Prison Break: Extraction of a Plug Jailed Between Two Mechanical Valves.

Paravalvular leak (PVL) occurs in up to 17% of prosthetic valve replacements, mainly in the mitral position. Surgery is the gold standard but carries high morbidity in patients with a history of previous sternotomies. Transcatheter closure of PVL offers a minimally invasive alternative with a very high success rate. Device migration, while uncommon, can be catastrophic when impinging on mechanical prosthetic valve performance. We report the case of a 37-year-old woman with prior mechanical mitral and aortic valve replacements, complicated by recurrent heart failure due to a mitral PVL resultin

Conducting Grief Support Groups in Prison: A Pragmatic Feasibility and Impact Study.

Unaddressed grief and trauma may precipitate and maintain problematic substance use, criminal behavior, and resultant incarceration. We detail the implementation and evaluation of an evidence-based grief support group model conducted in seven groups in the two substance-use disorder treatment programs within a state correctional facility. The three aims of this project were to (1) train Department of Corrections mental health clinicians in the grief support group model, (2) conduct seven grief support groups facilitated by trained clinical social workers in two substance-use disorder treatment

[Health and risk behaviors and risky conditions: prison as a challenge for healthcare provision].

Incarceration is associated with significant health burdens. Prisoners suffer disproportionately from chronic diseases, mental disorders, and substance use disorders. While primary care in prisons is usually provided regularly and sometimes even faster than outside, care for more complex treatment needs is often limited to few facilities. Structural deficits include restricted freedom of physician choice, limited specialist access, separation from the regular health system, and gaps at the transitions from incarceration to release.Despite the principle of equivalence, prison healthcare often f…

Prospective evaluation of the impact of repeated whole prison testing for hepatitis C.

INTRODUCTION: Chronic hepatitis C virus (HCV) infection is prevalent in prisons. Universal reception HCV testing is recommended, but acceptance can be suboptimal. To detect and treat missed HCV infections, a high-intensity test and treat (HITT) programme was introduced to rapidly test entire prisons. It remains unknown whether regular HITTs will be required to maintain prison microelimination. We aimed to assess the outcomes of HITTs conducted in a prison 4 years apart with ongoing reception testing. METHODS: A prospective observational evaluation of the impact of HITTs was conducted in Janua…

Patterns and Treatment Timelines of Tuberculosis in Males From Ecuador's Largest Prison.

The analysis of tuberculosis (TB) registers in prisons provides insights into TB transmission and highlights conditions that increase susceptibility. This study aims to characterize TB patient surveillance in Ecuador's largest male penitentiary over the course of 3 years. Using national TB administrative records from January 1, 2017, to December 31, 2019, we calculated five operational indicators. In addition, the number of clinical consultations (register entries) per patient over the 3 years was calculated. The linkage process involved merging data from the administrative files to create a n…

An epidemiological investigation of the prevalence of digestive diseases in the older prisoner population in Taiwan.

OBJECTIVE: The prison population has been increasing globally, and prisoner health is an important issue of global health. However, it has not received sufficient attention, especially in developing countries and regions. Among all the prisoners, the health status of older prisoners requires special attention due to their declining health and energy. This study aims to assess the prevalence of digestive diseases among older prisoners in Taiwan. METHODS: Data on older prisoners from the National Health Insurance Research Database from January 1 to December 31, 2013, were used in this study. Di

The prevalence of gambling problems in prison populations: A systematic review and meta-analysis.

INTRODUCTION: The overall prevalence of gambling problems across prison populations is currently unknown. The objective of the present study was therefore to quantitatively synthetize prevalence estimates of gambling problems in prison populations using a random effects meta-analytic model and to investigate if the estimates were moderated by time frame, cut-off levels, and sample size. METHODS: To be included the studies had to report original data on the prevalence of gambling problems in a prison sample and to be written in a European language, whereas data based on abstracts or qualitativ

Locked in and left out: the "prison penalty" for implementation of evidence-based interventions.

BACKGROUND: While the broader medical community grapples with the widely accepted notion that it takes an average of 17 years for research evidence to be incorporated into clinical practice, the implementation of evidence-based interventions in carceral settings (i.e., jails and prisons) faces longer delays, exacerbating health disparities. MAIN BODY: The "prison implementation penalty" describes the significant delay in and limited adoption of evidence-based healthcare practices in carceral settings. We explore the complex challenges of implementing evidence-based interventions in jails and …

Women's Shelter, a prison in reverse: when the protection of victims imprisons them.

This article aimed to know a Shelter for women in situations of intimate partner violence at imminent risk of death located in the state of Rio de Janeiro from the perception of its professionals. To this end, this qualitative research adopted semi-structured interviews with seven professionals directly involved with the care and assistance to the Shelter's user population. The findings were interpreted through Bardin's thematic content analysis. Moreover, the profile of the women and children sheltered in 2021 was drawn based on the data provided by the institution. In conceptual terms, this

A Process Model of Formative Work to Strengthen a Prison Health Surveillance System.

Worldwide, there is a lack of systematically collected health data on people who are incarcerated. Our objective in this paper was to describe a process model of formative work for a project to strengthen health surveillance for people incarcerated under a Canadian prison authority. We have developed project structures and processes, and we are evaluating project partnerships. To inform prison health surveillance foci, we are conducting a review of literature on best practices, a qualitative study to understand stakeholders' needs and priorities, and mapping work to understand available prison

Worldwide incidence of suicides in prison: a systematic review with meta-regression analyses.

BACKGROUND: Suicide is a leading cause of death during imprisonment. This systematic review aimed to synthesise available evidence of prison suicide incidence worldwide. METHODS: We systematically searched the scientific literature, data repositories, and prison system reports, supplemented by correspondence with prison administrations. We included reports on people living in prison but excluded studies in preselected groups (by age or offence type). Absolute numbers and incidence rates of suicide mortality per 100 000 person-years by sex and country were extracted from 2000 to 2021. IQRs wer

Obesity Epidemic in U.S. Prison Populations: A Meta-Analysis and Review of the Literature.

Limited existing evidence and health provider perceptions suggest that prevalence of obesity among incarcerated people residing in U.S. correctional institutions is high. Evaluating evidence of obesity and weight change during incarceration will allow for the determination of whether people are subject to weight gain during incarceration. A systematic review of three online databases, gray literature, and reference lists of articles of interest was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. The health needs of people leaving prison with a history of methamphetamine and/or opioid use.

INTRODUCTION: Methamphetamine use is more common than opioid use among prison entrants in some countries, including Australia, yet most research and policy focuses on opioid use. This suggests that traditional opioid-focused interventions are no longer appropriate for the majority of this group in countries such as Australia. To inform policy and practice, we compared socio-demographic characteristics and health needs of people leaving prison with a history of methamphetamine use and/or opioid use. METHODS: A cross-sectional survey of incarcerated adults administered the World Health Organiza
