Dynamic Contrast-Enhanced MRI Parameters and Normalized ADC Values Could Aid Differentiation of Skull Base Osteomyelitis from Nasopharyngeal Cancer.

BACKGROUND AND PURPOSE: The skull base osteomyelitis sometimes can be difficult to distinguish from nasopharyngeal cancer. This study aimed to investigate the differences between skull base osteomyelitis and nasopharyngeal cancer using dynamic contrast-enhanced MR imaging and normalized ADC values. MATERIALS AND METHODS: This study included 8 and 12 patients with skull base osteomyelitis and nasopharyngeal cancer, respectively, who underwent dynamic contrast-enhanced MR imaging and DWI before primary treatment. Quantitative dynamic contrast-enhanced MR imaging parameters and ADC values of the

Analysis of Surgical Approaches to Skull Base Tumors Involving the Pterygopalatine and Infratemporal Fossa.

Selecting an appropriate surgical approach for resection of huge skull base tumors involving pterygopalatine and infratemporal fossa is challenging because of their rarity and high possibility of vital anatomical structure injuries. To suggest the guidance of selecting the appropriate approach by analyzing outcomes and satisfactions of known surgical approaches with our previous experience, the authors retrospectively analyzed skull base tumor cases experienced for 24 years, and condensed to 4 well-known surgical approaches: maxillary swing, infratemporal fossa type C, transzygomatic, and a co

MRI Tractography Detecting Cranial Nerve Displacement in a Cystic Skull Base Tumor.

A 37-year-old man came to our neurosurgical department with a 2-month history of a progressive invalidating balance disorder. Cerebral magnetic resonance imaging found a T2-weighted hypersignal lesion of the right cerebellopontine angle that severely compressed the brainstem, however the position of cranial nerves was not clearly identified. The new MRI diffusion tool, tractography, allowed to reconstruct the trajectory of cranial nerves that were displaced by the tumor. As such, the acoustic facial bundle was severely flattened posteriorly and superiorly, while the lower nerves were pushed in

Transsphenoidal Approach in Endoscopic Endonasal Surgery for Skull Base Lesions: What Radiologists and Surgeons Need to Know.

In the last 2 decades, endoscopic endonasal transsphenoidal surgery has become the most popular choice of neurosurgeons and otolaryngologists to treat lesions of the skull base, with minimal invasiveness, lower incidence of complications, and lower morbidity and mortality rates compared with traditional approaches. The transsphenoidal route is the surgical approach of choice for most sellar tumors because of the relationship of the sphenoid bone to the nasal cavity below and the pituitary gland above. More recently, extended approaches have expanded the indications for transsphenoidal surgery

Clival chordoma manifesting as nasal bleeding. A case report.

Chordoma is a rare cartilaginous tumor, for which bleeding presentation is unusual. We report a case of rare hemorrhaged clival chordoma, which was diagnosed correctly by magnetic resonance imaging. A 32-year-old man presented with nasal bleeding. The tumor was totally removed via a trans-sphenoidal approach, from which the surgical specimen confirmed chordoma. Epistaxis seemed to be caused by the spreading of the intratumoral hemorrhage into the sphenoid sinus. This case demonstrates the importance of an exact differential diagnostic evaluation, including chordoma, by use of modern imaging te

Interstitial brachytherapy for skull base tumors.

1. Irrespective of the histology, all tumors responded to 125I permanent BT. 2. There were no intra- or postprocedural complications, making this modality extremely safe. 3. There were no acute or late complications attributable to continuous LDR radiation with 125I permanent BT. 4. The dose distribution can be tailored to fit the tumor shape by seed placement at the desired location and by selecting the seed strength. 5. Unlike any other radiation therapy procedure, with SBT, the placement of the 125I seed or seeds is verifiable at each step of the procedure on the CT screen, making the treat

Aggressive evolution of an occipital bone ABC-like lesion with dural invasion and intradural extension: a case report.

BACKGROUND: Aneurysmal bone cysts (ABCs) of the skull are rare, particularly those arising in the occipital bone. Intracranial extension or dural destruction is extremely uncommon. Here, we report an occipital ABC-like lesion that recurred 19 months after gross-total resection with dural defects, intradural extension, and molecular evidence supporting aggressive neoplastic evolution. CASE DESCRIPTION: A 37-year-old woman presented with 2 months of headache and 1 month of binocular diplopia. CT and MRI demonstrated a large expansile lytic lesion of the occipital bone with poorly defined interf

Surgery of the Anterior Infratemporal Fossa (AITF) and the Pterygopalatine Fossa (PPF) lesions: critical assessment of operative exposure, loss of functioning tissue and morbidity between Extradural Subtemporal Infratemporal Approach (ESITA) and Endonasal Endoscopic Transmaxillary Transpterygoid Approach (EEMP).

BACKGROUND: Surgery of the anterior infratemporal fossa (AITF) and pterygopalatine fossa (PPF) is technically demanding. The Extradural Subtemporal Infratemporal Approach (ESITA) and the Endonasal Endoscopic Transmaxillary Transpterygoid (EEMP) Approach are established routes for accessing these regions. This study quantitatively compares the surgical exposure, surgical freedom, angles of attack, and clinical outcomes achieved with these approaches. METHODS: ESITA and EEMP were performed on ten sides of five cadaveric heads. Resection volumes were calculated using 3D reconstructions from thin

Anatomical determinants of occipitocervical fusion in skull base chordoma resection: a systematic review of the literature with illustrative cases.

OBJECTIVE: Skull base chordomas are rare, locally osseo-destructive lesions that present unique surgical challenges due to their involvement of critical neurovascular and bony structures at the craniovertebral junction (CVJ). Radical cytoreductive surgery improves survival but also carries significant morbidity, including the potential for occipitocervical (OC) destabilization requiring instrumented fusion. The published experience on OC fusion after CVJ chordoma resection is limited, and the anatomical predictors of OC instability in this context remain unclear. METHODS: PubMed and Embase we

A New Technique for the Endoscopic Reconstruction of Skull Base Defects Using Multiple-balloon Catheters.

Cerebrospinal fluid (CSF) leakage is a major complication following endoscopic endonasal skull base surgery. Various skull base reconstruction methods are available, and the use of a vascularized nasoseptal flap (NSF) in skull base reconstruction has greatly contributed to a decrease in the CSF leak rate. A balloon catheter such as a sinus balloon or a Foley catheter is often used to support an NSF; however, in cases wherein nasal and/or paranasal structures supporting the balloon are lacking following the surgery, the NSF is not properly fixed and postoperative CSF leak may occur. Here we int

How I do it: endonasal transcribriform approach for resection of esthesioneuroblastoma.

BACKGROUND: Olfactory neuroblastoma, also known as esthesioneuroblastoma, accounts for only 3-6% of sinonasal malignancies but confers a 40% 5-year overall survival. METHOD: The authors describe techniques for the endonasal, minimally invasive resection of an esthesioneuroblastoma in a 69-year-old man who presented with headaches and anosmia and describe surgical nuances and their effect on adjuvant therapy planning. CONCLUSION: This approach, along with microsurgical techniques, helped increase tumor visualization, improved marginal resection, and reduced surgical risk, which may improve pa

Brain tissue oxygenation during transnasal endoscopic skull base procedures.

PURPOSE: We aimed to study brain tissue oxygenation during the period of controlled reduction of arterial blood pressure - a maneuver often used in extended endoscopic skull base surgery for bloodless operative field. METHODS: Intracranial pressure, arterial blood pressure and the resultant cerebral perfusion pressure were measured during extended endoscopic skull base surgery in 5 patients with diagnosed tumors of the skull base and arterial hypertension. Simultaneously, in those patients, we measured partial pressure of oxygen in the brain parenchyma (PbtO2). RESULTS: Values of PbtO2 lower

Modified Posterior Pedicle Middle Turbinate Flap: An Additional Option for Skull Base Resurfacing.

OBJECTIVES/HYPOTHESIS: Although the Hadad-Bassagusteguy flap represents the first choice for middle and posterior skull base reconstruction and coverage of exposed bony areas, in some cases it is unavailable. The aim of this study is to describe, as an alternative option in selected cases, a modified posterior pedicle middle turbinate flap (mPPMTF) extended to the lacrimal area. Anatomical features, step-by-step harvesting technique, and surgical applications are presented. STUDY DESIGN: Anatomic dissection study and case report. METHODS: Four mPPMTFs were raised in two fresh-frozen cadaver

Temporoparietal Fascia Flap Endonasal Transposition in Skull Base Reconstruction: Surgical Technique.

Reconstruction of wide skull base defects resulting from multimodal treatment of cranial base malignancies are challenging.1 Endonasal pedicled flaps (e.g., Hadad-Bassagusteguy flap)1,2 are generally the first choice; however, inadequacy for the size and location of the defect or their unavailability are common occurrences in salvage reconstructions, and prior irradiation is an additional unfavorable condition for local flap viability. The temporoparietal fascia flap (TPFF)3 is a regional flap vascularized by the superficial temporal artery, which is able to survive and integrate even in posti

The impact of expanded endonasal skull base surgery on midfacial growth in pediatric patients.

OBJECTIVE: Surgical resection of skull base tumors in children is increasingly accomplished through an expanded endonasal approach (EEA). We aim to evaluate the potential effect of the EEA on midfacial growth as a result of iatrogenic damage to nasal growth zones. METHODS: We performed a retrospective review of children undergoing craniopharyngioma resection via an open transcranial or EEA. Pre- and postoperative magnetic resonance imaging was evaluated for growth in four midfacial measurements based on established cephalometric landmarks: anterior midface height, posterior midface height, pa
