Consequences of and strategies to deal with residual post-treatment root canal infection.

Bacterial sampling of prepared root canals is used to determine the presence and character of the remaining microbiota. However, it is likely that current sampling techniques only identify organisms in the main branches of the root canal system whereas it is unlikely that they can sample areas beyond the apical end-point of preparation and filling, or in lateral canals, canal extensions, apical ramifications, isthmuses and within dentinal tubules. Thus, it may be impossible by current techniques to identify residual post-treatment root canal infection. In histologic observations of root apices

Scanning electron microscopic observations of apical root surfaces of teeth with apical periodontitis.

The aim of this study was to observe apical root surfaces of teeth with chronic periapical lesions. Five premolars and three molars with radiographically demonstrable periapical lesions were extracted and fixed in 2.5% phosphate-buffered glutaraldehyde solution for 9 days. The apical 3-mm portion of 17 roots was removed and prepared for scanning electron microscope. Lacunar resorption zones were frequently observed on the root surfaces. Bacteria and yeast cells were detected in some of the lacunae. Periapical bacterial plaque with a smooth structure was present mostly around the main apical fo

Periapical changes after orthodontic movement of root-filled ferret canines.

The effectiveness of orthodontic forces in moving root-filled teeth and the degree of apical resorption it causes have been observed in an animal model. Under general anesthesia, root canal therapy was performed on the mandibular canines on one side of 12 young male ferrets. Bilateral movement of the canines was induced with an orthodontic spring (150 to 175 g). Procion red dye was injected 1 wk presurgically to mark bone and cementum deposition. Three months later, tooth movement was assessed from pre- and posttreatment mandibular casts and by fluorescence microscopy from labeled bone deposit

Novel Uses of Optical Coherence Tomography in Periodontology.

Optical coherence tomography (OCT) is a noninvasive imaging technology that provides high-resolution, cross-sectional views of biological tissues using near-infrared light. Since its introduction in the early 1990s, OCT has expanded from ophthalmology into dentistry. With micrometer-level resolution and no ionizing radiation, dental OCT offers significant advantages over conventional x-ray-based imaging modalities such as intraoral radiography and cone beam computed tomography, particularly for detecting early or subtle changes in hard and soft tissues. In this article, we review the evolution

Microbiological aspects of traumatic injuries.

After traumatic injuries to teeth, microorganisms may invade the compromised pulp tissue and initiate pulp infection and periapical inflammation. In addition to bone resorption that typically accompanies pulp necrosis, root resorption frequently occurs. Root resorption has several variants that may occur shortly after the trauma or at a later stage. The pathological changes seen after traumatic injuries to teeth are invariably linked to the presence of microbial irritants. The presence of bacterial biofilms in the dental pulp space can be treated with regenerative or therapeutic endodontic pro

The ability of cone-beam computerized tomography to detect vertical root fractures in endodontically treated and nonendodontically treated teeth: a report of 3 cases.

A definitive diagnosis of vertical root fracture (VRFs) is often a challenging task for clinicians. Two-dimensional periapical radiographs (PRs) may be not helpful in such a diagnosis when the x-ray beam is not parallel to the plane of the fracture line. This report presents a set of 3 cases in which 1 endodontically treated and 2 nonendodontically treated mandibular molars were diagnosed with VRFs based on findings from clinical, radiographic, and cone-beam computerized tomographic (CBCT) examinations. After extraction, VRFs were confirmed in all of the teeth. Deep and narrow periodontal pock

Periodontal ligament changes in patients with progressive systemic sclerosis.

Progressive systemic sclerosis (PSS) is a generalized collagen disease which has oral manifestations, including widening of the periodontal ligament (PDL) space. The purpose of this study was to measure the extent of thickening of the PDL space in a group of 26 patients with PSS and in a similar group of age and sex-matched control patients. The width of the PDL was measured from periapical radiographs at four sites around seven selected teeth for every patient. The mean PDL width was found to be larger in all the patients with PSS than in the control patients, around both anterior and posteri

Radiographic signature in apical periodontitis improves prediction of apical lesion healing through survival prediction model.

This retrospective study aimed to evaluate the effectiveness of radiographic signatures of apical periodontitis (AP), particularly lesion boundary features, in predicting lesion healing periods using survival analysis. A total of 254 AP cases with apical lesions were included. Canny edge detection and fragment analysis (FA) were used to define the regions of interest (ROI) S1-S4 on radiographs. Radiographic signatures were extracted, and a radiomics score (rad-score) was developed using the least absolute shrinkage and selection operator (LASSO) Cox regression. Preliminary validation was perfo

Effectiveness of endodontic tissue engineering in treatment of apical periodontitis: A systematic review.

BACKGROUND: Regenerative endodontics has evolved in recent years with tissue engineering concepts in particular appearing promising. Endodontic tissue engineering (ETE) describes the various approaches based on the orthograde introduction of scaffolds or biomaterials (with or without cells) into the root canal to achieve pulp tissue regeneration. There are currently no systematic reviews investigating whether ETE is a suitable method for the treatment of endodontic disease in both mature and immature permanent teeth. OBJECTIVES: The purpose of this systematic review was to determine the effec
