at its base by means of

Mandibular reconstruction after hemimandibulectomy.

Ameloblastoma is a benign tumor of the mandible, which is not commonly diagnosed in the early stages. The extensive mandible resection may be needed for treatment. In this report, we present 6-year follow-up results of a patient who had undergone hemimandibulectomy and mandible reconstruction with free vascularized fibular flap, costochondral rib graft to restoration of the temporomandibular joint, and iliac bone graft to enhance the vertical height of the mandible. The long-term results are very satisfactory.

Chondroma in temporomandibular region--case report and therapeutic considerations.

PURPOSE: Chondromas are benign tumors composed of mature hyaline cartilage. These tumors are quite common in the bones of the hands and feet but extremely rare in jaw bones, and few such cases are reported in the literature. The aim of the present study was to carry out a literature review and present a clinical case of a patient with a chondroma in the right mandibular condyle treated with excision of the tumor. CONCLUSIONS: The importance of early diagnosis and treatment in order to impede or minimize any lasting effects is discussed.

Periapical radiolucency not requiring endodontic therapy: an unusual case.

The article is a case report of an unusual manifestation of primary hyperparathyroidism seen in the mandible. Primary hyperparathyroidism is a rare disorder that can present its first symptoms in the jaws. The pre- and post-treatment radiographic features of such cases have only rarely been reported in dental literature. This case report highlights the importance of careful clinical and radiographic examination before commencing root canal treatment.

Giant cell reparative granuloma in the mandible--case report and review of the literature.

OBJECTIVE: Central giant cell granuloma (GCG) is defined as an intraosseous lesion consisting of cellular fibrous tissue containing multiple foci of hemorrhage, aggregations of multinucleated giant cells. Peripheral GCG is a reactive exophytic lesion occurring on the gingiva and alveolar ridge originating from the periosteum or periodontal membrane, usually as a result of local irritating factors. STUDY DESIGN: The case report evaluated long-term therapy of giant cell granuloma. A 9-year-old boy generally healthy was admitted to our clinic with a swelling in the frontal area of mandible. Gian

Microsurgical reconstruction for radiation necrosis: an evolving disease.

We performed a retrospective chart review of a tertiary care medical center. Our objective was to report our experience with microvascular reconstruction in the head and neck in patients who presented with radiation-induced tissue damage. We will discuss the effects of radiation to soft tissues and bone in the head and neck as well as the challenges it presents for later free tissue transfer. Patients were identified who underwent free tissue transfer to the head and neck for radiation-induced tissue injury by the senior author at our institution. Data were collected to include location of the

Misdiagnosed fibrosarcoma of the mandible mimicking temporomandibular disorder: a rare condition.

The jawbones can be sites of various neoplastic conditions. Given the variety of processes affecting this particular anatomical area, formulation of a precise diagnosis can be challenging to clinicians. Limited jaw movement, pain, and facial asymmetry are common signs among patients, especially those with temporomandibular disorder (TMD). This paper reports a case of primary fibrosarcoma affecting the mandible and surrounding structures in a 14-year-old girl presenting signs and symptoms similar to TMD. Her condition was misdiagnosed, and she was treated for TMD over an extended period before

Implant placement after marsupialization of a dentigerous cyst.

This paper presents a case of a dentigerous cyst accompanied by a history of inflammation, resorption of the roots of the first molar and the lingual aspect of the distal root of the second molar, and devitalization of the 2 premolars. The treatment option chosen was marsupialization of the cyst, extraction of the involved teeth, endodontic treatment of both premolars, and implant insertion in the area of the first mandibular molar at a later stage. Twelve months postsurgery the area of interest was almost flattened and the radiographic examination revealed total disappearance of the radioluce

MRI and CT of a haemangioma of the mandible in Kasabach-Merritt syndrome.

Since the description in 1940 of Kasabach-Merritt syndrome (KMS) in patients with capillary haemangiomas, several other vascular tumours have been recognised as possible causes of this coagulopathy. The literature suggests a specific histological pattern of vascular tumours responsible for KMS, excluding capillary haemangioma [1]. There is an extensive literature on, haemangiomas accompanied by thrombocytopenia, and imaging of thrombosis in the lesion, especially cavernous haemangioma of the liver. However, no report has described a haemangioma of the mandible in the acute stage of the coagulo

Jaw mass in a pediatric patient.

The aneurysmal bone cyst is an unusual cause of a jaw mass in children. Left untreated, this relatively benign lesion can lead to deformity and destructive bone changes. We report a nine-year-old patient with a large untreated aneurysmal bone cyst, and briefly discuss the differential diagnosis of head and neck masses in children.
