Adjuvant and neoadjuvant chemotherapy for osteosarcoma: JCOG Bone and Soft Tissue Tumor Study Group.

The usefulness of adjuvant chemotherapy for high-grade osteosarcoma was established by two randomized, controlled trials conducted in the 1980s, which used six drugs, doxorubicin, cisplatin, high-dose methotrexate, bleomycin, cyclophosphamide and actinomycin D. Since then, development has been promoted in the direction of introducing preoperative chemotherapy, changing post-operative adjuvant chemotherapy according to histological effects, adding ifosfamide as a key drug and strengthening adjuvant chemotherapy. No clinical trials, however, have shown the effectiveness of study treatment, and t

Tumor-induced rickets in a child with a central giant cell granuloma: a case report.

Tumor-induced osteomalacia/rickets is a rare paraneoplastic disorder associated with a tumor-producing fibroblast growth factor 23 (FGF23). We present a child with symptoms of rickets as the first clinical sign of a central giant cell granuloma (CGCG) with high serum levels of FGF23, a hormone associated with decreased phosphate resorption. A 3-year-old boy presented with a limp and 6 months later with painless growth of the jaw. On examination gingival hypertrophy and genu varum were observed. Investigations revealed hypophosphatemia, normal 1,25 and 25 (OH) vitamin D, and high alkaline phosp

Inflammatory monomorphic undifferentiated sarcoma with distinct clinical and pathological features: a 'new' entity?

AIM: To describe two patients with a highly aggressive, apparently 'new' and rare soft tissue and bone malignancy of childhood and early adult life that we have named inflammatory monomorphic undifferentiated sarcoma. METHODS AND RESULTS: Two histologically identical tumours located in the proximal humerus and the anterior chest wall of males aged 6 and 31 presented as solitary, painful, tender, necrotising, masses, associated with fever, leukocytosis and negative microbiological cultures. The extensively necrotic resected tumours consisted of large, monomorphic epithelioid cells with vesicul

Cytopathology of extraskeletal myxoid chondrosarcoma: report of 8 cases.

BACKGROUND: Extraskeletal myxoid chondrosarcoma (EMC) is a rare soft-tissue sarcoma rarely subjected to cytologic analysis. With the exception of a few small series, the cytology literature of EMC is largely limited to single-case reports. The purpose was to evaluate the cytomorphology of 8 EMC cases as obtained by imprint/scrape cytology and fine-needle aspiration (FNA) biopsy, to review the literature, and to demonstrate the utility of cytogenetic analysis in the diagnosis of EMC. METHODS: The cytology files were reviewed for all soft-tissue lesions signed out as chondrosarcoma, myxoid sarc

Fate of a renal tubulopapillary adenoma transmitted by an organ donor.

Organ transplantation from cadaveric donors has a risk of cancer transmission. However, some reports indicate that kidneys bearing small carcinomas can be safely transplanted, as can other organs harvested from the same donor. We report herein the case of two allograft recipients (left kidney and heart with no evidence of tumor) who developed a renal carcinoma soon after transplantation. The initial tumor of the donor was a 17-mm tubulopapillary adenoma found on the right kidney, which was not transplanted. The left kidney recipient rejected all residual tumoral cells after graft removal and i

Oncogenic osteomalacia caused by a phosphaturic mesenchymal tumor of the oral cavity: a case report.

We report a case of oncogenic osteomalacia associated with a phosphaturic mesenchymal tumor in a 31-year-old woman. She was presented with severe generalized bone and muscle pain and was restricted to bed. She lost 20 cm in height over the 8 years since she had first noticed a pain in her thigh. A walnut-sized, hard, soft tissue tumor was found very easily beside her lower molar teeth Radiologic examination revealed a remarkable decrease in bone density and multiple pathologic fractures of spine, femur and phalangeal bones. Severe hypophosphatemia, hyperphosphaturia, low plasma 1,25-dihydroxyv

Malignancy in Maffucci's syndrome.

Maffucci's syndrome is characterized by multiple enchondromas and subcutaneous hemangiomas. These tumors typically present in early childhood and may lead to significant skeletal deformities. Malignant transformations are a common feature of this syndrome and have been reported in approximately 30% of reported cases, with chondrosarcomas being the most common.

[Fluid-fluid levels in bone and soft tissue tumors demonstrated by MR imaging].

Fluid-fluid levels in bone tumors have been described in aneurysmal bone cysts and other cystic tumors of bones and soft tissue tumors. We experienced three bone tumors (simple bone cyst, bone metastasis, and osteosarcoma) and three soft tissue tumors (fibrosarcoma, two cases of cavernous hemangioma) that showed fluid-fluid levels on MR, and investigated their cause. Causes included blood in the cystic spaces, hemorrhage in the tumor, the telangiectatic component of the osteosarcoma, and the cavernous component of the hemangioma. No specific diagnosis could be made based on the finding of flui

The malignant potential of enchondromatosis.

In a tri-institutional, retrospective study with long-term follow-up, forty-four patients who had multiple enchondromas were identified. Thirty-seven patients did not have hemangiomas (Ollier disease) and seven did (Maffucci syndrome). Of the thirty-seven patients who had Ollier disease, a low-grade chondrosarcoma developed in four; an astrocytoma, in one; and a granulosa-cell ovarian tumor, in one. In four of the seven patients who had Maffucci syndrome, there were six low-grade chondrosarcomas, one high-grade osteosarcoma, one pancreatic adenocarcinoma, one biliary adenocarcinoma, and one as

Round cell liposarcoma arising in the left foot: MRI and PET findings.

Liposarcomas are one of the most common soft-tissue sarcomas and commonly arise in the deep soft tissues of the extremities and retroperitoneum; however, the occurrence of liposarcomas in the foot or ankle is exceedingly rare. In this article, we present a 52-year-old man with round cell liposarcoma arising in the left foot. This tumor presented unusual manifestations of secondary osseous involvement in the metatarsal and tarsal bones of the left foot and solitary lymph node metastasis at the para-aortic region. Magnetic resonance imaging (MRI) and [(18)F]-fluoro-deoxy-glucose positron emissio

[High dose cisplatin combination chemotherapy for advanced breast cancer--a report on 50 cases].

Fifty patients with advanced breast cancer were treated by high dose cisplatin (DDP) combination chemotherapy during the period of August 1984 to June 1991. All of them had measurable disease. Among the fifty patients, 24 had previously received chemotherapy. DDP was given at a dose of 50-128 mg/m2 intravenously every 3 weeks. The regimen was PCMF (DDP + CTX + MTX + 5FU), PA (DDP+ADM) +/- other agents or PF (DDP + 5FU) +/- other agents. The overall response rate was 76% (38/50) with a CR rate of 20% (10/50). The median response duration and median survival time were 5.5 months and 15 months, r

From the archives of the AFIP: benign musculoskeletal lipomatous lesions.

Benign lipomatous lesions involving soft tissue are common musculoskeletal masses that are classified into nine distinct diagnoses: lipoma, lipomatosis, lipomatosis of nerve, lipoblastoma or lipoblastomatosis, angiolipoma, myolipoma of soft tissue, chondroid lipoma, spindle cell lipoma and pleomorphic lipoma, and hibernoma. Soft-tissue lipoma accounts for almost 50% of all soft-tissue tumors. Radiologic evaluation is diagnostic in up to 71% of cases. These lesions are identical to subcutaneous fat on computed tomographic (CT) and magnetic resonance (MR) images and may contain thin septa. Lipom

[Cranial fasciitis: a case report and review of the literature].

Cranial fasciitis is an uncommon, rapidly-growing, benign, non-tumoural, myofibroblastic lesion of the skull, found mainly among young children in their first year of life. It is histologically similar to nodular fasciitis and pseudosarcomatous fasciitis. It may mimic more aggressive pathologies, such as sarcomatosis or histiocytosis, due to its rapid, nodular growth in subcutaneous tissue. Complete resection is considered curative and, therefore, entails a low risk of metastases or malignant recurrences. We present the clinical, radiological and pathological findings in a 4-year-old boy with

Pioneering women in musculoskeletal pathology: A review highlighting their scientific achievements and remarkable impact in sarcoma care.

Bone and soft tissue pathology - perhaps more than in any other surgical pathology subspecialty - has been profoundly shaped by a cadre of trailblazing female pathologists who have seamlessly integrated morphologic expertise, molecular innovation, and educational leadership. While no single review can capture the full breadth of contributions made by women in this field, this article highlights the remarkable achievements of six such leaders - Drs. Sharon W. Weiss, Adrienne M. Flanagan, Julia A. Bridge, Cristina R. Antonescu, Judith V.M.G. Bovée, and Jeanne M. Meis - whose careers collectively

Tumors and Tumor-like Lesions of the Pediatric Hand and Wrist: Clinical Presentation and Risk Factors for Malignancy.

BACKGROUND: Malignant tumors of the pediatric hand and wrist are rare. We hypothesize that pain, large size, and palm/wrist location are risk factors for malignancy in children. METHODS: A retrospective review was conducted of patients younger than 18.9 years of age presenting to our institution with hand and wrist tumors or tumor-like lesions over a 15-year period. Patients with skin lesions, commonly encountered subcutaneous masses (lipomas, ganglion cysts, and sebaceous cysts), multiple hereditary exostoses, and Ollier disease were excluded. Data collection included demographics, clinical
