Telemedicine Consultation for Pediatric Trauma in Community ED Visits and Parent Experience of Care: A Stepped-Wedge Cluster-Randomized Clinical Trial.

IMPORTANCE: Telephone consultations for acutely injured children receiving care in emergency departments (EDs) in nonpediatric trauma centers frequently exclude direct communication and informed decision-making with parents. The use of telemedicine to facilitate real-time expert consultation and parental communication with pediatric trauma specialists is underexplored and may improve family experience and support clinical decision-making. OBJECTIVE: To examine the impact of telemedicine on parent experience of care, parent distress, and transfer frequency for acutely injured children presenti…

High utilization of early tracheostomy is associated with shortened ICU length-of-stay but not improved outcomes in pediatric TBI: A TQIP analysis.

BACKGROUND: Optimal tracheostomy timing in pediatric traumatic brain injury (TBI) remains uncertain. We evaluated whether center-level early tracheostomy (≤7 days) practices are associated with clinical outcomes. METHODS: The Trauma Quality Improvement Program (2017-2021) was queried for patients <18 years with severe TBI (Head AIS >3) who underwent tracheostomy. Centers in the highest tertile of early tracheostomy use were classified as early centers. Multivariate regression compared mortality, hospital and ICU length of stay (LOS), and respiratory complications. RESULTS: Among 975 patients

The impact of a multidisciplinary team on the management of fracture-related infections: A surveillance study in Brazil.

BACKGROUND: Fracture-Related Infection (FRI) is an increasing and challenging complication following orthopedic trauma surgery. Preventive and microbial diagnostic measures vary significantly particularly in low- and middle-income countries. The objectives of this national questionnaire were to investigate clinical practices towards preventive and diagnostic strategies adopted by Brazilian orthopedic trauma centers and to assess the impact of Multidisciplinary Teams (MDT) on the management of FRI. METHODS: A 34-item electronic questionnaire was developed via REDCap® and distributed to all tra…

Adequacy and accuracy of nontrauma center computed tomography: What are we missing?

BACKGROUND: Timely and appropriate use of computed tomography (CT) scans is critical to the evaluation of traumatic injuries. The objective of this study was to assess the adequacy of CT scans performed at nontrauma centers (NTCs) as they pertain to the management of trauma patients. METHODS: Adult patients transferred to our ACS-verified Level I trauma center from any NTC between May and December 2012 were enrolled prospectively. Available CT images from NTCs were reviewed in a blinded fashion by our facility's trauma radiologist; his interpretations were compared with those from the NTC. In…

Blunt cerebrovascular injury: incidence and long-term follow-up.

PURPOSE: Blunt cerebrovascular injuries (BCVI), which can result in ischemic stroke, are identified in 1-2% of all blunt trauma patients. Computed tomography angiography (CTA) scanning has improved and is the diagnostic modality of choice in BCVI suspected patients. Data about long-term functional outcomes and the incidence of ischemic stroke after BCVI are limited. The aim of this study was to determine BCVI incidence in relation to imaging modality improvements and to determine long-term functional outcomes. METHODS: All consecutive trauma patients from 2007 to 2016 with BCVI were identifie

Role of hydrogen peroxide injection for penetrating abdominal injury in creating CT Tractogram.

Penetrating abdominal trauma is responsible for approximately 35% of patients admitted to urban trauma centers, and up to 12% of those admitted in suburban or rural centers in the United States. Current protocol relies heavily on CT imaging as the diagnostic tool in evaluating for peritoneal violation in hemodynamically stable patients, however it is associated with false negative rates. In addition, visualization of the fascia of the rectus abdominis, the transversalis fascia, and the peritoneum cannot be reliably identified with CT. Studies have probed into the use of injecting IV contrast d…

Evaluation of a rib fracture management pathway in an Australian tertiary trauma hospital.

ObjectiveRib fractures are a common cause of hospitalisation in the adult trauma population and are associated with significant morbidity and mortality. This study aimed to assess the efficacy of a bundle of care known as the 'rib fracture pathway' in improving patient-centred outcomes and health economic outcomes in an Australian tertiary trauma hospital.MethodsThis single-centre retrospective cohort study analysed prospectively collected pain registry data from 2004 to 2022, encompassing the period before and after the implementation of the rib fracture pathway.ResultsThe implementation of a

Integrating extended focused assessment with sonography for trauma (eFAST) in the initial assessment of severe trauma: Impact on the management of 756 patients.

BACKGROUND: Before total body computed tomography scan, an initial rapid imaging assessment should be conducted in the trauma bay. It generally includes a chest x-ray, pelvic x-ray, and an extended focused ultrasonography assessment for trauma. This initial imaging assessment has been poorly described since the increase in the use of ultrasound. Therefore, our study aimed to evaluate the diagnostic accuracy and therapeutic impact of this initial imaging work-up in severe trauma patients. A secondary aim was to assess the therapeutic impact of a chest x-ray according to the lung ultrasonography

Direct oral anticoagulants compared with warfarin in patients with severe blunt trauma.

METHODS: We queried our Trauma Quality Improvement Program registry for patients who presented between 6/1/2011 and 9/1/2015 with severe (injury severity score (ISS)>15) blunt traumatic injury during anticoagulant use. Patients were then grouped into those prescribed warfarin and patients prescribed any of the available novel Direct Oral Anticoagulants (DOAC) medications. We excluded severe (AIS≧4) head injuries. RESULTS: There were no differences between DOAC and warfarin groups in terms of age, gender mean ISS, median hospital or intensive care unit lengths of stay, complication proportions

Pediatric pelvic ring injuries: How benign are they?

INTRODUCTION: With an incidence of less than 0.2% of all pediatric fractures, pelvic ring injuries are rare. Historically they were conservatively treated, but because malunion and long-term morbidity are associated with unstable injuries, a trend towards operative treatment can be observed. The purpose was to determine clinical and radiographic outcomes following these complex pediatric pelvic ring injuries. PATIENTS AND METHODS: This Level IV retrospective analysis was completed at a private orthopaedic practice in association with a Level One teaching trauma center. There were 33 children

Upper extremity injury patterns unique to utility terrain off-road vehicles.

OBJECTIVES: Utility terrain vehicles (UTVs), equipped with roll cages and safety harnesses, are increasingly popular recreational vehicles. Unlike all-terrain vehicles (ATVs) which lack these features and have well-documented injury patterns, UTV injury patterns are not represented well in literature. This study aims to characterize these distinct injury patterns and their impact. METHODS: A retrospective chart review from a Level 1 trauma center was conducted, including all UTV- and ATV-related injuries between 2018 and 2022. Statistical analyses utilized Chi-square, t-tests, and Mann-Whitne

Acute colonic pseudo-obstruction in polytrauma patients.

BACKGROUND: Acute colonic pseudo-obstruction (ACPO) is characterized by severe colonic distension without mechanical obstruction. It has an uncertain pathogenesis and poses diagnostic challenges. This study aimed to explore risk factors and clinical outcomes of ACPO in polytrauma patients and contribute information to the limited literature on this condition. METHODS: This retrospective study, conducted at a Level 1 trauma center, analyzed data from trauma patients with ACPO admitted between July 2009 and June 2018. A control cohort of major trauma patients was used. Data review encompassed p

Augmenting Denver criteria yields increased BCVI detection, with screening showing markedly increased risk for subsequent ischemic stroke.

PURPOSE: BCVI may lead to ischemic stroke, disability, and death, while being often initially clinically silent. Screening criteria for BCVI based on clinical findings and trauma mechanism have improved detection, with Denver criteria being most common. Up to 30% of patients do not meet BCVI screening criteria. The aim of this study was to analyze the effect of augmented Denver criteria on detection, and to determine the relative risk for ischemic stroke. METHODS: Denver screening criteria were augmented by any high-energy trauma of the cervical spine, thorax, abdomen, or pelvis. All acute bl
