Assisted Ventilation.

Controlled Mechanical Ventilation may be essential in the setting of severe respiratory failure but consequences to the patient including increased use of sedation and neuromuscular blockade may contribute to delirium, atelectasis, and diaphragm dysfunction. Assisted ventilation allows spontaneous breathing activity to restore physiological displacement of the diaphragm and recruit better perfused lung regions. Pressure Support Ventilation is the most frequently used mode of assisted mechanical ventilation. However, this mode continues to provide a monotonous pattern of support for respiration

[Myasthenia, from the internist's point of view].

Myasthenia gravis is an autoimmune disease due to specific antibodies inducing a neuromuscular transmission defect causing muscle fatigability. If onset of the disease may be at any age, myasthenia gravis concerns mostly young adults, in majority females. The disease characteristic features are the following: ocular symptoms (ptosis or diplopia) as main initial manifestation, extension to other muscles in 80 % of the cases, variability of the deficit, effort induced worsening, successive periods of exacerbation during the disease course, severity depending on respiratory and swallowing impairm

Effects of restraint and immobilization on electrosensory behaviors of weakly electric fish.

Weakly electric fishes have been an important model system in behavioral neuroscience for more than 40 years. These fishes use a specialized electric organ to produce an electric field that is typically below 1 volt/cm and serves in many behaviors including social communication and prey detection. Electrical behaviors are easy to study because inexpensive and widely available tools enable continuous monitoring of the electric field of individual or groups of interacting fish. Weakly electric fish have been routinely used in tightly controlled neurophysiological experiments in which the animal

Human brain activation during phonation and exhalation: common volitional control for two upper airway functions.

Phonation is defined as a laryngeal motor behavior used for speech production, which involves a highly specialized coordination of laryngeal and respiratory neuromuscular control. During speech, brief periods of vocal fold vibration for vowels are interspersed by voiced and unvoiced consonants, glottal stops and glottal fricatives (/h/). It remains unknown whether laryngeal/respiratory coordination of phonation for speech relies on separate neural systems from respiratory control or whether a common system controls both behaviors. To identify the central control system for human phonation, we

[Acute iatrogenic myopathy during treatment of status asthmaticus. Our experience and review of the literature].

CASE REPORT: A young man was admitted to the ICU for acute asthmatic respiratory failure. He was sedated with propofol, curarized with pancuronium and put on mechanical ventilation for 15 day because of the persistent asthmatics crisis. Drug therapy for asthma included high-dose corticosteroids. When sedation and paralysis were stopped, the patient, that was fully conscious, showed flaccid quadriplegia. A muscular biopsy showed elective loss of the myosin thick filaments. Electrophysiologic evaluation was compatible with both neurologic or muscular pathology. The evolution of the disease was g

Tourniquet effect on rocuronium use during lower extremity fracture fixation.

INTRODUCTION: The effect that a tourniquet has on muscle paralytic, specifically rocuronium, and thus muscle paralysis is unknown. The aim of this study was to examine the effect of tourniquet use on the duration of paralytic effect during fracture fixation of the lower extremity and that tourniquet use would prolong the paralytics effect on the muscle distal to it. METHODS: Patients undergoing open reduction and internal fixation of ankle, tibial plafond, or calcaneus fractures were prospectively evaluated at a single academic level 1 trauma center. After induction of general anesthesia with

Effects of 48-H natural hypobaric hypoxia on intermuscular coherence during a repetitive upper-limb task.

Natural hypobaric hypoxia (HH) is a relevant environmental stressor in occupational and sport settings, yet its short-term effects on frequency-specific coupling between antagonist muscles during dynamic upper-limb tasks remain unclear. This study examined whether acute and 48-h natural HH modifies biceps-triceps intermuscular coherence (IMC) during repetitive reaching. Ten healthy lowlanders (4 males/6 females) completed the same 180-s metronome-paced reaching task under normobaric normoxia (NN), acute HH, and after 48 h of continuous exposure at 3,600 m. Surface EMG was recorded from the bic

Using implementation science to decrease variation and high opioid administration in a surgical ICU.

BACKGROUND: High doses and prolonged duration of opioids are associated with tolerance, dependence, and increased mortality. Unfortunately, despite recent efforts to curb outpatient opioid prescribing because of the ongoing epidemic, utilization remains high in the intensive care setting, with intubated patients commonly receiving infusions with a potency much higher than doses required to achieve pain control. We attempted to use implementation science techniques to monitor and reduce excessive opioid prescribing in ventilated patients in our surgical intensive care unit (SICU). METHODS: We

Preventing lower limb injuries in women's football: Influence of the Stop X prevention programme on jump performance and isokinetics in professional and young elite-level female football - a prospective observational cohort study.

BACKGROUND: Lower limb injuries, particularly anterior cruciate ligament (ACL) and thigh injuries, are highly prevalent in women's football, often resulting from sudden changes in direction and poor landing mechanics. Neuromuscular training is critical to address these injury risk factors. This study evaluated the adaptions observed during implementation of the Stop X programme, a specialised neuromuscular training developed by the German Knee Society, on jump performance and isokinetic strength in elite female players. METHODS: In this prospective observational cohort study, 147 female playe

CCR2-dependent monocytes gut-homing contributes to enteric neuroplastic changes in a mouse model of diet-induced obesity.

BACKGROUND: Enteric inflammation and immune cell activation promote enteric neuroplastic changes and CCR2-dependent monocyte recruitment, contributing to obesity-associated colonic dysmotility and chronic inflammation. Here, we examined the role of C-C motif chemokine ligand (CCL2)/C-C chemokine receptor 2 (CCR2)-mediated blood monocytes gut-homing in colonic motor dysfunctions in mice with high fat diet (HFD)-induced obesity. METHODS: Wild-type C57BL/6J mice were fed HFD or standard diet (SD) for 8 weeks. Subgroups received the CCR2 antagonist RS504393 (2 mg/kg/day, oral) or vehicle througho
