Carcinoid heart disease: a potentially fatal complication of carcinoid syndrome.

Carcinoid heart disease is a unique and serious cardiac complication of the neuroendocrine tumour that affects the right side of the heart, especially the tricuspid and pulmonic valves, eventually causing right heart failure. We present a middle-aged man with a history of well-differentiated neuroendocrine tumours of the small intestine with extensive metastases to the liver, mesentery and spine who is receiving monthly octreotide therapy. He presented with generalised fatigue, severe ascites and worsening dyspnoea. Both the transthoracic echocardiography and transoesophageal echocardiography …

[Ostemalacia due to a tumor secreting FGF-23].

A case of oncogenic osteomalacia in a 50-year-old male is here presented. He suffered severe bone pain and marked muscular weakness of 4 years' duration. There were several vertebral deformities in the thoracic spine, bilateral fractures of the iliopubic branches, another fracture in the left ischiopubic branch, and a Looser's zone in the right proximal tibia. An octreotide-Tc scan allowed to identify a small tumor in the posterior aspect of the right knee. It was surgically removed. Microscopically, it was a phosphaturic mesenchymal tumor-mixed connective tissue (PMT-MCT). Expression of FGF-2…

Percutaneous drainage treatment of traumatic pancreatic rupture with pancreatic transection.

Management of traumatic pancreatic pseudocyst associated with pancreatic duct laceration is controversial. Surgical therapy has been clasica … eous drainage under ultrasound guidance was performed and was associated with the administration of octreotide (to inhibit exocrine pancreatic secretion). The drainage flow decreased gradually until ceasing, and the pseudocyst disappeared.

Somatostatin for the prevention of complications following pancreatoduodenectomy.

Pancreatoduodenectomy (PD) may be followed by serious complications chiefly associated with exocrine pancreatic secretion. Somatostatin and its analogues are able to inhibit pancreatic secretion and thus have been advocated for the prevention of complications following pancreatic surgery. In four published multicentric randomized trials, octreotide administration was associated with a decrease in overall complication rate. However and surprisingly, the benefit is less clear when focusing on complications associated with pancreatic secretion as in pancreatic fistula or in high-risk patients (PD…

A Rare Presentation of a Duodenal Neuroendocrine Tumor.

Neuroendocrine tumors (NETs), which are a rare type of tumor, are defined as epithelial cells with predominantly neuroendocrine differentiat … ete computed tomography (CT) scans. The surgery team was consulted for resection of the mass and an octreotide scan was performed prior to resection to further evaluate the mass and to check for any signs of metastatic disease. The octreotide scan demonstrated intense radiotracer accumulation within the duodenal mass consistent with a neuroendocrine tumor an …

Expanding the therapeutic horizon of 177Lu-DOTATATE: a review of current evidence.

Neuroendocrine tumors (NETs) are a heterogenous group of neoplasms originating from neuroendocrine cells, most frequently found in the gastrointestinal tract and pancreas. A defining feature of NETs is the overexpression of somatostatin receptors (SSTRs), particularly subtype 2 (SSTR2), which is the primary target for both diagnostic imaging and therapeutic interventions. Peptide receptor radionuclide therapy (PRRT) using 177Lu-DOTATATE, a radiolabeled somatostatin analog, has emerged as a transformative alternative for patients with advanced or progressive well-differentiated NETs. Considerin

Interventional Radiology in the Era of Radiotheranostics: An Update for Interventional Radiologists!

In 2018, the Food and Drug Administration (FDA) approved lutetium-177 (177Lu) DOTATATE (LUTATHERA, Advanced Accelerator Applications [Novartis]) for the treatment of somatostatin receptor positive gastroenteropancreatic neuroendocrine tumors (GEP-NETs). Since then, 177Lu DOTATATE has been increasingly used as a treatment option for NETs. Less than 5 years later, in March 2022, 177Lu vipivotide tetraxetan (PLUVICTO, Advanced Accelerator Applications) received FDA approval for the treatment of prostatespecific membrane antigen-positive castration-resistant metastatic prostate cancer. Both drugs

Efficacy and safety of PRRT for the treatment of pancreatic neuroendocrine tumors: Systematic review and meta-analysis.

BACKGROUND: PRRT showed promising potential in the management of patients with pNETs. However, there is still a lack of evidence on its relative efficacy and safety compared with other treatment options. This review aims to synthesize the existing evidence on the efficacy and safety of PRRT (without SSA in key comparisons) for pNETs compared to different treatments. METHOD: An electronic search was conducted from inception to December 2024. Comparative studies including RCTs, cohorts and case-control studies focused on using PRRT in the treatment of pNETs were included. Efficacy outcomes incl

Lu-177 DOTATATE Therapy in a Treatment Naive Metastatic NET Pineal Gland With Drop Metastasis to Spinal Cord: A Great Leap Forward in Oncology.

Neuroendocrine tumors (NETs) are a heterogeneous group of tumors, which are usually indolent with metastatic disease at the initial presentation in most clinical situations. Intramedullary spinal cord metastasis (ICSM) is usually rare in the case of neuroendocrine tumors. Peptide receptor radionuclide therapy (PRRT) with Lu-177 DOTATATE has been in use for treatment of metastatic NETs for a long time. Here, the authors present favorable response to PRRT in a treatment-naive patient with metastatic NET of the pineal gland with brain and spinal cord metastases.

Approach to the Patient: Concept and Application of Targeted Radiotherapy in the Paraganglioma Patient.

Paragangliomas can metastasize, posing potential challenges both in symptomatic management and disease control. Systemic targeted radiotherapies using 131I-MIBG and 177Lu-DOTATATE are a mainstay in the treatment of metastatic paragangliomas. This clinical scenario and discussion aim to enhance physicians' knowledge of the stepwise approach to treat these patients with paraganglioma-targeted radiotherapies. It comprehensively discusses current approaches to selecting paraganglioma patients for targeted radiotherapies and how to choose between the two radiotherapies based on specific patient and

Transformation From Atypical to Anaplastic Metastatic Meningioma: Evolutionary Turning Point Marked by Strong 18 F-FDG Avidity and Uptake Collapse From 68 GA-DOTATOC.

World Health Organization (WHO) grade III anaplastic meningioma is scarce. In this way, most studies compared WHO grade I and II. Otherwise, some authors are uncertain about using 18 F-FDG as a diagnostic tool to estimate the WHO grade, especially high. We report the case from a man with a grading tumor evolution from WHO grade II atypical to grade III anaplastic metastatic meningioma. This turning point was imaged using 68 Ga-DOTATOC and 18 F-FDG. Diagnostic was confirmed by histology. The case underlines the unparalleled power of molecular imaging characterized by high sensitivity staging an

A dosimetric comparison of systemic peptide receptor radionuclide therapy and intra-arterial peptide receptor radionuclide therapy in patients with liver dominant gastroenteropancreatic neuroendocrine tumours.

OBJECTIVES: Intra-arterial radionuclide therapy (IART) treatment allows direct delivery of 177 Lu-DOTATATE to the overexpressed somatostatin-positive neuroendocrine liver metastases, which led to higher tumour concentration compared with systemic radionuclide therapy (SRT). The aim was to evaluate and compare the absorbed doses of both IART and SRT to organs and hepatic metastatic sites. METHODS: A total of 48 patients received SRT and IART. In SRT, activity was administered intravenously, whereas in IART, activity was administered directly into hepatic arteries. The sequential whole-body ima

The images showed one in the middle shaft of left femur without corresponding morphology change on the CT portion of the subsequent SPECT/CT images. The patient declined surgical exploration of the left femur. Another activity was in the right breast, which was resected and pathologically confirmed as breast carcinoma. Postsurgically, the patient's symptoms were not improved. Four years later, a repeat 99mTc-HYNIC-TOC scintigraphy showed more prom

Overcoming nephrotoxicity in peptide receptor radionuclide therapy using [177Lu]Lu-DOTA-TATE for the treatment of neuroendocrine tumours.

Peptide receptor radionuclide therapy (PRRT) is used for the treatment of patients with unresectable or metastasized somatostatin receptor type 2 (SSTR2)-expressing gastroenteropancreatic neuroendocrine tumours (GEP-NETs). The radiolabelled somatostatin analogue [177Lu]Lu-DOTA-TATE delivers its radiation dose to SSTR2-overexpressing tumour cells, resulting in selective cell killing during radioactive decay. While tumour control can be achieved in many patients, complete remissions remain rare, causing the majority of patients to relapse after a certain period of time. This raises the question

Orbital Metastases of Neuroendocrine Tumors Treated With 177Lu-DOTATATE PRRT or 131I-MIBG Therapies.

Neuroendocrine tumors have a propensity to metastasize to the orbit, although the reason is unknown. A review of 251 neuroendocrine tumors treated with Lu-DOTATATE or I-MIBG at our institution since 2003 revealed 4 patients with orbital metastases (1.6%), 2 treated with Lu-DOTATATE and 2 with I-MIBG. Of these 4 patients, 1 patient was symptomatic with diplopia and eye pain, and 2 patients had physical signs of orbital involvement. The symptomatic orbital metastasis improved with Lu-DOTATATE therapy, and proptosis improved with I-MIBG therapy in 1 of 2 patients. We present the imaging findings
