Mind the gap: a quality improvement study to determine health care value of rapid-access telemedicine addiction treatment in recently incarcerated individuals.

BACKGROUND: Previously incarcerated individuals experience a significant and vulnerable time post-release, highlighted by a greater than 129 times higher risk of overdose death. Many factors contribute to this high mortality including lack of health insurance and medications for opioid use disorder (MOUD) prescriptions upon release. Telemedicine provisions for MOUD greatly expanded access to MOUD for persons who previously could not engage in healthcare settings during critical times. Few rapid access, multidisciplinary transitional services exist to capture people in the immediate post-releas

Designing Integrated Virtual Care Partnerships: Insights from a Practice-Based Case Series at Mayo Clinic.

Access to high-quality primary and specialty care remains a persistent challenge in the United States, with millions of individuals facing barriers due to geography, provider shortages, and health inequities. As health care systems seek innovative solutions, virtual care providers have emerged as critical partners in expanding access, enhancing continuity, and supporting transitions across the health care continuum. This manuscript explores the integration of virtual care providers within an academic medical center, highlighting the dual roles of collaborative and referral models in bridging g

Communicating information and professional knowledge in acquired brain injury rehabilitation trajectories - a qualitative study of physiotherapy practice.

Purpose: The communication of information and professional knowledge is paramount during patient transitions, and conveying practice knowledge is an important part of the health professional's role. Physiotherapy interventions in both specialist and primary health care are often necessary to aid persons in rehabilitation following an acquired brain injury (ABI). The aim of this study was to investigate how physiotherapists experience the way patient information is communicated across health care levels in ABI rehabilitation. Methods: We performed interviews with a total of 19 physiotherapists r

Trajectories and predictors of self-care from pre-discharge to 12 months after discharge in people with spinal cord injury: a longitudinal study using growth mixture modeling.

INTRODUCTION: Self-care is essential for preventing secondary complications and supporting community reintegration after spinal cord injury (SCI). However, evidence is limited on how self-care changes over time after discharge and whether distinct subgroups exhibit different trajectories. METHODS: Using convenience sampling, 220 patients with SCI were recruited from a tertiary hospital in northern China between August 2023 and December 2024. Self-care was measured with the Self-Care in Spinal Cord Injuries Inventory (SC-SCII) at pre-discharge baseline and at 1, 3, 6, and 12 months after disch

Using Implementation Science to Promote Integration and Sustainability of Community Health Workers in the HIV Workforce.

BACKGROUND: Community health worker (CHW) interventions are an evidence-based practice adopted by health care settings to increase retention in care and viral suppression for people living with HIV (PLWH) from racial/ethnic minority communities. However, disparate funding, unclear roles vis a vis other care team members, limited training and promotion opportunities, and a lack of standards for wages and tasks limit the ability to effectively use CHWs as part of the health care team. Caregiving at the margins: An ethnographic exploration of family caregivers experiences providing care for structurally vulnerable populations at the end-of-life.

BACKGROUND: People experiencing structural vulnerability (e.g. homelessness, poverty, racism, criminalization of illicit drug use and mental health stigma) face significant barriers to accessing care at the end-of-life. 'Family' caregivers have the potential to play critical roles in providing care to these populations, yet little is known regarding 'who' caregivers are in this context and what their experiences may be. AIM: To describe family caregiving in the context of structural vulnerability, to understand who these caregivers are, and the unique challenges, burdens and barriers they fac

Qualitative Systematic Review and Meta-Aggregation of Stakeholders' Perspectives on Stepped Care in Facilitating Return to Community After Hospitalization for Anxiety and Depression.

The stepped care model aims to dynamically align the intensity of care with the patients' needs, adjusting mental health interventions accordingly. While stepped up care for mental illness has been widely researched, there is limited evidence on how stepped down care supports recovery after acute mental illness. This JBI qualitative systematic review sought to understand stakeholders' perspectives of stepped care in supporting patients' transitions from acute mental healthcare to community settings. CINAHL, Medline, Embase, PsycINFO and Web of Science were searched, and data were synthesized u

Integrating the primary care perspective in the implementation of a diabetes prevention program for patients with recent gestational diabetes: a qualitative descriptive study.

BACKGROUND: Women with gestational diabetes mellitus (GDM) are at high risk for the development of type 2 diabetes (T2D). Although postpartum diabetes prevention programs (DPPs) reduce the risk of diabetes after pregnancy, the transition from prenatal to postpartum diabetes care is suboptimal. As primary care providers (PCPs) are generally responsible for providing postpartum preventive care, incorporating PCPs' perspectives in the co-design of a DPP is essential to optimize the integration of prenatal and postpartum diabetes care and DPP implementation. METHODS: We used a qualitative descrip

Co-design of an intervention to enhance healthcare transition for adolescents and young adults with chronic medical conditions.

Adolescents and young adults (AYAs) with chronic health conditions must transition from paediatric to adult healthcare systems, yet many face significant challenges during this process. This qualitative study explored mechanisms influencing healthcare transition and examined components of a novel intervention-Transition Compass-featuring automated messaging, access to a transition nurse coordinator, and educational modules. Data were collected via individual interviews (n = 15), AYA-parent dyadic interviews (n = 4), and focus groups (n = 15) with AYAs, parents/carers, and healthcare profession

Adjusting to the transition into assisted living: Opportunities for nurse practitioners.

BACKGROUND AND PURPOSE: Transition from an independent living residence to assisted living (AL) is challenging. The study purpose was to understand such a transition from the perspective of older women. METHODS: A hermeneutic phenomenological approach was used to explore how 17 older women living in a Continuing Care Retirement Community, many of whom were recently widowed, experienced this transition. CONCLUSIONS: Three major themes emerged from the interviews: preplanning, executing, and adjusting to the transition. Even with facility, family and staff assistance, the transition was challe

Continuity of Care Within a Single Patient Support Program for Patients with Rheumatoid Arthritis Prescribed Second or Later Line Advanced Therapy.

INTRODUCTION: Guidelines suggest patients with rheumatoid arthritis (RA) inadequately controlled by tumor-necrosis-factor-inhibitors (TNFis) may benefit from switching to Janus-kinase-inhibitors (JAKis); however, care coordination and access can be complicated. Disruptions in transitioning to JAKi treatment could lead to disease flares requiring hospitalization; however, transitioning between products within the same patient support program (PSP) services aimed at ensuring continuity of care may minimize disruptions. METHODS: A retrospective, longitudinal cohort study of adult patients with R

Development of an evidence-based individualized transition plan for spina bifida.

OBJECTIVE: In spina bifida (SB), transition of care from the pediatric to adult healthcare settings remains an opportunity for improvement. Transition of care is necessarily multidimensional and focuses on increasing independence, autonomy, and personal responsibility for health-related tasks. While prior research has demonstrated that effective transition can improve health outcomes and quality of life while reducing healthcare utilization, little is known about the most advantageous transition program components/design. The individualized transition plan (ITP) was developed to optimize the r

Follow-up practices for children and adolescents with celiac disease: results of an international survey.

Adequate follow-up in celiac disease is important to improve dietary compliance and treat disease-related symptoms and possible complications. However, data on the follow-up of celiac children is scarce. We aimed to assess current pediatric celiac follow-up practices across Europe. Pediatricians and pediatric gastroenterologists from 35 countries in Europe, Israel, Turkey, and Russia completed an anonymous survey which comprised a 52-item questionnaire developed by the ESPGHAN Special Interest Group on Celiac Disease. A total of 911 physicians, the majority of whom exclusively worked in pediat

Caring for youth living with HIV across the continuum: turning gaps into opportunities.

With the increasing proportion of youth living with human immunodeficiency virus (YLHIV) and the aging of the perinatally infected population, there is a need for clinical services that are "youth friendly" to address the multiple challenges YLHIV face in terms of engagement in care and maintenance of combination antiretroviral therapy (cART). Little is known about how and where YLHIV receive their care. Further, the impact of the care structure on engagement and retention outcomes for YLHIV is ill defined. In order to better classify how YLHIV receive care in the United States, we performed a

Listeria monocytogenes - from saprophyte to intracellular pathogen.

Listeria monocytogenes is a bacterium that lives in the soil as a saprophyte but is capable of making the transition into a pathogen following its ingestion by susceptible humans or animals. Recent studies suggest that L. monocytogenes mediates its saprophyte-to-cytosolic-parasite transition through the careful modulation of the activity of a virulence regulatory protein known as PrfA, using a range of environmental cues that include available carbon sources. In this Progress article we describe the regulation of PrfA and its role in the L. monocytogenes transition from the saprophytic stage t

Refractory Constipation: What is the Clinician to Do?

Most clinicians will agree that chronic constipation is characterized by abnormal bowel movement consistency and/or frequency plus or minus evacuation symptoms, but patient perception of constipation varies widely and includes symptoms that may or may not meet official defining criteria. Although intermittent constipation is extremely common, only a small minority of patients seek care for their symptoms. Among these patients, dissatisfaction with the currently available laxative options is not uncommon, and many patients will require specialized care for severe or refractory symptoms-especial
