Three-Dimensional Evaluation of Mandibular Asymmetry in Adult Violin and Viola Players: A Prospective Cone-Beam Computed Tomography Study.

Long-term violin and viola performances involve sustained asymmetric loading of the craniofacial skeleton; however, their association with 3-dimensional mandibular asymmetry has not been well characterized. This prospective study aimed to evaluate mandibular asymmetry using cone-beam computed tomography (CBCT) in adult violin and viola players who have undergone prolonged intensive musical training since childhood. We prospectively included 20 adult violin and viola players, enrolled in or who graduated from the College of Music, Seoul National University. Participants underwent standardized q

Development of a Novel Adjustable Osteotomy Guide for Use in Free Fibula Flap Osteotomies and Comparison With Conventional Methods on 3D-Printed Models.

CT DICOM data from 5 representative mandibular reconstruction cases were segmented to generate digital models of the maxilla, mandible, and fibula, and then 3D printed (PLA). The authors developed a reusable adjustable osteotomy guide (AOG) to translate in-house virtual surgical planning (VSP) measurements (segment lengths and osteotomy angles) into controlled free fibula flap osteotomies without patient-specific printed cutting guides. For each case, duplicate 3D-printed models were allocated to a conventional wire-template technique (CT) without VSP and to the AOG technique guided by in-hous…

Accuracy of three-dimensional periodontal ligament models generated using cone-beam computed tomography at different resolutions for the assessment of periodontal bone loss.

OBJECTIVE: To develop a method for generating three-dimensional (3D) digital models of the periodontal ligament (PDL) using 3D cone-beam computed tomography (CBCT) reconstruction and to evaluate the accuracy and agreement of the 3D PDL models in the measurement of periodontal bone loss. METHODS: CBCT data collected from four patients with skeletal Class III malocclusion prior to periodontal surgery were reconstructed at three voxel sizes (0.2 mm, 0.25 mm, and 0.3 mm), and 3D tooth and alveolar bone models were generated to obtain digital PDL models for the maxillary and mandibular anterior te

Pronator quadratus musculo-osseous free flap for wide hard palatal defect reconstruction: An anatomical study.

Wide hard palate defects include congenital and acquired defects that are six square centimeters or larger in size. Obturator prostheses and autologous soft tissue transfers have been used to reconstruct palatal defects. This study aims to repair wide, hard palatal defects by using a pronator quadratus musculo-osseous free flap to achieve subtotal reconstruction. Seventeen formalin-fixed cadavers were dissected. Free musculo-osseous pronator quadratus flaps were prepared after a 12 cm curvilinear volar skin incision. Standard 30 × 23 mm (690 ± 52.12 mm2) hard palate defects were made by chisel

Hallermann-Streiff syndrome: experience with 15 patients and review of the literature.

Hallermann-Streiff syndrome is rare, with approximately 150 case reports in the world literature. The syndrome consists of proportionate nanism; hypotrichosis; atrophy and extreme thinness of the skin, particularly over the facial area; an unusual "bird-like" face with mandibular hypoplasia; a prominent thin, pointed nose; congenital cataracts; and severe dental abnormalities. This appears to be a sporadic mutation, and the inheritance pattern is unknown. Clinical management must focus on the more life-threatening and developmental issues early on, and aesthetic deformities can be addressed af

Two middle-aged cases of deep overbite without molar support treated by orthognathic surgery.

INTRODUCTION: Patients with severe overbite in middle age often undergo prosthetic treatment without a diagnosis of dentofacial deformity, but soft tissue trauma can occur in such patients due to the lack of inter-occlusal space. Comprehensive surgical orthodontic treatment and prosthetic treatment are indicated to correct the overbite and soft tissue injury for such patients. PRESENTATION OF CASE: We report the cases of two middle-aged patients with dentofacial deformity and severe overbite without molar support. In both cases, prosthetic treatment had been performed for many years without a

A case of mandible hypoplasia treated with autologous bone graft from mandibular symphysis: Expression of VEGF and receptors in bone regeneration.

The vascular endothelial growth factor (VEGF)/VEGF receptor (VEGFR) system plays an important role in angiogenesis and osteogenesis during both skeletal development and postnatal bone growth and repair. Indeed, protein expression changes of this system could contribute to craniofacial defects commonly associated with a variety of congenital syndromes. Similarly to other craniofacial bones, mandible arises from neural crest cells of the neuroectodermal germ layer, and undergoes membranous ossification. Here, we report a case of left mandibular hypoplasia in a 42-year-old man treated with autolo

AI-based synthetic CT attenuation correction enables reliable quantitative SPECT in unilateral condylar hyperplasia.

PURPOSE: Unilateral condylar hyperplasia (UCH) is a rare mandibular growth disorder in which accurate assessment of condylar metabolic activity is essential for surgical decision-making. Quantitative 99mTc-methylene diphosphonate (MDP) SPECT/CT is commonly used for this purpose; however, CT-based attenuation correction (CTAC) increases radiation exposure and may be affected by registration errors. Artificial intelligence (AI)-generated synthetic CT (SyCT) has been proposed as a CT-independent alternative. This study aimed to evaluate the agreement between SyCT-based attenuation correction (SyC

[Role of fibula in replacement of mandible].

The mandible and the fibula are two totally different bones of the human skeleton. The fibula is a long straight bone of the lower leg playing secondary role compared with the tibia. The mandible, or jaw bone, is the only facial bone that moves and has complex spatial structure. The blood supply of the mandible is mainly endosteal, the inferior mandibular artery, which is one of the more important branch of the maxillary artery is responsible for its arterial supply. The fibula shows the uniform pattern of periosteal blood supply receiving many small branches from the peroneal artery, and havi

Determine the accuracy of CBCT reconstructed panoramic images in periodontal assessment.

OBJECTIVES: Periodontal probing and intraoral radiography are conventional methods for periodontal assessment. Evaluating periodontium using CBCT-reconstructed multiplanar or cross-sectional views is labor-intensive and time-consuming. The objective of this study is to evaluate the accuracy of CBCT-simulated panoramic projections for periodontal assessment and determine whether a more efficient evaluation method can be developed. METHODS: CBCTs taken at East Carolina University School of Dental Medicine from 2014-2023 were screened. The inclusion criteria were: (1) diagnostic-quality CBCT sca

Surgical simulation of Class III edentulous patient using a 3D craniofacial model: report of a case.

A case of edentulous prognathism in a 46-year-old Japanese male is presented. We described the outcome of the patient who underwent simultaneous sagittal splitting ramus osteotomy of the edentulous mandible, interpositional bone graft of severely atrophic edentulous jaws, and delayed placement of titanium implants for reconstruction. We highly recommend performing a surgical simulation using a craniofacial model of the patient's anatomy created using CT image data. The procedure provides almost ideal maxillary and mandibular contours.

Sliding posterior tongue flap.

A modification of the hemitongue advancement flap is presented which provides greater extensibility to the base of the tongue. This technique allows for reconstruction of T2 tumor defects of the midlateral tongue ranging from 4 to 6 cm in length. Mobility of the tongue is minimally compromised, preserving good speech and deglutition. Excessive bulk is not present to impinge on the mandibular alveololingual sulcus, thus allowing early use of lower dentures without the need for adjustment and relining.
