Syphilis vaccine: challenges, controversies and opportunities.

Syphilis is a sexually or vertically (mother to fetus) transmitted disease caused by the infection of Treponema pallidum subspecie pallidum (TPA). The incidence of syphilis has increased over the past years despite the fact that this bacterium is an obligate human pathogen, the infection route is well known, and the disease can be successfully treated with penicillin. As complementary measures to preventive campaigns and early treatment of infected individuals, development of a syphilis vaccine may be crucial for controlling disease spread and/or severity, particularly in countries where the e…

Status epilepticus with focal neurologic deficits: a rare presentation of Neurosyphilis.

Neurosyphilis is a late complication of primary syphilis and occurs with a multitude of vague symptoms. In this study, we report a patient with neurosyphilis who presented with status epilepticus, hemiplegia, and aphasia, which may be easily misdiagnosed. After performing the reactive serum test, including the toluidine red unheated serum test and the Treponema pallidum particle agglutination assay test, and cerebrospinal fluid analysis, as well as the brain Magnetic Resonance Imaging (MRI) results, we consider it general paresis of the insane, also known as dementia paralytica. The patient wa…

Epidemiologic and Genomic Reidentification of Yaws, Liberia.

We confirmed endemicity and autochthonous transmission of yaws in Liberia after a population-based, community-led burden estimation (56,825 … re workers. We recovered whole bacterial genomes from 12 cases and describe a monophyletic clade of Treponema pallidum subspecies pertenue, phylogenetically distinct from known TPE lineages, including those affecting neighboring nonhuman primate populations (Taï Forest, Côte d'Ivoire). Yaws is endemic in Liberia but exhibits low focal population prevalence …

Renal manifestations of syphilis.

Syphilis is a sexually transmitted disease caused by spirochaete Treponema pallidum. The incidence of syphilis is rising across the globe. It has been described in the literature as a great imitator due to the vast range of clinical manifestations that can occur in the disease. Renal manifestations are rare but a feature of secondary syphilis. It can cause glomerulopathies, tubular pathology and vasculitic lesions in the kidney. Membranous nephropathy is the most commonly reported glomerular lesion associated with syphilis. With two recent cases of secondary membranous nephropathy due to syphi…

Predicting asymptomatic neurosyphilis using peripheral blood indicators.

BACKGROUND: The high misdiagnosis rate of asymptomatic neurosyphilis (ANS) has long challenged infectious disease clinicians. We aim to develop a model for diagnosing ANS in asymptomatic syphilis (AS) patients without CSF indicators. RESULTS: 277 AS patients with HIV-negative and underwent lumbar puncture were enrolled in this horizontal study. The area under the curve for predicting ANS by CSF leukocytes and protein was 0.643 and 0.675 [95% CI, 0.583-0.699VS.0.616-0.729]. Through LRM, the AUC increased to 0.806 [95% CI, 0.732-0.832], and the Youden's index was 0.430. If the score is ≤ 0.159,

Missing pieces of the puzzle to effectively control digital dermatitis.

Since the first report of bovine digital dermatitis (DD) in 1974, there is a large body of the literature published; however, effective prevention and control of the disease remain elusive. Although many aspects of the pathogenesis of DD have been investigated, even some of the most basic questions such as the aetiology of this disease remain under debate. Treponema spp. have been strongly associated with DD lesions and occur in abundance in advanced lesions; however, efforts to induce disease with pure cultures of these organisms have been largely underwhelming and inconsistent. Furthermore, …

Eradicating successfully yaws from India: The strategy & global lessons.

Yaws, a non-venereal treponematosis, affecting primarily the tribal populations, has been considered historically as one of the most neglected tropical diseases in the world. In 1996, India piloted an initiative to eradicate yaws based on a strategy consisting of active case finding through house-to-house search and treatment of cases and their contacts with long acting penicillin. Thereafter, the campaign implemented in all 51 endemic districts in 10 states of the country led to the achievement of a yaws-free status in 2004. In the post-elimination phase, surveillance activities accompanied b…

Yaws.

Yaws is an infectious disease caused by Treponema pallidum pertenue-a bacterium that closely resembles the causative agent of syphilis-and is spread by skin-to-skin contact in humid tropical regions. Yaws causes disfiguring, and sometimes painful lesions of the skin and bones. As with syphilis, clinical manifestations can be divided into three stages; however, unlike syphilis, mother-to-child transmission does not occur. A major campaign to eradicate yaws in the 1950s and 1960s, by mass treatment of affected communities with longacting, injectable penicillin, reduced the number of cases by 95%…

Accelerated lesion development in experimental syphilis.

In experimental syphilis, lesion development changes when different numbers of treponemes are injected. Lesions at sites inoculated with lower concentrations frequently exhibit delayed incubation periods, less tissue degradation, and more rapid healing. In this report I describe an unusual finding, namely, that accelerated rather than delayed incubation period occurred after intradermal inoculation of different dosages of Treponema pallidum. When rabbits were injected with only one inoculum containing 10(6), 10(5), 10(4), or 10(3) treponemes per rabbit, the corresponding incubation periods wer…

Urinary excretion of an unidentified treponeme in a case of leptospirosis.

Two kinds of spirochetes were observed by dark-field microscopy in the urine of a leptospirosis patient on day 26 after the onset of the dis … e sera were nonreactive in syphilis serology tests, but showed a weak fluorescence in a fluorescent treponemal antibody absorption (FTA-ABS) test when Treponema calligyrum was used as antigen. It could not be determined whether this reaction was due to antibodies raised in response to antigens of the excreted treponemes.

An improved method for recovering clinical isolates and assessing pathogenicity of blood from secondary syphilis patients.

BACKGROUND: Recovering clinical isolates of Treponema pallidum (Tp) can expedite the research on molecular epidemiology and vaccine development of syphilis. Nevertheless, obtaining numerous isolates through rabbit infectivity testing remains unsatisfactory, particularly in secondary syphilis cases. This study re-evaluates the reliability of orchitis as a phenotypic indicator for assessing Tp clinical strains during rabbit infectivity testing. METHODS: Treponemal and nontreponemal tests were used to analyze the correlation between serological titers and Tp burden. An optimal titer threshold wa…
