[Transparency of results in assisted reproductive technology].

Clear results in ART need to take into account the technical performances of the centre but also patients selection, cycles cancelling and embryo transfer policies. The risk of a partial transparency is an inappropriate selection of patients. From classical criterias like pregnancies per oocytes retrievals or embryo transfers we are heading towards more global indicators like live births of singletons by initiated cycles rate.

Endometrial receptivity: clinical assessment in relation to fertility, infertility, and antifertility.

Fertility in humans and other mammalian species depends absolutely on synchronous events that render the developing blastocyst and the receiving uterus competent for implantation. Endometrial receptivity is defined as the period during which the endometrial epithelium acquires functional, but transient, ovarian steroid-dependent status supportive to blastocyst acceptance and implantation. Once inside the uterus, the blastocyst is surrounded by an intact luminal epithelium, which is considered to act as barrier to its attachment, except for this short period of high endometrial receptivity to b

Predicting the impact of in vitro fertilisation and other forms of assisted conception on perinatal and infant mortality in England and Wales: examining the role of multiplicity.

The increased risk of perinatal and infant mortality observed among in vitro fertilisation (IVF) births and other assisted conception births is thought to be largely attributable to multiplicity. Using mortality statistics and estimates of the proportion of births following infertility treatment, we predicted the excess stillbirths and infant deaths associated with twins and higher order births resulting from assisted conception in England and Wales. According to our results, approximately 73 deaths could have been avoided in 2001 if all IVF infants had been born as singletons or as naturally

Prevention of multiple pregnancy following IVF in Spain.

Since the development of assisted reproduction techniques most countries have witnessed increased rates of multiple pregnancy. Despite the guidelines proposed by various scientific societies these rates continue to be abnormally high. In Spain, as in other Mediterranean countries, a greater number of embryos are transferred than in northern and central European countries and the incidence of multiple pregnancies is greater in comparison. Effective strategies must be established to prevent multiple pregnancy without reducing overall pregnancy rates. In the authors' institute, taking into accoun

Xenotransplantation: a tool for reproductive biology and animal conservation?

The transplantation of reproductive organs, including ovaries and ovarian tissue, was pioneered over 100 years ago. In the 1960s, ovarian grafting was used as a tool to investigate ovarian function, but with the recent development of more effective cryopreservation protocols for ovarian tissue, germline preservation and propagation have now become realistic goals. This review describes progress in ovarian banking and ovarian tissue transplantation, with emphasis on how fresh and frozen ovarian tissue can be used in assisted reproduction for both humans and animals. This paper focuses most clos

Spontaneous abortion among pregnancies conceived using assisted reproductive technology in the United States.

OBJECTIVE: To examine rates and risk factors for spontaneous abortion among pregnancies conceived using assisted reproductive technology (ART). METHODS: Subjects were 62,228 clinical pregnancies resulting from ART procedures initiated in 1996-1998 in US clinics. Spontaneous abortion was based on ART clinic report and was defined as loss of the entire pregnancy. Spontaneous abortion rates for ART pregnancies were compared with spontaneous abortion rates from the National Survey of Family Growth, a population-based survey of US women 15-44 years. RESULTS: The spontaneous abortion rate among AR

Unsupervised Deep Learning based Longitudinal Follicular Growth Tracking during IVF Cycle using 3D Transvaginal Ultrasound in Assisted Reproduction.

Longitudinal follicle tracking is needed in clinical practice for diagnosis and management in assisted reproduction. Follicles are tracked over the in-vitro fertilization (IVF) cycle, and this analysis is usually performed manually by a medical practitioner. It is a challenging manual analysis and is prone to error as it is largely operator dependent. In this paper we propose a two-stage framework to address the clinical need for follicular growth tracking. The first stage comprises of an unsupervised deep learning network SFR-Net to automate registration of each and every follicle across the

Anatomical stages of penile erection in the agouti (Dasyprocta leporina) induced by electro-ejaculation.

The reproductive system of the male agouti is not well documented. This study describes the specific anatomical features of the free part of the penis occurring during penile erection in the agouti. Electro-ejaculation was used to induce erection in three male agoutis that had previously produced offspring. Results proved that there were four stages in the erection process. Stage 1 involved protrusion of the penis from the preputial orifice. The lateral penile cartilages were then spread (stage 2). During stage 3, there was the blooming of the head of the glans penis (penile flower) and eversi

Fertility preservation for social indications: a cost-based decision analysis.

OBJECTIVE: Age-related infertility remains a problem that assisted reproductive techniques (ART) have limited ability to overcome. Correspondingly, because an increasing number of women are choosing to delay childbearing, fertility preservation strategies, initially intended for patients undergoing gonadotoxic therapies, are being applied to this group of healthy women. Studies supporting the effectiveness of this practice are lacking. DESIGN: Decision analytic techniques. SETTING: We compared the cost-effectiveness of three strategies for women planning delayed childbearing until age 40: oo

Beyond metabolic comorbidity: metabolic health stratification reveals BMI-ART associations in tubal factor infertility.

BACKGROUND: The association between body mass index (BMI) and assisted reproductive technology (ART) outcomes remains inconsistent, potentially due to confounding by metabolic comorbidities and underlying infertility etiologies. This study introduced metabolic health stratification to reproductive medicine to examine whether the impact of BMI on ART outcomes differs by metabolic status, independent of ovarian reserve impairment or reproductive endocrinopathies. METHODS: This retrospective cohort study included 3,770 women with tubal factor infertility and normal ovarian reserve who underwent

Microbiota and infertility: a translational review of mechanisms and clinical applications in assisted reproduction.

Infertility constitutes a major global health concern, affecting approximately 17.5% of couples of reproductive age. Although advances in assisted reproductive technologies (ART) have expanded treatment options, success rates remain highly variable due to host-specific and biological determinants. This review synthesizes current evidence on the reproductive impact of the human microbiota and its translational relevance to ART outcomes. Vaginal microbial communities dominated by Lactobacillus, particularly L. crispatus, are associated with improved conception and implantation, whereas genital o

Assessing the epigenetic consequences of ART in male infertility: what risks to embryo and offspring health?

Male infertility affects approximately 7 to 8% of men worldwide and is influenced by both genetic and epigenetic factors. Epigenetic mechanisms - including DNA methylation, histone modifications, and non-coding RNAs - are essential for spermatogenesis, sperm function, and early embryonic development. Disruptions in these processes can impair fertility and embryo viability. Assisted reproductive technologies (ART), particularly intracytoplasmic sperm injection (ICSI), have enabled conception in previously untreatable cases; however, by bypassing natural selection barriers, they may facilitate t

Epitranscriptomic modifications in embryonic development: insights into natural and ART-induced mechanisms and implications.

BACKGROUND: Mammalian embryo development involves a complex process governed by multiple layers of cellular and molecular regulation mechanisms. ART is widely used around the world to assist fertility in humans, with ∼12 million babies being born by ART in the last 40 years. These technologies are also used extensively for reproductive purposes in other mammalian species that have many analogies with human reproductive biology. Epitranscriptomic marks, including RNA modifications such as N6-methyladenosine (m6A) and N1-methyladenosine (m1A), modulate gene expression during gametogenesis and em

Lifestyle factors impact sperm microbiota and are associated with biological and clinical issues in infertile patients undergoing assisted reproductive technologies, a proof-of-concept study.

Sperm microbiota originates from local (urogenital, intestinal, cutaneous) and partner microbiomes, influenced by sexual activity. Its composition and diversity may impact male fertility, but research remains limited. This study aimed to investigate the relationship between lifestyle factors, sperm microbiota composition, and fertility outcomes. A monocentric prospective study was conducted on men undergoing assisted reproductive technology at Poitiers University Hospital between May and December 2021. Samples were analysed using high-throughput sequencing and bioinformatic pipelines to assess

Investigating the effect of preconception untreated subclinical hypothyroidism (TSH above normal range and below 6.0 mIU/L) on assisted reproductive technology (ART) outcomes.

Subclinical hypothyroidism (SCH) is a biochemical diagnosis with ongoing uncertainty regarding management in reproductive medicine. We performed a retrospective cohort study evaluating fertility and pregnancy outcomes in women undergoing assisted reproductive technology (ART), focusing on untreated preconception SCH with TSH up to 6.0mIU/L. Secondary analysis used data from women screened for thyroid function and thyroid peroxidase antibodies (TPOAb) during the TABLET trial at Birmingham Women's Hospital, UK (June 2012-December 2013). Women with TSH ≥6.0mIU/L received levothyroxine. The primar

Seizure Disorders in Adults: Principles of Treatment of Epilepsy.

Male fertility concerns include prevention of pregnancy and planning for conception, including management of infertility. Male-directed contraception is currently limited to condom use or vasectomy. Vasectomy is a safe, office-based procedure that reliably results in sterility. Several options for reversible male contraception are currently under investigation. Use of high-dose testosterone with or without progestin and use of new androgens provide several potential pathways for reversible male contraception. Reversible occlusion of the vas deferens may provide a nonhormonal option for male co

Rethinking cytomegalovirus screening in gamete donation: a critical appraisal of practices and risks.

In the context of gamete donation, cytomegalovirus (CMV) serological screening was originally implemented to mitigate concerns about viral transmission, particularly following the HIV epidemic. Although emerging evidence questions the reliability of serological markers in accurately reflecting the presence of the virus in genital secretions, current North American practices still heavily rely on CMV serological testing to identify sperm donors at risk of transmitting CMV during fertility treatments. This review explores the complexities of CMV transmission in assisted reproductive technologies
