Perforated appendicitis: prospective outcome analysis for 150 children.

BACKGROUND/PURPOSE: Controversy persists in the management of perforated appendicitis with regard to antibiotic choice and duration, operative timing, drain utilization, and wound closure. For 2 decades at the authors' institution, patients were treated with ampicillin, gentamicin, and clindamycin for 10 inpatient days, with drains in the abdomen, resulting in lower complication rates than most other published series. Managed care pressures have led to less aggressive medical management regimens with length of stay and financial factors viewed as principal outcome measures with little emphasis

Article Processing Charges in General Surgery Journals: Implications for Equitable Publishing Access in Low-Income and Middle-Income Countries.

INTRODUCTION: Open access publishing promotes broad dissemination of scientific knowledge, enhancing transparency and accessibility. However, article processing charges (APCs) required by many journals create significant financial barriers, particularly for researchers from low- and middle-income countries (LMICs). Prior studies across multiple disciplines have shown that APCs disproportionately restrict LMIC authors' ability to publish, perpetuating inequities in academic representation. The impact of APCs within general surgery remains underexplored. This study aims to quantify APCs in surgi

Optimizing indirect targeting of the centromedian nucleus for deep brain stimulation by incorporating third ventricular anatomy.

OBJECTIVE: Deep brain stimulation (DBS) of the centromedian nucleus (CM) is used to treat diverse brain diseases including epilepsy, Tourette syndrome, and disorders of consciousness. However, the CM is challenging to visualize on routine MRI. Many surgeons use an indirect targeting method based on established stereotactic coordinates. The authors aimed to quantify how often a DBS electrode's contacts were positioned within the CM using this approach, and to identify alternative indirect coordinates that are more accurate. METHODS: Indirect targeting of the CM was performed on 100 MR images o

Topical steroids for nasal polyps.

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) represents inflammatory changes throughout the nose and sinuses from a group of disorders which all lead to swelling and overgrowth of the nasal mucosa. Topical corticosteroids have been the most widely used treatment, with each clinician using different regimes, at different doses, in different settings and with or without sinus surgery. CRSwNP requires ongoing medical management to prevent recurrence. OBJECTIVES: To assess the effects of topical corticosteroids on CRSwNP and to analyse various subgroups, including patients who ha

Contextual Engagement to Enhance Effective Transcranial Direct Current Stimulation: A Theoretical and Mechanistic Perspective.

Transcranial direct current stimulation (tDCS) is a form of noninvasive brain stimulation that delivers a low amount of electrical energy to targeted brain regions. Recently, a tDCS device was approved by the U.S. Food and Drug Administration for treatment of major depressive disorder. Although this is an important step forward, the observed effect size was modest, and finding ways to improve tDCS efficacy is important. One way to do so is to leverage contextual dependence of tDCS effects. tDCS modulates neuronal excitability in a state-dependent manner, and concurrent cognitive, emotional, or

Squamous Cell Carcinoma of Finger Invading Middle Phalanx: Surgical Considerations.

Squamous cell carcinoma (SCC) of the nail unit is a rare but highly treatable malignancy. A tissue biopsy is recommended to establish the diagnosis and guide treatment. Mohs surgery is the standard of care to preserve tissue and function for early SCC. The authors report a 76-year-old patient with a very advanced and neglected SCC of the nail unit that grossly extended to the middle phalanx. No prior reports of such an extensive growth have been documented. Due to extensive skin and bone involvement, we proceeded with amputation at the level of the proximal phalanx to obtain negative margins.

Authors' response to letter to the editor: Sex differences in neuropathological response to traumatic brain injury: increased neuronal loss and astrogliosis in females.

The present correspondence constitutes a response to the Letter to the Editor regarding our publication Sex Differences in Neuropathological Response to Traumatic Brain Injury: Increased Neuronal Loss and Astrogliosis in Females. We are pleased that our paper has attracted attention and hereby provide comments on the authors' observations and suggestions. In this reply, we clarify our rationale for analysing the cerebral cortex, the absence of behavioural data, and the choice of the 30-day post-injury timeframe, which was based on prior experience with this model of brain injury. Furthermore,

Successful Management of Rhinocerebral Mucormycosis With Frontal Bone Invasion in a Steroid-Induced Immunocompromised Patient.

Rhinocerebral mucormycosis is a rare but life-threatening fungal infection that progresses rapidly, particularly in immunocompromised patients. The authors report a case of a 65-year-old female with no prior history of diabetes or malignancy who developed invasive mucormycosis following corticosteroid treatment for immune thrombocytopenia. The infection extended from the nasal cavity through the cribriform plate into the frontal bone and dura mater. Initial endoscopic debridement with nasoseptal flap reconstruction was followed by a second-stage bicoronal resection and titanium plate reconstru

Mechanism of action, resistance, interaction, pharmacokinetics, pharmacodynamics, and safety of fostemsavir.

The Food and Drug Administration (FDA) has licensed many antiretroviral medications to treat human immunodeficiency virus type 1 (HIV-1), however, treatment options for people with multi-drug resistant HIV remain limited. Medication resistance, undesirable effects, prior tolerance, and previous interlacement incapacity to deliver new drug classes all lead to the requirement for new medication classes and drug combination therapy. Fostemsavir (FTR) is a new CD-4 attachment inhibitor medicine that was recently authorized by the United States FDA to treat HIV-1. In individuals with multidrug-resi

Oculoplastic Operative Considerations for Kabuki Syndrome: A Case Report and Review of the Literature.

Kabuki syndrome is a rare genetic disease with multisystemic effects including ocular manifestations. The authors report a patient with known Kabuki syndrome who presented with bilateral euryblepharon, bilateral ptosis, OD hypotropia, and blue sclera. A bilateral lateral tarsal strip procedure was performed followed by a left frontalis sling with a silicone implant and a right external levator advancement with success. Two hours after the case conclusion, the patient developed a right upper eyelid hematoma. The hematoma was emergently expressed, and the wound was cauterized and sutured. On car

Resolution of Marked Bradycardia Following Angioplasty.

Bradycardia, a condition commonly managed conservatively and, subsequently, with pacemaker implantation presents a unique challenge when coexisting with coronary artery disease (CAD). This case report delves into an unusual scenario where bradycardia and its related symptoms resolved following coronary angioplasty. Our goal is to contribute valuable data to the argument for a comprehensive evaluation of bradycardic patients for underlying CAD prior to considering pacemaker implantation. This approach aims to prevent unnecessary pacemaker implantations and offers insights into the optimal manag

Asymptomatic Left Ventricle Systolic Dysfunction.

Heart failure is a common debilitating illness, associated with significant morbidity and mortality, rehospitalisation and societal costs. Current guidelines and position statements emphasise the management of patients with overt symptomatic disease, but the increasing prevalence of congestive heart failure underscores the need to identify and manage patients with early left ventricular dysfunction prior to symptom onset. Asymptomatic left ventricular systolic dysfunction (ALVSD), classified as stage B heart failure, is defined as depressed left ventricular systolic function in the absence of

Expectorated metastatic adrenocortical carcinoma: a rare presentation of a rare tumour.

A 61-year-old woman who was originally diagnosed with locally advanced adrenocortical carcinoma (ACC) 10 years ago, developed massive haemoptysis while away from the UK. Initial investigations revealed a left upper lobe lesion requiring embolisation. Bronchoscopic evaluation was unsuccessful in identifying an underlying cause, and cytology was suggestive of an inflammatory cause. A rigid bronchoscopy was planned but prior to this the patient coughed up a lump of tissue which was sent for histopathological examination. Morphology and immunohistochemistry were consistent with metastatic ACC. ACC

Complete Anterior Capsule Phimosis following Cataract Surgery in a Patient with a History of Retinopathy of Prematurity, Nystagmus, and a Narrow Angle.

The authors present a case of complete anterior capsule phimosis and vision decline which developed 4 weeks postoperatively in the right eye after uncomplicated cataract surgery. Prior ocular history included retinopathy of prematurity in both eyes, acute angle closure glaucoma in the left eye, prophylactic laser peripheral iridotomy for a narrow angle in the right eye, and nystagmus in both eyes. This condition was addressed by surgically releasing the anterior capsule with microscissors to open the pupillary space which had been completely obscured by the anterior capsule, also causing the h

The contamination of valsartan and other sartans, Part 2: Untargeted screening reveals contamination with amides additionally to known nitrosamine impurities.

The recent incidences of contaminated valsartan drug products gave rise to review the suitability of current impurity profiling workflows implemented at authorities and pharmaceutical companies. The major drawback of targeted impurity profiling, where a considerable amount of prior knowledge about possible contaminants is necessary, is the fact that unexpected impurities are overlooked easily. Here, a generic untargeted approach was applied on sartan containing drug products. The untargeted workflow allowed for the discrimination of different batches, different production sites, and difference

Vernet syndrome: intracranial extension of a slow-growing mass.

Vernet syndrome, often referred to as jugular foramen syndrome, is a rare clinical entity characterised by a set of signs and symptoms caused by dysfunction of IX, X and XI cranial nerves. Although paraganglioma of the head and neck is the most frequent aetiology, it may also be caused by meningioma, VIII cranial nerve schwannoma, pontocerebellar cistern metastases, head and neck trauma, infections and very rarely by cholesteatoma which extends to the petrous apex. The authors describe a case of a patient with a jugulotympanic paraganglioma in which evolution ends up in Vernet syndrome. The pa

The association of smoking with contact dermatitis and hand eczema - a review.

Given the high prevalence of allergic contact dermatitis (ACD), irritant contact dermatitis (ICD), hand eczema (HE), and smoking, and the impact that smoking has on skin disease, the authors reviewed the existing literature to assess the association between smoking and contact dermatitis or hand eczema. Using the PubMed and SCOPUS databases, a literature search identified articles related to allergic contact dermatitis, irritant contact dermatitis, and hand eczema and a possible association with smoking. The search period included articles prior to and including April 2016. Seven of eight arti

Delayed Superficial Migration of Retained Hyaluronic Acid Years Following Periocular Injection.

Cosmetic injection of hyaluronic acid (HA) and other fillers is increasingly common, and the late complications of these relatively new procedures are now coming to medical attention. Three patients with delayed periocular swelling that began years after injection of HA are described, with CT, MRI, and histopathologic characterization. While HA fillers are marketed as having a temporary effect of several months, the authors demonstrate that they may persist in the body for up to 9 years. Unlike most previous reports, there was no inflammatory reaction or encapsulation, simply infiltration into
