Hemolytic markers following the transfusion of uncrossmatched, cold-stored, low-titer, group O+ whole blood in civilian trauma patients.

BACKGROUND: Low-titer group O whole blood (LTOWB) is increasingly being used in the civilian trauma setting, although there is a risk of hemolysis. This study evaluated the impact on hemolytic markers following the transfusion of 4 or more units of uncrossmatched LTOWB. METHODS: Civilian adult trauma patients who received four or more units of leukoreduced group O+, low-titer (<50 anti-A and anti-B), platelet-replete uncrossmatched whole blood during their initial resuscitation and who survived for more than 24 hours after the transfusion were included in this retrospective study. Lactate deh

Transient coagulation abnormalities after incompatible blood transfusion.

This report describes an unusual hemolytic transfusion reaction. A group O donor was inadvertently transfused with 100 ml of group A red cells during the course of plasmapheresis. Granulocytopenia, thrombocytopenia, hypofibrinogenemia and low levels of factors V, VIII, and IX developed immediately after the infusion. The levels of the cellular elements and coagulation proteins returned to normal in less than an hour; too rapid for a de novo synthesis after consumption of coagulation proteins. We postulate that activation of the kinin system was responsible for the phenomena rather than intrava

ABO and Rhesus Blood Groups in Acute Puumala Hantavirus Infection.

Puumala hantavirus (PUUV) causes hemorrhagic fever with renal syndrome. We aimed to evaluate whether ABO and rhesus blood groups associate with the susceptibility or the severity of PUUV infection. We analyzed blood groups in 289 adult patients treated in Tampere University hospital due to PUUV infection during the years 1982-2017. Patients' blood group distribution was compared to that of healthy, voluntary blood donors living in the Tampere University Hospital responsibility area (n = 21,833). The severity of PUUV infection, as judged by the severity of acute kidney injury (AKI), thrombocyto

Anti-D in a mother, hemizygous for the variant RHD*DNB gene, associated with hemolytic disease of the fetus and newborn.

BACKGROUND: Individuals with the partial D phenotype when exposed to D+ red blood cells (RBCs) carrying the epitopes they lack may develop anti-D specific for the missing epitopes. DNB is the most common partial D in Caucasians and the clinical significance for anti-D in these individuals is unknown. STUDY DESIGN AND METHODS: This article describes the serologic genotyping results and clinical manifestations in two group D+ babies of a mother presenting as group O, D+ with alloanti-D. RESULTS: The mother was hemizygous for RHD*DNB gene and sequencing confirmed a single-nucleotide change at c

Blood count changes in malaria patients according to blood groups (ABO/Rh) and sickle cell trait.

INTRODUCTION: Introduction: Malaria continues to be the leading cause of hospitalization and death in Angola, a country in sub- Saharan Africa. In 2023, in the first quarter, 2,744,682 cases were registered, and of these 2,673 patients died due to malaria disease. Previous studies have shown that the ABO blood group can affect the progression of malaria to severe conditions after P. falciparum infection, while the sickle cell gene offers relative protection. OBJECTIVE: We investigated changes in the blood count according to blood groups (ABO/Rh) and sickle cell trait in patients with malaria

[Hemolytic disease of the newborn due to ABO incompatibility (author's transl)].

Up to now, the exact diagnosis of ABO hemolytic disease of the newborn cannot be made either clinically or serologically. Affected babies are almost all either group A or group B from mothers of group O. In contrast to Rh hemolytic disease the immunological findings do not correlate well with the severity of the clinical course. Sometimes, it is impossible to differentiate between ABO hemolytic disease and non-antibody mediated hyperbilirubinemia. Pathogenetic aspects are discussed which may explain the differences between ABO and Rh hemolytic disease of the newborn. Aids to immunological and

Innovative strategies in kidney paired donation: single-center experience achieving the highest annual transplant volume globally.

BACKGROUND: Kidney Paired Donation (KPD) programs expand transplant opportunities for immunologically incompatible donor-recipient pairs. This study describes the operational framework and clinical outcomes of a high-volume, single-center KPD program, which became the highest-volume center globally in 2024. METHODS: We analyzed all kidney transplants performed through our KPD program between January and December 2024. The program aimed to achieve full HLA and ABO compatibility for incompatible pairs, while also incorporating additional strategies: inclusion of compatible pairs to improve HLA

Impact of ABO incompatibility on adult deceased donor liver transplantation outcomes: A 20-year retrospective study using the SRTR database.

BACKGROUND: ABO-incompatible (ABOi) deceased donor liver transplantation (DDLT) is seldom used due to concerns regarding postoperative outcomes, derived from predominantly older cohorts with small sample sizes. The aim of this study is to assess the impact of ABO incompatibility on outcomes of DDLT in a large, contemporary cohort. METHODS: Adult DDLT patients were included from the Scientific Registry of Transplant Recipients (SRTR) database who underwent transplant from 2004 to 2024. Demographics and complications were compared between ABOi and ABO-compatible (ABO-c) cohorts. Multivariate Co

The Real-World Safety and Effectiveness of Rituximab in Preventing Antibody-Mediated Rejection in ABO-Incompatible Liver Transplantation: All-Case Postmarketing Surveillance in Japan.

BACKGROUND: In ABO-incompatible (ABOi) liver transplantation, antibody-mediated rejection (AMR) is a challenging post-transplant complication. In Japan, rituximab was approved in 2016 to prevent AMR in ABOi kidney and liver transplantation and in 2023 to prevent and treat AMR in kidney, liver, heart, lung, pancreas, and intestine transplantation. METHODS: We conducted an all-case postmarketing surveillance to assess the safety and effectiveness of rituximab in ABOi liver transplant recipients, with a registration period between February 29, 2016 and May 9, 2021. The safety endpoint was the oc

Outcomes of ABO-incompatible kidney transplants with very high isoagglutinin titers: a single-center experience and literature review.

BACKGROUND: ABO-incompatible kidney transplantation (ABOi-KTx) represents a possible solution to address the shortage of kidney donors. However, these transplants present immunological challenges, particularly when isoagglutinin titers are elevated pretransplant. METHODS: Single-center retrospective study describing clinical and biological outcomes of 8 patients who underwent ABOi-KTx with initial isoagglutinin titers ≥ 1/512. All patients followed a desensitization protocol combining immunosuppression (rituximab, tacrolimus, mycophenolate mofetil, steroids), and specific or semi-specific aph

Outcome of desensitization in human leukocyte antigen and ABO incompatible living donor kidney transplantation: Single center experience of first 200 incompatible transplants.

BACKGROUND AND AIMS: Although desensitization is well established, concerns about graft outcome, patient survival and rejection still exist. The present study aims at comparing outcomes of renal transplant recipients across simultaneous ABO and human leukocyte antigen (HLA) incompatibility barriers to those with ABO or HLA incompatibility alone. MATERIALS AND METHODS: This was a retrospective study conducted from October 2015 to December 2018. All patients with a clinical diagnosis of chronic kidney disease, who were prospective HLA incompatible (HLAi) and/or ABO incompatible (ABOi) renal tra
