Trabeculotomy has received increasing interest as a glaucoma procedure not only in congenital glaucoma but also in adult-onset open-angle glaucoma. In this review, the development, surgical technique, mechanism of action, and results after trabeculotomy in different types of glaucoma are described. Combined trabeculotomy and trabeculectomy and combined cataract and trabeculotomy techniques are discussed. Some remarks are given on laser trabeculotomy ab interno and other developments of trabecular surgery.

BACKGROUND Overhanging filtering blebs (OFB) are a known complication of trabeculectomy, leading to visual disturbances, discomfort, and cosmetic concerns. Their management remains challenging, with high recurrence rates despite surgical interventions. This case report presents the clinical course, management, and outcome of an 18-year-old woman with an OFB who underwent bleb revision incorporating isopropyl alcohol as an adjunctive measure. CASE REPORT An 18-year-old woman with a history of trabeculectomy and prior bleb revision presented with a large, cystic, overhanging filtering bleb that …

Glaucoma surgery is performed to lower intraocular pressure (IOP); ideally, the IOP reduction is safely maintained for an extended period of time. Although trabeculectomy was considered the gold standard for glaucoma surgery for many years because of its effective IOP reduction, yet now it is considered unsafe because of serious complications. In recent years, minimally invasive glaucoma surgery (MIGS), which emphasizes safety and can be performed rapidly, has become widespread. Because MIGS does not involve conjunctival incisions, patients can undergo future trabeculectomy. If IOP reduction c…

OBJECTIVE: to study the efficacy and safety of the use of subconjunctival triamcinolone acetate alone or in combination with mitomycin C as a modulator of trabeculectomy healing in rabbits.
METHODS: we submitted thirty male, albino, New Zealand rabbits to bilateral trabeculectomy. We divided the animals into four experimental groups with 15 eyes per group: control, mitomycin C, triamcinolone acetate and triamcinolone acetate + mitomycin C. We performed aplanation tonometry and clinical analysis of the bleb through the Moorfields Graduation System in the postoperative period. For the evaluatio…

The revision of a failed filtering bleb using mitomycin C was performed in 40 eyes (28 eyes with primary open angle glaucoma and 12 eyes with secondary open angle glaucoma) in which the previous trabeculectomy using 5-fluorouracil had failed to control intraocular pressure (IOP). Presurgical IOP ranged from 21 approximately 36 mm Hg (average: 24.7 mmHg) with the maximum tolerable antiglaucoma medication. The follow-up period was at least 2 years (24 approximately 60, mean: 39.8 months). Mitomycin C (0.04%, 5 ml, 5 min) was applied under the subconjunctival space. After rinsing, the wall of the…

Trabeculectomy remains the 'gold standard' intraocular pressure (IOP)-lowering procedure for moderate-to-severe glaucoma; however, this approach is associated with the need for substantial post-operative management. Micro-invasive glaucoma surgery (MIGS) procedures aim to reduce the need for intra- and post-operative management and provide a less invasive means of lowering IOP. Generally, MIGS procedures are associated with only modest reductions in IOP and are targeted at patients with mild-to-moderate glaucoma, highlighting an unmet need for a less invasive treatment of advanced and refracto…

This surgery method is thought for cases, where previous operations have not been successful or for such, in which severe scaring tissue reactions must be expected. The diaphanoscopy of the anterior chamber angle is considered helpfull for the choice of the surgery way. Between 1975 and february 1978 trabeculectomy combined with radical lobelike iridencleisis and marginal cyclodialyse was performed in 4 cases and the i.o. pressure could be normalised. Complications did not occure.

A bleb infection and subsequent endophthalmitis developed in the left eye of a 68-year-old man who had had a trabeculectomy. Vitreous injections of vancomycin and gentamicin were given, and vitreous cultures grew alpha-Streptococcus. Postoperatively, as the intraocular inflammation resolved, intraocular pressure (IOP) markedly decreased secondary, in our judgment, to ciliary body ischemia. A pars plana vitrectomy relieved the tractional bands and restored the IOP.

PURPOSE: Late bleb leaks may follow months to years after filtering surgery especially with the use of antimetabolites. Complications related to beb leaks may lead to a decrease in visual acuity through complicated hypotony or ocular infection. Our retrospective study reports the anatomical and functional results of bleb reconstruction involving the resection of the bleb associated with the covering of the trabeculectomy site with a rotational conjunctival flap.

Hypotony is a common complication following trabeculectomy in which antimetabolites are used. Autologous blood injection is an accepted form of treatment for hypotony that occurs secondary to overfiltration; however, injection into the filtering bleb has been associated with a rise in intraocular pressure for some patients with chronic postoperative hypotony. The authors describe a patient in whom corneal blood staining with raised intraocular pressure and loss of vision occurred as a result of autologous blood injection.

Tube exposure is a recognised complication of glaucoma drainage devices (GDD). We report a case of successful repair of GDD tube exposure using a corneal patch graft and double-layer amniotic membrane in a visually precious eye with previous multiple surgeries. A 47-year-old man with juvenile open-angle glaucoma (JOAG) underwent trabeculectomy in both eyes (OU) and two GDD implantations in the right eye (OD). The left eye (OS) trabeculectomy failed, leading to panophthalmitis and evisceration. Six years after the second GDD in the OD, the superonasal tube became exposed (3.8 mm). Initial repai…
