Relationship Between Pityriasis Lichenoides and Mycosis Fungoides: A Clinicopathological, Immunohistochemical, and Molecular Study.

BACKGROUND: Several cases of pityriasis lichenoides (PL) have been reported to evolve into mycosis fungoides (MF). OBJECTIVE: To elucidate clues to this progression. METHODS: Fifty-eight patients with PL between 2000 and 2013 (follow-up: 3-16 years, average: 8.3). RESULTS: A total of 3 (5.2%) of the 58 patients with PL developed MF after 3-11 years of prolonged clinical course. Papules and small plaques characterized PLs, and patches and larger plaques subsequent MFs. A total of 35 of 41 (85%) followed up non-MF associated patients with PL reported lasting complete remissions. Histopatholog…

Pityriasis lichenoides et varioliformis acuta in skin of color: new observations by dermoscopy.

BACKGROUND: Pityriasis lichenoides is an uncommon skin disease that presents in three different forms: pityriasis lichenoides et varioliformis acuta (PLEVA), pityriasis lichenoides chronica (PLC) and febrile ulceronecrotic-Mucha-Habermann disease. These represent a spectrum of a disease. PLEVA presents as skin eruption of multiple, small, red papules that develop into polymorphic lesions with periods of varying remissions, as well as possible sequels of hyper/hypopigmentation and varicella-like scars. Diagnosis of this condition is mainly clinical, and sometimes clinical differentiation from o…

Red Cell Distribution Width and Mean Platelet Volume in Patients With Pityriasis Rosea.

BACKGROUND: Pityriasis rosea (PR) is an inflammatory skin disorder of unknown etiology. However, it is suggested to be related with the reactivation of human herpes virus 7 (HHV-7) and/or HHV-6. It is sometimes diffucult to distinguish PR from PR-like drug eruptions and other inflammatory disorders, so we need new parameters which are cheap and easy in determining PR. Red blood cell distribution width (RDW) and mean platelet volume (MPV) have been studied as inflammatory markers in recent studies. However, the RDW and MPV in PR patients have not been investigated. This is the first study inves…

SIXTEEN YEARS OF PITYRIASIS VERSICOLOR IN METROPOLITAN AREA OF PORTO ALEGRE, SOUTHERN BRAZIL.

Pityriasis versicolor is the most common of the diseases caused by Malassezia yeasts. The aim of this study is to determine the prevalence of pityriasis versicolor and its etiological aspects in the metropolitan area of Porto Alegre, Brazil. A retrospective cross-sectional study with data from patients of a reference hospital from 1996 to 2011 was performed. Collected data included: date, age, gender, ethnicity, anatomical region of lesion and the direct mycological examination results. Among the positive results in the direct mycological examination, 5.8% (2,239) were positive for pityriasis …

Tissue immunostaining for factor XIIIa in dermal dendrocytes of pityriasis alba skin lesions.

BACKGROUND: Pityriasis alba affects 1% of the world population and about 9.9% of the children in Brazil. However, its etiology remains uncertain. OBJECTIVE: The objective of the present study was to evaluate the immunoexpression of factor XIIIa in dermal dendrocytes of skin lesions of pityriasis alba. METHOD: Twenty patients with pityriasis alba and 20 patients with atopic dermatitis underwent biopsy. The dermal dendrocytes marked by factor XIIIa were counted by means of immunohistochemical analysis. RESULTS: The mean amount of dermal dendrocytes found in the patients with pityriasis alba w…

Rapid reversal of hyperpigmentation in pityriasis versicolor upon short-term topical cycloserine application.

The clinical phenomena of pityriasis versicolor (PV), a common Malassezia-associated skin disease, such as hyperpigmentation, depigmentation and fluorescence of the lesions may at least partly be explained by the generation of Trp-derived indole pigments through the action of transaminase 1 (TAM 1). Cycloserine, a TAM inhibitor, was able to completely inhibit pigment production in M. furfur in vitro in a dose-dependent manner. Application of a 0.2-mol l(-1) aqueous cycloserine solution b.i.d. for 5 days in three patients with hyperpigmented PV resulted in complete healing within 3-5 days witho…

Resolution of pityriasis rubra pilaris-induced cicatricial ectropion with systemic low-dose methotrexate.

PURPOSE: To report a case of pityriasis rubra pilaris (PRP)-induced bilateral lower eyelid cicatricial ectropion that resolved with systemic low-dose methotrexate. DESIGN: Observational case report. METHODS: A retrospective case review of a patient with cicatricial ectropion caused by PRP. RESULTS: A 82-year-old female presented with bilateral lower eyelid cicatricial ectropion secondary to PRP. The patient did not systemically respond to conventional oral retinoid therapy. She was treated with low-dose methotrexate and experienced dramatic resolution of her signs and symptoms, including co…

Investigation of herpes simplex virus DNA in pityriasis rosea by polymerase chain reaction.

BACKGROUND: Pityriasis rosea (PR) is an acute, inflammatory disease of unknown cause. Clinical and experimental findings indicate an infectious etiology of PR. Our purpose is to examine the skin lesions and blood samples of PR patients by polymerase chain reaction (PCR) for the presence of HSV type 1 and 2 DNA. METHODS: The lesional skin biopsies from 10 patients and blood samples from two randomized patients with clinically and histologically confirmed pityriasis rosea were examined by PCR. RESULTS: No HSV 1 and HSV 2 DNA was detected in the lesional biopsy and blood samples. CONCLUSIONS: …

Human herpes virus-like particles in pityriasis rosea lesions: an electron microscopy study.

BACKGROUND: In a previous study we detected virions with electron microscopy features of human herpes viruses in the supernatant of cocultured mononuclear cells from patients with acute pityriasis rosea. Because of their morphology and of polymerase chain reaction studies, we ascribed them to human herpes virus 7. OBJECTIVE: To find such virions in the lesional skin of pityriasis rosea patients. METHODS: Skin specimens from lesions of 21 patients with acute pityriasis rosea were examined by elecron microscopy. RESULTS: In 15 (71%) patients, human herpes virus particles in various stages of …

Serum triglyceride elevation following high-dose vitamin A treatment for pityriasis rubra pilaris.

We describe two patients with pityriasis rubra pilaris in whom hypertriglyceridemia developed following the oral administration of high-dose vitamin A (retinol). The effects of transient hypertriglyceridemia are unknown, as both patients tolerated high-dose vitamin A therapy with minor, if any, other side effects. Both patients had minimal clinical improvement with this therapy. Patients being treated with 1,000,000 IU of vitamin A each day for ten to 14 days should have frequent determinations of serum lipid levels and liver function during the period of drug administration.

Treatment of pityriasis rubra pilaris with an oral aromatic retinoid (RO 10--9359).

A patient with widespread PRP was first treated with an oral aromatic retinoid (RO 10--9359) and PUVA in successive periods. Retinoid treatment resulted in marked improvement but during PUVA treatment the disease became re-activated. The patient was finally treated with aromatic retinoid alone, which brought the disease to remission in 6 weeks.

[Idiopathic eruptive macular pigmentation following pityriasis rosea. Presentation of two cases (author's transl)].

We describe two cases of acquired hyperpigmented macular eruption following pityriasis rosea. This condition is characterized by an eruption of discrete grayish macules on the trunk. On the back, the lesion are oval and their long axis follows the cutaneous liners. Similar features have been described by Runtova. The clinical type of this pigmented eruption is quite comparable to the cases recently described by Degos, Civatte and Belaïch under the title of "idiopathic eruptive macular pigmentation". Sometimes the "idiopathic" character of this pigmented eruption may be lacking, when a retrospe…
