Scaphoid excision and 4-corner fusion using retrograde headless compression screws.

Scapholunate advanced collapse is a predictable form of wrist arthritis resulting from longstanding scapholunate instability. Four-corner fusion and scaphoid excision is a reliable procedure used to treat scapholunate advanced collapse wrist that improves pain and preserves range of motion. Multiple methods of achieving fixation have been described for the procedure including K-wires, staples, and headless compression screws. In previously described techniques, the compression screws are inserted in an antegrade manner, breaching the articular surface of the lunate. Even small areas of chondra

Biomechanical comparison of two headless compression screws for scaphoid fixation.

This study compared compression generation between two headless compression screws: the Synthes 3.0-mm and the Acutrak standard. Twenty scaphoids were harvested from 10 pairs of fresh cadaveric forearms. A washer-shaped load cell was inserted between the halves of each scaphoid created by a simulated fracture via osteotomy. One scaphoid of each pair was tested with the Synthes and the other with the Acutrak. Parameters of interest were peak screw torque and fracture site compression. Differences in parameters of interest were checked for significance (p < .05) with paired t tests. No significa

Interfragmentary compression forces of scaphoid screws in a sawbone cylinder model.

Various screws have been developed to stabilise fractures of the scaphoid. Commonly used are the Herbert, the HBS, the 3-mm AO and the Acutrak screws. Not long ago a new screw, the Twin Fix, was introduced. This is cannulated and similar in shape and appearance to the classical Herbert screw. In our test series we compared the maximum achievable compression forces of the Twin Fix screw with that of three other screws (AO, HBS and Acutrak screws). To avoid the variations of density, stiffness and rigidity in natural bone, a polyurethane sawbone-based test setup was used. The test series include

[The effect of inactivity-induced osteoporosis on the scintigraphic image of bone lesions of the wrist].

In addition to its established oncological indications the sensitivity of bone scintigraphy is of steadily increasing significance in traumatology. Inactivity-induced osteoporosis plays a major role during the immobilization period in the plaster cast. In the region of the joints remodelling intensity may reach such a high level that the non-injured bone shows a higher rate of accumulation than the fracture. This process already begins between the third and fourth week of immobilization. The highest uptake is found after fracture of the scaphoid bone at the end of twelve weeks of immobilizatio…

Restoring carpal alignment: Long-term outcomes of modified Fisk-Fernandez technique in scaphoid nonunion.

BACKGROUND: Scaphoid nonunion remains a challenging condition due to its tenuous vascularity and complex wrist biomechanics, often resulting in deformity, instability, and progressive degenerative changes. The modified Fisk-Fernandez technique combines structural bone grafting with stable fixation to address both biological and mechanical factors. METHODS: This retrospective cohort study included 82 patients with scaphoid waist nonunion treated between 2017 and 2026. Clinical outcomes were assessed using QuickDASH and Visual Analog Scale (VAS) scores, while radiological evaluation included sc

[Computerized tomography of the hand with high resolution technique].

The results of 175 high-resolution computed tomography (CT) scans of the hand and wrist are analysed and compared to the results of conventional radiographs. Advantages of the procedure can be demonstrated when assessing pathological changes within the complex three-dimensional structure of the hand and wrist, and in the analysis of articular surfaces. In some cases, fractures and dislocations of the distal radioulnar joint and the carpal bones were missed in conventional radiographs. In cases of scaphoid fracture and nonunion, CT scans give valuable additional information surpassing the possi

Surgical management and imaging evaluation of post-traumatic periscaphoid osteoarthritis (SLAC and SNAC wrists).

An untreated scapholunate interosseous ligament injury or scaphoid nonunion leads to progressive post-traumatic periscaphoid osteoarthritis and carpal collapse. Symptomatic forms, known as scapholunate advanced collapse (SLAC) or scaphoid nonunion advanced collapse (SNAC), are common and disabling, requiring surgical management. Numerous operative options exist depending on the stage of collapse and the extent of joint involvement. We apply a modified Watson radiological classification consisting of four stages of pancarpal collapse, integrating the etiological origin (SLAC or SNAC) and the ar

Current Trends in 4-Corner and 3-Corner Fusions: Comparing Open and Arthroscopic Approaches.

The 4-corner and 3-corner fusions are established treatments for scapholunate and scaphoid nonunion advanced collapse arthritis, effectively alleviating pain while maintaining partial mobility. Traditionally performed via open techniques, advancements in arthroscopy now allow for minimally invasive approaches that may prove to reduce complications and accelerate recovery. Various fixation methods, including compression screws, dorsal plates, staples, and Kirschner wires influence stability and outcomes. Although arthroscopic approaches show promise in preserving motion and grip strength, long-

Preiser's Disease-Current Concepts of Etiology and Management.

The term Preiser's disease typically is used to describe idiopathic avascular necrosis of the scaphoid, but there have been a number of putative etiologies considered. It is rare and the natural history is not fully understood. Management of the condition should be based on patient factors as well as the stage of disease with regard to the scaphoid and the surrounding wrist. This chapter appraises the available evidence and aims to provide the reader with a framework to manage this rare condition.

Scaphoid Fractures in Athletes.

Scaphoid fractures are common injuries in athletes. Most can be treated with cast immobilization, with an expected rate of union of 90% to 95%. Cast treatment, however, has the disadvantages of longer immobilization time, joint stiffness, reduced grip strength, and longer time to return to manual work or athletics. Closed reduction and percutaneous screw fixation generally are preferred in athletes to allow a quicker return to sport; if closed reduction cannot be obtained, open reduction and internal fixation may be required.

The role of magnetic resonance imaging in the evaluation of scaphoid fractures.

Radiographs are currently accepted as the first-line modality for the investigation of scaphoid fractures. Early use of magnetic resonance imaging (MRI) in patients with radio-occult scaphoid fractures has been shown to be beneficial in the management of such patients. Incorporation of early MRI into the management protocol for scaphoid fracture is recommended.
