Use of desipramine in the medically ill.

The use of desipramine in treating major depressive disorder in the hospitalized medically ill patient is described. Desipramine has a low incidence of anticholinergic side effects, causes little orthostatic hypotension, and has a quinidine-like effect on cardiac arrhythmias. Desipramine should be used with caution in patients with risk of heart block and bundle branch block. Guidelines for administration of desipramine in the medically ill are discussed, and drug interactions with medications commonly used in this population are reviewed. Finally, the importance of providing psychotherapy alo

Counterregulatory responses of healthy cats to insulin-induced hypoglycemia.

In health, insulin-induced hypoglycemia (IIH) activates counterregulatory hormone responses and parasympathetic (PS) and sympathoadrenal systems, which leads to increased glucagon secretion. In diabetes mellitus, these responses are impaired, resulting in greater severity and delayed recovery from hypoglycemia. These counterregulatory responses in health and disease have been documented in humans, rodents, and dogs, but not yet in cats. The aim of this study was to describe and quantify glucagon, PS, and cortisol responses in healthy purpose-bred cats at increasing levels of IIH. Glucagon, cor

Activation of the spinal neuronal network responsible for visceral control during locomotion.

It has been established that stepping of the decerebrate cat was accompanied by involvement of the urinary system: external urethral sphincter (EUS) and detrusor muscle activation, as well as the corresponding increase of the intravesical pressure. Detrusor and EUS evoked EMG activity matched the limbs locomotor movements. Immunohistochemical labeling of the immediate early gene c-fos expression was used to reveal the neural mechanisms of such somatovisceral interconnection within the sacral neural pathways. Study showed that two locomotor modes (forward and backward walking) had significantly

TRPC1, TRPC3, and TRPC4 in Rat Orofacial Structures.

TRPC (transient receptor potential cation channel subfamily C) members are nonselective monovalent cation channels and control Ca2+ inflow. In this study, immunohistochemistry for TRPC1, TRPC3, and TRPC4 was performed on rat oral and craniofacial structures to elucidate their distribution and function in the peripheries. In the trigeminal ganglion (TG), 56.1, 84.1, and 68.3% of sensory neurons were immunoreactive (IR) for TRPC1, TRPC3, and TRPC4, respectively. A double immunofluorescence method revealed that small to medium-sized TG neurons co-expressed TRPCs and calcitonin gene-related peptid

Gastroesophageal reflux is not associated with short-term variability of parasympathetic activity in children.

PURPOSE: A lower parasympathetic activity was described in patients with the gastroesophageal reflux disease. We aimed to determine whether gastroesophageal reflux (GER) episodes are associated with a short-term parasympathetic tone variability in children. METHODS: In order to address this question we performed simultaneous 24-h esophageal multichannel intraluminal impedance-pH and electrocardiographic monitoring in 16 children (age range 6-18 years), including 8 with asthma and 2 with gastroesophageal reflux disease. After describing duration, height, and acidity of 483 GER episodes we also

Individuals with tension and migraine headaches exhibit increased heart rate variability during post-stress mindfulness meditation practice but a decrease during a post-stress control condition - A randomized, controlled experiment.

BACKGROUND: Current research suggests that associations between headache conditions (migraine, tension) and imbalances in the autonomic nervous system (ANS) are due to stress-related dysregulation in the activity of the parasympathetic-sympathetic branches. Mindfulness meditation has demonstrated effectiveness in reducing pain-related distress, and in enhancing heart rate variability-a vagal-mediated marker of ANS balance. This study examined HRV during cognitive stress and mindfulness meditation in individuals with migraine and tension headaches. METHODS: Undergraduate students with tension

Vitamin D Insufficiency is Associated with Reduced Parasympathetic Nerve Fiber Function in Type 2 Diabetes.

OBJECTIVE: Vitamin D insufficiency is prevalent in subjects with type 2 diabetes mellitus (T2DM) and is associated with peripheral neuropathy. However, there are little data regarding vitamin D status in patients with cardiovascular autonomic neuropathy. Our objective was to evaluate the association of cardiovascular autonomic function, 25-hydroxyvitamin D (25[OH]D) insufficiency (i.e., levels <30 ng/mL), and multiple metabolic parameters in subjects with T2DM. METHODS: We examined 50 individuals with T2DM. Cardiovascular autonomic function (i.e., parasympathetic function) was assessed by RR-

Autonomic effects of cognitive reappraisal and acceptance in social anxiety: evidence for common and distinct pathways for parasympathetic reactivity.

Few studies investigated the effects of emotion regulation strategies on autonomic parameters in socially anxious individuals. We asked 99 socially anxious participants to give an impromptu speech in front of an audience in a virtual reality environment. In the anticipation phase, they practiced an emotion regulation strategy: negative functional reappraisal, acceptance, negative dysfunctional reappraisal. All strategies led to decreases in parasympathetic activity and increases in heart rate during anticipation. Parasympathetic activity remained low in the recovery phase, while heart rate inc

Parasympathetic vasoactive intestinal peptide (VIP): a likely contributor to clozapine-induced sialorrhoea.

OBJECTIVE: The parasympathetic transmitter vasoactive intestinal peptide (VIP) increases salivary gland blood flow and evokes protein secretion and, in some species, such as rats, a small fluid secretion. It interacts synergistically with muscarinics for protein and fluid output. Human salivary acini are supplied with VIP-containing nerves. We hypothesise that VIP and clozapine, acting together, evoke a volume of saliva greater than the sum of those induced by each drug given separately. It was further considered whether, in the current test situation, circulatory events influenced the magnitu

Azelastine nasal spray inhibiting parasympathetic function of tracheal smooth muscle.

BACKGROUND: Azelastine hydrochloride is a histamine receptor-1 (H(1)) antagonist with anti-inflammatory properties that is available in the United States as Astelin Nasal Spray for rhinitis patients who are suffering from sneezing and rhinorrhea. The effect of H(1) antagonists on nasal mucosa in vivo is well known; however, the effect of the drug on tracheal smooth muscle has rarely been explored. During administration via oral intake or inhalation of the H(1) antagonist for nasal symptoms, it might affect the trachea. METHODS: We examined the effectiveness of azelastine on isolated rat trach

Parasympathetic overactivity in patients with nasal septum deformities.

Nasal septum deformities (NSD) are one of the most frequent reasons for nasal obstruction presented with a reduction in nasal airflow and chronic mucosal irritation. Nasocardiac reflex which includes afferent stimulus with maxillary division of the trigeminal nerve and the efferent pathway of the heart via the vagus nerve is not a well-known part of autonomic nervous system (ANS). Heart rate variability (HRV) is a parameter reflecting the ANS activity on heart. The purpose of this study is to evaluate ANS functions in patients with NSD by HRV analysis. Twenty-nine patients with NSD and 26 cont

Parasympathetic non-adrenergic, non-cholinergic transmission in rat parotid glands: effects of cholecystokinin-A and -B receptor antagonists on the secretory response.

Recent studies show i.v. administered pentagastrin and cholecystokinin to evoke protein/amylase secretion from the rat parotid gland and to stimulate gland protein synthesis, the two phenomena being abolished by cholecystokinin receptor antagonists. In the rat parotid gland, non-adrenergic, non-cholinergic transmission mechanisms contribute to secretion of fluid and protein/amylase. Since cholecystokinin may act as a neurotransmitter, activation of cholecystokinin receptors of the gland might contribute to the parasympathetic nerve-evoked secretion. In this study, the parasympathetic innervati

Acute Erycibe henryi Prain ("Ting Kung Teng") poisoning.

Erycibe henryi Prain ("Ting Kung Teng"), a species of Convolvulaceae, has been used in Chinese medicine to relieve pain involving the musculoskeletal system, such as arthritis, sciatica, and traumatic tissue swelling. E. henryi can be mistaken for another herbal plant, Tripterygium wilfordii Hook F, used to treat gouty arthritis. We report here three cases of E. henryi poisoning. All three cases presented with vomiting, diarrhea, salivation, diaphoresis, lacrimation, and rhinorrhea; two patients also had miosis, hypothermia, bradycardia, hypotension, and ventricular tachyarrhythmias. Parasympathetic cardiac nerve stimulation with implanted coronary sinus lead.

A patient with drug-refractory paroxysmal atrial fibrillation associated with rapid ventricular rate underwent biatrial pacemaker implantation. During elective replacement of the pacemaker, a significant voltage- and frequency-dependent decrease in ventricular rate was achieved by high-frequency electrical stimulation (17 Hz) of parasympathetic cardiac nerves innervating the AV node with the implanted bipolar coronary sinus electrode. The negative dromotropic effect of parasympathetic stimulation was eliminated by intravenous administration of 1-mg atropine.

[Vegetative functions characteristics in patients with chronic gastritis].

Basing on examination of 311 patients with chronic gastritis (CG), three clinicopathogenetic variants of the disease course were determined. Variant 1 is characterized by superficial involvement of the gastric mucosa, normal or hyperactive secretory function, normokinetic motor type, dominating parasympathetic vegetative tonicity and reactions of the parasympathetic type, rare segmental disorders, absolute hyperhistaminemia. Variant 2 is characterized by moderate atrophy of the gastric glands, secretory and motor hypofunction, unaffected vegetative status, normohistaminemia. Variant 3 runs wit

Comparison of the behavioural effects of infusion of carbachol and acetylcholinesterase into the rat substantia nigra.

It has been postulated for many years that acetylcholinesterase (AChE) may play a nonclassical role in the substantia nigra, unrelated to its ability to hydrolyse acetylcholine. In this study the behavioural effects of unilateral infusion of AChE and a cholinergic agonist, carbachol, were compared. Carbachol induced ipsiversive circling over a very short time scale (minutes), whereas AChE induced contraversive circling, but over a longer time course-10 days. Both agents showed selectivity of response within the substantia nigra: acetylcholinesterase was only effective when infused into the mos

The effect of removing external sodium upon the cholinergic regulation of potassium permeability in the rat submandibular gland in vitro.

Fragments of rat submandibular gland were loaded with 86Rb+ and superfused so that the rate of 86Rb(+)-efflux could be quantified as an indicator of potassium permeability. Acetylcholine evoked an increase in permeability consisting of a transient, calcium-independent response and a sustained, calcium-dependent. Total removal of external sodium significantly inhibited both phases of this response. The results thus confirm that the cholinergic regulation of potassium permeability is compromised by removal of external sodium but do not support the view that this is due, exclusively, to an effect

The mechanism of corneal accommodation in chicks.

Corneal accommodation can account for up to 9 D of accommodation in a freely behaving chick. We have explored the possibility that changes in corneal curvature are due to changes in intraocular pressure (IOP) during accommodation. In an in vitro preparation we demonstrate that increasing the pressure will tend to flatten the cornea. We have used electrical stimulation of the Edinger-Westphal (EW) nucleus to further test the pressure hypothesis in vivo by recording IOP changes in the eye during EW-stimulated accommodation and by artificially modulating the IOP to assess the effects on corneal c

Inhibition of cholinergic neurotransmission in isolated guinea-pig main bronchi by SR 48968.

SR 48968 (10(-6) to 10(-5) M) inhibited the cholinergic response of the isolated guinea-pig main bronchus to electrical field stimulation. Since this effect was reversed by naloxone 10(-5) M and since SR 48968 had no effect on the contractile response to exogenous acetylcholine, we conclude that SR 48968 acts at a prejunctional level and that opioid receptors are involved. This effect was observed at concentrations approximately 75,000 times higher than those needed for blockade of tachykinin NK2 receptors.
