Decision Making of the In-season Athlete with Anterior Shoulder Instability.

In-season management of anterior shoulder instability in athletes is a complex problem. Athletes often wish to play through their current season, though recurrent instability rates are high, particularly in contact sports. Athletes are generally considered safe to return to play when they are relatively pain-free, and their strength and range of motion match the uninjured extremity. If an athlete is unable to progress toward recovering strength and range of motion, surgical management is an option, though this is often a season-ending decision.

Arthroscopic management of the contact athlete with instability.

The shoulder is the most commonly dislocated joint in the body, with a greater incidence of instability in contact and collision athletes. In contact and collision athletes that have failed nonoperative treatment, the most important factors to consider when planning surgery are amount of bone loss (glenoid, humeral head); patient age; and shoulder hyperlaxity. Clinical outcomes, instability recurrence rate, and return to sport rate are not significantly different between arthroscopic suture anchor and open techniques. Lateral decubitus positioning with distraction and four portal (including se

Arthroscopic repair of a chondrolabral lesion associated with anterior glenohumeral dislocation.

Chondrolabral lesions are uncommon after anteroinferior glenohumeral dislocations. This report describes a new dual-lesion complex that involved an avulsion of the anteroinferior glenoid labrum and a flap tear of the adjacent articular cartilage [glenoid labral tear and articular cartilage flap (GLAF) lesion]. The chondral component involved a large undermined region of the anterior half of the lower glenoid articular cartilage, and the labral component involved an avulsion from the 2.30-6 o'clock position on the glenoid. The labral tear was reconstructed with 3 suture anchors to form a neo-la

Post-traumatic glenohumeral cartilage lesions: a systematic review.

BACKGROUND: Any cartilage damage to the glenohumeral joint should be avoided, as these damages may result in osteoarthritis of the shoulder. To understand the pathomechanism leading to shoulder cartilage damage, we conducted a systematic review on the subject of articular cartilage lesions caused by traumas where non impression fracture of the subchondral bone is present. METHODS: PubMed (MEDLINE), ScienceDirect (EMBASE, BIOBASE, BIOSIS Previews) and the COCHRANE database of systematic reviews were systematically scanned using a defined search strategy to identify relevant articles in this fi

[Functional outcome of Eden-Hybinette-Lange operation in post-traumatic recurrent shoulder dislocation].

OBJECTIVE: Post-traumatic recurrent dislocation is a major problem in the operative treatment of shoulder dysfunction in young athletes. This prospectively designed study evaluates the long-term results of a modified Eden-Hybinette procedure in young male athletes. The criteria were: capacity in sports, functional limit, and persistent pain. Genuine disorders of the glenoid or muscular imbalance of the shoulder joint were criteria for exclusion. PATIENTS AND METHODS: From 1982 to 1990, 143 patients underwent surgery. Seventy percent were reevaluated within a minimum period of 18 months after

[Shoulder arthroscopy in degenerative and inflammatory diseases].

A new concept of shoulder arthroscopy is presented based on experience with 800 shoulder arthroscopies. All open surgical procedure on the shoulder are preceded by arthroscopy. The procedure is performed with the patient lying on his or her side. The medium is CO2, and the advantages are less bleeding, a natural picture, no fluid edema. After diagnostic arthroscopy (with CO2), operative arthroscopy (with fluid) or open joint surgery follows. The surgery can easily be performed in the same position for good access with any approach. Preliminary diagnostic arthroscopy helps to plan the procedure

Tragic result of traditional Chinese medicine manipulation: an unusual case report of bilateral anterior shoulder dislocations.

BACKGROUND: In Taiwan, there is a good universal healthcare system to the patients; however, the majority of Taiwanese seek the complementary and alternative medicine when they are injured or ill. The traditional Chinese medicine, which is a branch of complementary alternative medicine, is prevalent in Taiwan. Without proper sequence of maneuvers, either traditional Chinese medicine or conventional medicine might cause unexpected complications. We report a case of 76-year-old woman who was manipulated by a bonesetter, leading to bilateral anterior shoulder dislocations. To the authors' best kn

Injuries Depicted in Sport-Related Films.

The perception that children and adolescents have of injuries may be influenced by their depiction in sport-related films. The objective of this study was to determine the depiction of injuries in a select number of sport-related films. Forty-four films were analyzed. Injury category, on-the-field management, and long-term management were determined for each injury. Major injuries were defined as injuries requiring prompt intervention and immediate discontinuation of sport participation. A total of 49 injuries were depicted, categorized as fracture (n = 11), contusion (n = 10), brain concussio

Effectiveness of a sport-specific exercise programme for overhead athletes with unilateral subacromial shoulder pain: a study protocol of a randomised controlled trial.

Subacromial shoulder pain is one of the most common musculoskeletal conditions affecting overhead athletes, particularly those engaged in high-intensity training modalities such as CrossFit. The prevalence of shoulder injuries in this population continues to rise due to increased participation, repetitive overhead movements, and biomechanical stress on the rotator cuff and scapular stabilisers. While various rehabilitation approaches exist, no consensus exists on the most effective exercise intervention for improving shoulder function and pain outcomes in athletes with subacromial shoulder pai…

Recovery of shoulder abduction in traumatic brachial plexus palsy: a systematic review and meta-analysis of nerve transfer versus nerve graft.

Brachial plexus palsy is a surgically manageable condition. Re-animating the shoulder is a high priority for restoring upper extremity function. Methods for reinnervating injured nerves include the transfer of a healthy nerve or fascicle distal to the site of injury, or grafting a healthy sensory nerve to restore motor function. Studies aiming to compare these two techniques for restoring shoulder abduction have yielded conflicting results. We conducted a systematic review and meta-analysis following the PRISMA guidelines. We reviewed the PubMed database for studies comparing nerve transfer an

Overuse and impingement syndromes of the shoulder in the athlete.

Overuse and impingement syndromes in the shoulders of athletes are predominantly caused by instability of the glenohumeral joint. Glenohumeral joint instability is usually acquired from repetitive overuse of the rotator cuff and shoulder girdle muscles, or injury of the static and dynamic stabilizers of the glenohumeral joint. Congenital hypermobility of the joint may also contribute to these syndromes in some individuals. The throwing action may lead to a cascade of injuries to the static and dynamic stabilizers of the posterosuperior glenohumeral joint, caused by the repetitive, high-energy

Nonpenetrating subclavian artery trauma.

Nonpenetrating injury to the subclavian vessels is uncommon. During a 6-year period we have treated 167 patients with injuries to the subclavian and superior mediastinal arteries. Fifteen of these injuries (9%) occurred after blunt trauma. In 10 patients the proximal segment (first and second parts) of the artery was involved. No patient had an isolated injury; the most frequent associated injuries were rib fractures (n = 11), with the first rib being involved in four of these. Total brachial plexus disruption was found in nine patients. All patients with distal artery involvement had a clavic

Upper extremity neuromuscular function can distinguish between individuals with and without glenohumeral labral repair.

The purpose of this study was to determine whether common measures of neuromuscular function could distinguish injury status indicated by group membership (glenohumeral labral repair, uninjured controls). 16 individuals with glenohumeral labral repair (24.1 ± 5.0 years, 36.7 ± 33.3 months after surgery) and 14 uninjured controls (23.8 ± 2.7 years) volunteered. We measured mass-normalized shoulder abduction and wrist flexion maximal voluntary isometric contraction torque (Nm/kg), motoneuron pool excitability of the flexor carpi radialis (Hoffmann reflex), corticospinal excitability of the upper

A novel technique for proximal interphalangeal joint fracture-dislocations: the second toe middle phalanx osteochondral graft.

Many different surgical techniques have been used to treat unstable dorsal proximal interphalangeal joint fracture-dislocations. The authors have used the base of the middle phalanx of the second toe base as an alternative autograft to treat this type of injury. This retrospective study assessed the clinical outcomes of this procedure in 11 patients. Range of motion, grip strength, Disability of the Arm, Shoulder and Hand score and donor site morbidity were assessed at regular intervals postoperatively. Nine patients had acute injuries and two had chronic injuries. The mean range of motion in

Spontaneous vaginal delivery: a risk factor for Erb's palsy?

OBJECTIVE: Our purpose was to determine whether Erb's palsies occurring in the absence of shoulder dystocia differ from those occurring after shoulder dystocia. STUDY DESIGN: We compared the time course of resolution and incidence of persistent injury at 1 year of age in 17 cases of Erb's palsy without shoulder dystocia and 23 cases associated with shoulder dystocia. RESULTS: The rate of persistence at 1 year was significantly higher in those Erb's cases without identified shoulder dystocia, 7 of 17 (41.2%) versus 2 of 23 (8.7%), p = 0.04. These cases of Erb's palsies also took longer to res
