Surgical treatment of post-traumatic vertebral osteonecrosis including intravertebral expansive implants-clinical, functional and imaging outcomes and a center experience over 7 years.

BACKGROUND: The effective treatment of post-traumatic vertebral osteonecrosis continues to be an under discussion and controversial subject. Armed kyphoplasty with expansive intravertebral implants is an emerging procedure, which, in theory, allow for a more effective preservation of the restored vertebral height. The development of the indications for these recent devices has given rise to auspicious outcomes in vertebral non-union situations. The aim of this study is to evaluate the clinical, functional and imaging outcomes of the surgical treatment of situations of post-traumatic vertebral

Decompression Tube Fixation on Adjacent Teeth for Cystic Lesions in the Jaws: A Long-Term Follow-Up Analysis of Predictive Factors.

Introduction Decompression is an effective conservative treatment for large odontogenic cysts; however, conventional tube stabilization often encounters issues such as device dislodgement or mucosal trauma. This study aimed to analyze the therapeutic efficacy and clinical outcomes of a novel decompression tube fixation method secured directly to adjacent teeth using stainless steel wire. Materials and methods We retrospectively reviewed 20 patients (11 males, 9 females; mean age: 41.5 years) with jaw cysts treated between 2013 and 2023. Quantification of cyst dimensions and two-dimensional (2D

Motorized Plate Lengthening of the Femur in Children: A Preliminary Report.

BACKGROUND: Limb lengthening by distraction osteogenesis can be performed with motorized internal devices, but intramedullary implants risk avascular necrosis of the femoral head in young children. A method of internal limb lengthening using a motorized expandable plate has been developed and preliminary results are presented. METHODS: Seven skeletally immature patients (ages 2.7 to 9.7 y) with congenital femoral deficiencies underwent femoral lengthening with the use of a magnetic expandable plate. Surgical details, lengthening parameters, Limb Lengthening and Reconstruction Society-Angular

Dynamic hip screw fixation of subtrochanteric femoral fractures.

INTRODUCTION: A subtrochanteric proximal femur fracture occurs in the 5 cm of bone immediately distal to the lesser trochanter. UK national guidelines advise that adults with subtrochanteric fractures should be treated with an intramedullary nail (IMN). This study aims to compare peri-operative outcome measures of patients with subtrochanteric fractures treated with either an IMN or a dynamic hip screw (DHS) construct. MATERIALS AND METHODS: We retrospectively reviewed subtrochanteric fractures presenting at our institution over 4.5 years (October 2014-May 2019), classifying them into two tre

Surgical technique and early outcomes of intramedullary nailing of displaced proximal humeral fractures in an Asian population using a contemporary straight nail design.

BACKGROUND: Despite being a common orthopedic injury, the optimal management of proximal humeral fractures remains a topic of debate. Although intramedullary nails have been used to treat these fractures, several complications have been reported with older nail devices. We describe our surgical technique and aim to evaluate the early functional and radiological outcomes of displaced Neer's two- to four-part proximal humeral fractures undergoing fixation with a contemporary straight intramedullary nail. METHODS: A prospective cohort study of 22 consecutive patients (mean age 55.7 ± 18.0) with

Operative Management of Periarticular Medial Clavicle Fractures-Report of 10 Cases.

BACKGROUND:: The periarticular medial clavicle fracture is a rare injury and can be treated conservatively in the majority of cases. However, up to 8% of the patients develop symptomatic nonunion, and fracture dislocation correlates with the number of poor functional results. Operative treatment may be beneficial in these cases. Studies with large series of operated patients are still missing. METHODS:: We investigated 10 patients with operative treatment of periarticular medial clavicle fractures. Preoperative X-ray or computed tomography scan was obtained, and follow-up assessment was perfor

Arthroscopically Assisted Stabilization of Chronic Acromioclavicular Joint Instability.

UNLABELLED: This video article demonstrates biological and synthetic acromioclavicular (AC) and coracoclavicular stabilization with use of a hamstring tendon graft and a low-profile TightRope implant (Arthrex). The low-profile TightRope reduces soft-tissue irritation due to knot stacks1. The tendon graft is wrapped around the clavicle and the coracoid to avoid weakening of the osseous structures as a result of clavicular and coracoidal tunnel placement2. DESCRIPTION: For this procedure, the patient is placed in the beach chair position. After establishing standard posterior, anteroinferior, a

Primary anterior lumbar interbody fusion, with and without posterior instrumentation: a 1,377-patient cohort from a multicenter spine registry.

BACKGROUND CONTEXT: Lumbar interbody instrumentation techniques are common and effective surgical options for a variety of lumbar degenerative pathologies. Anterior lumbar interbody fusion (ALIF) has become a versatile and powerful means of decompression, stabilization, and reconstruction. As an anterior only technique, the integrity of the posterior muscle and ligaments remain intact. Adding posterior instrumentation to ALIF is common and may confer benefits in terms of higher fusion rate but could contribute to adjacent segment degeneration due to additional rigidity. Large clinical studies

Symptomatic plate removal after treatment of facial fractures.

AIMS: To identify the rates and reasons for plate removal (PR) among patients treated for facial fractures. MATERIALS AND METHODS: A retrospective review of files of 238 patients. RESULTS: Forty-eight patients (20.2%) had plates removed. The reason for removal was objective in 33.3% and subjective in 29.2%. The most common subjective reason was cold sensitivity, and the most common objective reason was wound dehiscence/infection. Women had PR for subjective reasons more often than men (p=0.018). Removal was performed more often for subjective reasons after zygomatico-orbital fractures than a

The effectiveness of physician-directed external fixation pin site care in preventing pin site infection in a high-risk patient population.

Pin tract infection is one of the most frequently reported complications associated with the use of external fixation. Application of compression dressings to prevent motion at the pin-skin interface as well as periodic antiseptic cleansing of the pin sites has been advocated; however, no consensus has been reached on the most effective method of pin site care. This retrospective study was conducted to evaluate the effectiveness of a weekly, physician-directed pin site care protocol on reducing the rate of pin tract infections in a high-risk patient population.

Video-assisted ALIF with cage and anterior plate fixation for L5-S1 spondylolisthesis.

INTRODUCTION: Spondylolysis and spondylolisthesis grade 0, 1, and 2 are mainly asymptomatic but with aging process and different factors some back pain can occur and lead to chronic low back pain. The conservative treatment with physiotherapy and steroid injection is the gold standard but in some cases is not efficient enough and a surgical treatment is proposed. OBJECTIVES: The goal of this study is to propose a new technique to treat grade 0, 1, and 2 spondylolisthesis with an anterior video-assisted fusion and stabilization. METHODS: Twenty patients with chronic low back pain since more t

[Replacement of the vertebral body with an expansion implant (Synex)].

PURPOSE OF THE STUDY: This paper describes replacement of the vertebral body with the expansion implant Synex. Usually, autologous bone graft is used to replace the vertebral body. In patients with bone cancer or multiple injuries to the spine, cement filling is preferred whereas, in other indicated cases, implants are inserted, of which Harms' titanium cage has been the most common one. However, this needs filling with a large amount of bone tissue and it is often difficult to adjust its size into the space available. Telescopic devices, on the other hand, are easier to implant and their appl
