A quantitative analysis of the Eutherian orbit: correlations with masticatory apparatus.

The mammalian orbit, or eye-socket, is a highly plastic region of the skull. It comprises between seven and nine bones, all of which vary widely in their contribution to this region among the different mammalian orders and families. It is hypothesised that the structure of the mammalian orbit is principally influenced by the forces generated by the jaw-closing musculature. In order to quantify the orbit, fourteen linear, angular and area measurements were taken from 84 species of placental mammals using a Microscribe-3D digitiser. The results were then analysed using principal components analy…

Salivary duct carcinoma metastatic to eyelid and orbit-a case report.

BACKGROUND: Salivary duct carcinoma is a rare, extremely aggressive malignant tumor, demonstrating invasive growth with early regional and distant metastasis. We describe a case of parotid gland salivary duct carcinoma metastatic to the eyelid and orbit, as confirmed by immunohistochemical analysis. To the best of our knowledge, this is the first such case reported in the literature. METHODS: A 43-year-old male had left lower eyelid and left inferior fornix/anterior orbital masses. The patient underwent left lower eyelid and anterior orbital biopsies for histopathological evaluation. Immunohi…

Wegener's granulomatosis of the orbit: two cases requiring endoscopic surgical decompression.

OBJECTIVES: We report two unusual and contrasting cases of Wegener's granulomatosis involving the orbit, requiring urgent endoscopic orbital decompression surgery. CASE SERIES: Both patients presented with an acute deterioration in visual function which failed to improve with medical management. Case one presented with systemic features of the condition, in contrast with case two, who presented with a more localised form of the disease. Whereas case one exhibited primary orbital disease with maxillary sinus extension (i.e. focal disease), case two illustrates orbital involvement secondary to …

Solitary fibrous tumor of the orbit: morphological, cytogenetic and molecular features.

Solitary fibrous tumor (SFT), a benign neoplasm arising in mesenchymal structures, was initially described in the pleura but subsequently has also been documented in other locations. It is uncommon in the orbit, where it closely resembles other benign spindle-shaped mesenchymal tumors of this area such as schwannoma, meningioma or hemangiopericytoma. We present a case of orbital SFT in a 34-year-old woman. The radiological study showed the presence of an enhanced uptake lesion measuring 2 cm in major diameter. The histopathological evaluation revealed alternating cellular and hypocellular area…

Metastasis to the orbit from transitional cell carcinoma of the bladder.

PURPOSE: To demonstrate the pathological features of the extremely rare metastatic transitional cell carcinoma (TCC) from the bladder to the orbit, and to review the literature on metastatic TCC to the orbit. METHODS: A 74-year-old man experienced 2 weeks of red eye, proptosis, diplopia, pain, and visual loss in the right eye. Three years previous to the current presentation, the patient had undergone a transurethral resection for superficial and moderately differentiated TCC of the bladder. A transseptal anterior orbitotomy was performed. RESULTS: Histopathological examination of the orbita…

Status of the internal orbit after reduction of zygomaticomaxillary complex fractures.

PURPOSE: We sought to determine the status of the internal orbit before and after reduction of zygomaticomaxillary complex (ZMC) fractures when treated without internal orbital reconstruction. PATIENTS AND METHODS: We conducted a retrospective study of preoperative and postoperative computed tomography (CT) scans in 65 patients with unilateral ZMC fractures who were treated by reduction of the ZMC complex without internal orbital reconstruction. The size and location of the internal orbital defects, orbital soft tissue displacement, and orbital volume were assessed in the preoperative and pos…

Adult onset Langerhans cell histiocytosis of the orbit--a case report.

PURPOSE: To report an interesting case of adult onset Langerhans cell histiocytosis in a 41-year-old male with clinicopathological correlation. DESIGN: Interventional case report. MATERIALS AND METHODS: A 41-year-old male presented with an ill-defined mass occupying the temporal quadrant of the left orbit. Computerized tomography revealed an ill-defined extraconal mass involving the superolateral aspect of the left orbit with areas of osteolysis involving the lateral and superior orbital walls. The mass excised at orbitotomy showed microscopic features consistent with Langerhans cell histioc…

Primary liposarcoma of the orbit: a clinicopathologic study of seven cases.

Liposarcoma, the most common soft tissue sarcoma in adults, will rarely involve the orbit, either primarily or as a metastasis. We describe seven primary orbital liposarcomas, representing the largest documented series of primary orbital liposarcoma to date. Affected patients were three males and four females ranging in age from 28 to 69 years (median, 51 years). Five patients presented with painless proptosis, one patient had painful proptosis, and no details of presenting symptoms are available in one case. The site distribution was retrobulbar (3 cases), lateral orbital wall (2 cases), medi…

Objective quantification of the motion of soft tissues in the orbit.

Orbital soft-tissue motion analysis aids in the localization and diagnosis of orbital disorders. A technique has been developed to objectively quantify and visualize motion in the orbit during gaze. T1-weighted MR volume sequences are acquired during gaze and soft-tissue motion is quantified using optical flow techniques. The flow field is visualized using color-coding: orientation of the flow vector is coded by hue and magnitude by saturation of the pixel. Current clinical circumstances limit MR image acquisition to short sequences and short acquisition times. The effect of these limitations …

Granulocytic sarcoma of orbit preceding acute myeloid leukaemia: a case report.

A rare case of granulocytic sarcoma of orbit preceding the onset of acute meyloblastic leukaemia is presented. A 7-year-old boy presented with proptosis and no other systemic complaints. The appearance of an enlarged muscle in initial tomographic imaging with a normal peripheral blood picture caused a diagnostic problem. Biopsy suggested a diagnosis of granulocytic sarcoma. Subsequent bone marrow study revealed the diagnosis of acute myeloid leukaemia.

Pericranial flap correction of superior sulcus depression in the anophthalmic orbit.

Distortion of the supratarsal sulcus of the upper eyelid after orbital trauma is a well-recognized and troublesome problem. This is particularly true of the anophthalmic orbit. The authors present two patients in whom this deformity has been addressed using a pedicled pericranial flap. They found this technique provides abundant, well-vascularized tissue that is manipulated easily to conform to the demands of the defect. In addition, the vascularity of the tissue provides predictability of the result when compared with other described techniques such as fat and dermis-fat grafts.

Solitary fibrous tumour of the orbit. Report of a new case.

PURPOSE: Solitary fibrous tumour (SFT) of the orbit is a very rare lesion that may be misdiagnosed as fibrous histiocytoma, haemangiopericytoma, or other orbital tumour. We studied a case of SFT of the orbit in a 35-year-old woman. METHODS: The patient, with a 1-year history, reported a 'pressure' sensation behind her right eye, but no pain, vision changes, or other associated symptoms. Ocular examination was normal. Magnetic resonance imaging (MRI) showed a well-circumscribed medial mass in the right orbit. The tumour was excised. RESULTS: The tumour was a cellular spindle-cell neoplasm wit…

Iodine-131-metaiodobenzylguanidine uptake in metastatic carcinoid tumor to the orbit.

Metastatic tumor is one of several etiologies of space-occupying masses in the orbit that accounts for 1%-13% of all orbital masses (1). In the adult patient population, breast cancer is the most common tumor to metastasize to the orbit followed by metastases from the lung, prostate and gastrointestinal tract (2). It is rare for carcinoid tumors to metastasize to the eye or to the orbit. Carcinoid tumors arise from Kulchitsky cells that originate in the neural crest. Histologically, these tumors resemble, but are not as aggressive as, adenocarcinomas. Most carcinoids arise in the gastrointesti…

Transnasal approach to the orbit, the interorbital space, and the nasopharynx.

We present a surgical procedure that provides exposure to the medial part of the orbit, the interorbital space, and the nasopharynx. A midline incision of the nose is followed by a pedicled osteotomy of the lateral nasal wall and the medial wall of the orbit in one piece. This transnasal approach provides good exposure to the medial parts of the craniofacial junction to remove foreign bodies or benign tumors.

Protrusion of postoperative maxillary sinus mucocele into the orbit: case reports.

Maxillary sinus mucocele, which appears more than 10 years after Caldwell-Luc surgery, is very rare in the United States or Europe. However, it is very common in Japan. It is called "postoperative maxillary sinus mucocele" or "postoperative maxillary cyst." In this paper, postoperative maxillary sinus mucoceles, which protruded into the orbits, are reported. One patient, a 50-year-old man, underwent Caldwell-Luc surgery 31 years ago, and the other patient, a 61-year-old woman, underwent Caldwell-Luc surgery at the age of 18. The incidence of this disease, symptoms, diagnosis and treatment are …

Orbit, skull base, and pharynx: contrast-enhanced fat suppression MR imaging.

The high signal intensity of fat on T1-weighted magnetic resonance images has limited the utility of gadopentetate dimeglumine in imaging of the extracranial head and neck. Enhancing lesions may be obscured either by proximity to fat or by chemical misregistration artifact. The authors evaluated the role of a gadolinium-enhanced fat suppression imaging technique in the detection of extracranial head and neck abnormalities in 29 patients. These studies were directly compared with conventional pre- and postcontrast T1- and T2-weighted SE sequences. In detecting and defining the extent of abnorma
