Cardiac Arrhythmias and Sleep Disorders.

Sleep disordered breathing (SDB), periodic limb movements of sleep, insomnia, sleep curtailment, and sleep architectural changes are associated with cardiac arrhythmias in community-based and clinical cohort studies. Findings appear to be strongest in magnitude and most consistent for SDB and atrial fibrillation (AF). The mechanisms most consistently identified in animal experimental models include autonomic nervous system fluctuations and intrathoracic pressure alterations, the latter resulting in direct mechanical effects on the thin-walled atria. Other studies point toward intermittent hypo

Interactions between sleep, inflammation, immunity and infections: A narrative review.

BACKGROUND: Over the past decades, it has become increasingly evident that sleep disturbance contributes to inflammation-mediated disease, including depression, mainly through activation of the innate immune system and to an increased risk of infections. METHODS: A comprehensive literature search was performed in PubMed to identify relevant research findings in the field of immunity, inflammation and infections, with a focus on translational research findings from the past 5 years. RESULTS: Physiological sleep is characterized by a dynamic interplay between the immune system and sleep archit

Salivary Cortisol and Melatonin, Sleep, and Behavioral Patterns in Older Adults Living With Dementia.

BACKGROUND: Over half of the older adults living with dementia have behavioral and psychological symptoms of dementia (BPSD), including sleep disturbance; however, little is known about physiological markers. Salivary cortisol and melatonin have been identified as potential biomarkers of BPSD, with evidence suggesting a relationship between these biomarkers and various behavioral factors, as well as sleep and activity patterns. OBJECTIVES: The aim of this study was to investigate the time-dependent changes in salivary cortisol and melatonin levels in older adults with dementia, their relation

The Sleep Environment, Napping, and Sleep Outcomes among Urban Children With and Without Asthma.

OBJECTIVES: Children with asthma living in U.S. urban neighborhoods experience increased risk for asthma morbidity and poor sleep outcomes. In addition to asthma, environmental factors (e.g. noise, uncomfortable temperature, light exposure) related to urban poverty may disturb children's sleep. This study examined the association between environmental factors and sleep outcomes among urban children with and without asthma, and whether napping underlies the environment-sleep link. Additionally, the study tested whether these associations differed by health status (i.e. asthma) or race/ethnicity

Dual orexin receptor antagonists for the treatment of insomnia: systematic review and network meta-analysis.

BACKGROUND: Several randomized clinical trials (RCTs) have shown that dual orexin receptor antagonists (DORAs) are effective in the treatment of chronic insomnia. However, the superiority of one particular DORA over the others remains unclear. OBJECTIVE: To perform a network meta-analysis to evaluate the efficacy of different DORAs in patients with chronic insomnia. METHODS: The Medline, Embase, and Cochrane Central databases were searched for RCTs that compared DORA with placebo in patients ≥ 18 years of age with a diagnosis of insomnia disorder. We pooled outcomes for wake time after sleep

Using a Non-Wearable Actigraphy in Nursing Care for Dementia With Lewy Bodies.

People who have dementia with Lewy bodies often have sleep disorders. We used non-wearable devices to record and categorize the sleep patterns of patients with Lewy body dementia. Individual sleep data at a dementia-care unit in Japan were recorded using non-wearables. One week's worth of data from 18 patients was analyzed. Median metrics for all participants were the following: sleep efficiency, 68% (23-89); sleep duration at night, 6.8 hours (1.6-11.1); times getting out of bed at night, 3.5 (0-13). We identified three types of abnormal sleep: extremely short sleep duration, excessive sleep

A systematic review of the validity of non-invasive sleep-measuring devices in mid-to-late life adults: Future utility for Alzheimer's disease research.

Changes in sleep during mid-to-late life are associated with risk for Alzheimer's disease (AD). Mechanistic understanding of this association necessitates measurement tools able to quantify these sleep changes longitudinally and accurately. We conducted a systematic review with meta-analysis of validity studies of non-invasive sleep-measuring devices published since 2015 that record sleep metrics associated with AD in adults over 40 (mean 52.9, SD 6.1 years). We reviewed 52 studies, including 32 wearable and ten non-wearable single or multi-sensor devices validated against polysomnography (min

Clinical applications of artificial intelligence in sleep medicine: a sleep clinician's perspective.

BACKGROUND: The past few years have seen a rapid emergence of artificial intelligence (AI)-enabled technology in the field of sleep medicine. AI refers to the capability of computer systems to perform tasks conventionally considered to require human intelligence, such as speech recognition, decision-making, and visual recognition of patterns and objects. The practice of sleep tracking and measuring physiological signals in sleep is widely practiced. Therefore, sleep monitoring in both the laboratory and ambulatory environments results in the accrual of massive amounts of data that uniquely pos

Observational and experimental studies on sleep duration and aggression: A systematic review and meta-analysis.

Short sleep duration has been linked to higher levels of aggression. To synthetize all available research on this association, a systematic review and meta-analysis was performed. We included observational and experimental studies, using various measures of sleep duration and aggression. Eighty eligible papers were identified, describing 82 studies comprising a total number of 76.761 participants. Meta-analysis of results was possible for 60 studies. Pooled observational results on the association between sleep duration and aggression showed a correlation estimate of -0.16 (95%CI -0.19, -0.12;

Severe Obstructive Sleep Apnea Disrupts Vigilance-State-Dependent Metabolism.

The direct pathophysiological effects of obstructive sleep apnea (OSA) have been well described. However, the systemic and metabolic consequences of OSA are less well understood. The aim of this secondary analysis was to translate recent findings in healthy subjects on vigilance-state-dependent metabolism into the context of OSA patients and answer the question of how symptomatic OSA influences metabolism and whether these changes might explain metabolic and cardiovascular consequences of OSA. Patients with suspected OSA were assigned according to their oxygen desaturation index (ODI) and Epwo

Sleep Disturbance, Sleep Disorders and Co-Morbidities in the Care of the Older Person.

Sleep complaints can be both common and complex in the older patient. Their consideration is an important aspect of holistic care, and may have an impact on quality of life, mortality, falls and disease risk. Sleep assessment should form part of the comprehensive geriatric assessment. If sleep disturbance is brought to light, consideration of sleep disorders, co-morbidity and medication management should form part of a multifaceted approach. Appreciation of the bi-directional relationship and complex interplay between co-morbidity and sleep in older patients is an important element of patient
