The postpartum use of plasma exchange with fresh-frozen plasma was assessed in a group of seven women with severe preeclampsia-eclampsia and HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count) that persisted greater than 72 hours after delivery. Aortic dissection is a common complication of Marfan's syndrome. We report such a patient who survived two separate dissections which both occurred shortly after uneventful pregnancies. The first dissection was a self-limiting DeBakey type III but the second commenced in the left common iliac artery and extended retrogradely into the lower abdominal aorta. This latter type of dissection does not appear to have been previously reported.

A case of a 69-year-old woman with postpartum hypopituitarism (Sheehan's syndrome) associated with congestive heart failure and severe hyponatremia is reported. She developed congestive heart failure after cholecystectomy, and marked improvement was noted by treatment with oxygen, digoxin, furosemide, and dopamine. This case report describes a false-positive hepatobiliary scan in a young woman suspected to have acute cholecystitis who apparently had none of the reasons stated in the literature for a false-positive scan. The literature review shows that the negative predictive value of hepatobiliary scanning for acute cholecystitis is nearly 100 percent, while the positive predictive value is also quite good if conditions known to cause false-positive scans are ruled out. A post-traumatic hematoma may present either acutely as a presacral collection or chronically as a retrorectal mass. A case of postpartum presacral hematoma and its management in the acute setting is described. A case of a chronic, calcified hematoma presenting as an asymptomatic retrorectal mass is also reported. CT scanning of the presacral space is the most accurate means of preoperative diagnosis. Therapy must be tailored to the preoperative diagnosis and to the size of the lesion.

Lumbar epidural analgesia was performed in a 28 year old primigravida through a L2-L3 catheter. The first dose induced satisfactory analgesia for 1 h. After the second dose of 8 ml of 0.25 bupivacaine, the patient's systolic blood pressure fell from 110 to 80 mmHg. Thirty-nine gravidae with congestive cardiac failure and ten pregnant patients with acute pulmonary edema treated at the Ahmadu Bello University Teaching Hospital in Kaduna, Nigeria, over a five-year period are presented. Congestive cardiac failure occurred more frequently in the postnatal period, and parity and socioeconomic class were not significantly related to cardiac failure.
