Pancreas transplantation.

Clinical pancreas transplantation is a procedure being applied with increasing effectiveness for the treatment of diabetes. The authors examine, through the Pancreas Transplant Registry, all known cases of islet transplantation since 1970 and all pancreas transplantation since 1977 and then summarize their own work on pancreas transplants at the University of Minnesota.

Pancreatic transplantation.

This study involved a retrospective analysis of the various factors which may have contributed to the outcome of pancreatic allograft and patient survival in our initial series of 13 segmental pancreatic transplants. Six of these patients received simultaneously renal allografts from the same donor. Pretransplant factors, such as prolonged warm ischemia and hypothermic storage periods greater than 12 hours after harvesting, correlated with an increase in acute post-transplant pancreatic failure. No clear correlation could be established between surgical techniques and final outcome of the panc…

Kidneys for transplantation.

A profound kidney shortage compromises the effective care of renal failure patients in the United States and other nations. This article discusses the need for kidneys, the procurement of live donor and cadaver organs, and strategies to increase organ donation. It suggests that the kidney shortage can be minimized and perhaps eliminated through the use of living related donors whenever appropriate, and through more efficient procurement of volunteered cadaver kidneys. Two other strategies to increase the supply of kidneys-the sale of organs and "contracting out" laws-are rejected. The first is

Kidney transplantation.

Since 1954, more than 26,000 kidney transplants have been performed and transplantation is now an accepted method for the treatment of end-stage renal disease. Recent advances in transplantation include operation not only on ideal recipients, but also on a much broader group of patients; an improved understanding of rejection mechanisms; better employment of standard immunosuppressive drugs; identification of an additional major histocompatibility antigen system; appreciation that blood transfusions prior to transplantation may facilitate acceptance of cadaver kidneys; and development of intra…

Liver transplantation.

From March 1963 through June 1976, 111 patients received orthotopic liver homografts. Forty-two of the recipients had congenital biliary atresia. Other common diagnoses were chronic aggressive hepatitis, Laennec's cirrhosis, and primary hepatic malignancy. There were also other assorted, less common diagnoses. Thirty-one of the 111 patients (28%) lived at least one year and 15 are still alive with follow-ups of 2 1/2 to 8 1/2 years. Seven of the patients lived for more than five years, and 6 of these 7 are still alive. In 1975 and 1976, clinical-pathologic correlations on all these patients we

Renal transplantation.

Kidney transplantation is now a very acceptable form of treatment of end-stage renal failure. About 50% of kidney grafts are still functioning after five years. If the kidney graft fails, and failure is usually due to rejection, patients are returned to dialysis for definitive treatment or to await another transplant. Although kidneys from living related donors offer the best chance of success, most kidneys in Australia are transplanted from cadavers. A shortage of cadaver kidneys necessitates a wait of 6 to 12 months for a transplant.

Kidney transplantation.

Fifty patients per million per annum in Australia suffer from end stage renal failure and could be helped by dialysis and transplantation. At the present time, only half this number are being referred to renal units in Australia where we believe facilities are available to help all that could benefit from this treatment. This article discusses patient selection, the results and hazards of renal transplantation. As a result of this treatment programme, 50 per cent of patients can expect virtually a normal life with a kidney graft functioning five years or more.

[Tooth transplantation].

In the light of the thesis of J. Thibault, the authors suggest, in association with transplantation of wisdom tooth buds into the alveolus of the first molar, a technique consisting of placing the graft in its pre-eruptive position, removing in a block the bud, the gubernaculum and the sub-jacent gingival mucosa. Under these conditions, eruption occurs at the same time as development of the roots, which represents the most physiological solution. At the same time as formation of the root, the authors noted a paradoxal rhizalysis, hypercementosis resulting in ankylosis and conservation of vital

Heart Transplantation.

Heart transplantation (HTx) remains the last therapeutic resort for patients with advanced heart failure. The present work is a clinically focused review discussing current issues in heart transplantation. Several factors have been associated with the outcome of HTx, such as ABO and HLA compatibility, graft size, ischemic time, age, infections, and the cause of death, as well as imaging and laboratory tests. In 2018, UNOS changed the organ allocation policy for HTx. The aim of this change was to prioritize patients with a more severe clinical condition resulting in a reduction in mortality of

Facial Transplantation.

Reconstruction of severe facial deformities poses a unique surgical challenge: restoring the aesthetic form and function of the face. Facial transplantation has emerged over the last decade as an option for reconstruction of these defects in carefully selected patients. As the world experience with facial transplantation grows, debate remains regarding whether such a highly technical, resource-intensive procedure is warranted, all to improve quality of life but not necessarily prolong it. This article reviews the current state of facial transplantation with focus on the current controversies a
