Twenty nail dystrophy in identical twins.

Twelve-year-old identical female twins have had lifelong twenty nail dystrophy. There is no evidence of other skin disorders. The condition has remained unchanged and has been unresponsive to a wide range of therapies. The benign clinical course of their nail disorder suggests twenty nail dystrophy may not be related to lichen planus or other cutaneous diseases. One of the causes of twenty nail dystrophy of childhood may be a localized tissue malformation, analogous to inflammatory linear verrucous epidermal naevus.

Clinical observations on nail changes in psoriasis.

410 cases of psoriasis [282 males (68%) and 127 females (31%)] were interviewed and examined to study the nail changes. The prevalence of nail changes was 78.0% (males = females). Common changes were pitting (67.5%) and onycholysis (67.2%). Dystrophy of varying degrees occurred in 35.0%, subungual hyperkeratosis in 24.7%, discoloration in 18.4%, loss of nails in 2.8% and pustulation in 1.3%. Pitting and onycholysis was the most common combination (45.6%). Nail changes were significantly more common in patients who have moderate to severe psoriasis as compared with patients with mild psoriasis;…

[Yellow nail syndrome with chylous effusions].

Yellow nail syndrome is characterized by a yellow discolouration of the nails associated with idiopathic lymphoedema and pleuropulmonary manifestations. Pleural effusions are usually of the exudate type with a normal lipid content. The case reported here of a 74-year old man who had yellow nails together with chylothorax and chylous ascites provides an opportunity to discuss the origin of these rare effusions which, like the syndrome itself, were most probably due to primary abnormalities of the lymphatic system.

[Skin and nail changes in foot deformities].

Hallux valgus leads to lateral deviation of the big toe's longitudinal axis. In consequence, the forces working on the tip of the toe during unrolling of the foot are shifted in medial direction. As a result, subungual clavi, unguis incarnatus, unguis convolutus, or laterally turning onychogryposis like a cork-screw develop.

Familial severe twenty-nail dystrophy.

Severe nail dystrophy is a recently described acquired nail disorder. The nails are variably involved and may show thinning, thickening, pitting, ridging, koilonychia, opalescence, and loss of luster. Not uncommonly, some nails are completely spared. Although most of the cases reported were among children, and nail changes showed gradual resolution, in a few cases the disorder is seen in adulthood. We recently investigated a pedigree extending through five generations in which twenty-one of the thirty-seven members were variably afflicted with the dystrophic nail changes. To our knowledge, the…

Squamous-cel carcinoma of the nail bed.

Squamous-cell carcinoma of the nail bed is uncommon and is frequently misdiagnosed as a verruca vulgaris. Causes, clinical and histologic features, and procedures of treatment of subungual carcinoma are reviewed. In our patient the clinical appearance was that of a chronic infection associated with onycholysis.

Further clinical experience with the extensible nail.

The extensible nail for treatment of children with brittle bones offers distinct advantages over the conventional rod. It appears to elongate with growth and thus obviates periodic replacement of the rod as the child grows. Complications have been few in this and other series. Recent modifications of the technique for the femur and tibia, and various changes in the original design, constitute significant improvements in the intramedullary nail method.

Ender intramedullary nailing of pertrochanteric fractures.

The remarkable success in Europe of the multiple flexible Ender nails in treating intertrochanteric and pertrochanteric fractures of the femur in the elderly makes it essential for the American orthopedist to understand this procedure as well as those refinements of technique accomplished since 1969. ' Most pitfalls of the nailing have not been outlined, except to call them "complications." With proper understanding of the method, the operation will result in few of these "complications." At Landeskrankenhaus, Graz, 130 cases have been reviewed. There were no superficial or deep infections rep…

Condylocephalic nailing for pathologic subtrochanteric fracture.

Pathologic subtrochanteric fracture in a femur rarefied by fibrous dysplasia of bone was successfully treated with condylocephalic nailing and bone grafting. This procedure is recommended not only for traumatic fractures but also for pathologic fractures secondary to bone tumor.

Squamous cell carcinoma of the nail bed.

Twenty-seven cases of squamous cell carcinoma of the nail bed seen by the author have been reviewed. Sixteen were thought to be primary tumors and eleven to be secondary to chronic exposure to radiation. All had been present for a long time prior to definitive treatment. The tumor was of low grade malignancy with a tendency to invade the distal phalanx. No metastases have been found. Treatment was amputation of the distal phalanx and involved tissues.
