Impact of home healthcare reform on place of death for people with dementia: A nationwide cohort study accounting for cultural factors of impending death discharge.

BACKGROUND: Although home is frequently the preferred place of death, little is known regarding how home healthcare (HHC) influences this outcome for people with dementia (PWD), particularly within Asian contexts. This study investigates the impact of HHC and its 2016 "Integrated Home-Based Medical Care" reform on home death in Taiwan, explicitly accounting for the cultural phenomenon of "impending death discharge." METHODS: This nationwide retrospective cohort study utilized Taiwan's National Health Insurance Research Database. We identified PWD decedents from 2011 to 2022 and conducted a ne…

Accounting for recombination rate variation improves inference of barrier loci and reveals the role of both natural and sexual selection in an incipient bird radiation.

Examining genomic patterns of differentiation across lineage pairs at different stages of the speciation continuum, in combination with recombination maps, can help disentangle the effects of linked and divergent selection and identify lineage-specific targets of selection that may act as barrier loci during speciation. Here, we apply this framework to genomic data from African and Indian Ocean bird species of the genus Zosterops (Zosteropidae) to identify candidate barrier loci between ecologically, phenotypically, and genetically distinct Reunion gray white-eye (Zosterops borbonicus) parapat

Accounting for the role of the gastro-intestinal tract in the ammonia and urea nitrogen dynamics of freshwater rainbow trout on long-term satiation feeding.

The contribution of the gut to the ingestion, production, absorption and excretion of the extra ammonia and urea nitrogen (urea-N) associated with feeding ('exogenous' fraction) has received limited attention. Analysis of commercial pellet food revealed appreciable concentrations of ammonia and urea-N. Long-term satiation feeding increased whole-trout ammonia and urea-N excretion rates by 2.5-fold above fasting levels. Blood was sampled from the dorsal aorta, posterior, mid- and anterior sub-intestinal veins, as well as the hepatic portal vein in situ. Ammonia, urea-N and fluid flux rates were

Accounting for repeat intervention costs in the economic comparison of laser interstitial thermal therapy and anterior temporal lobectomy for treatment of refractory temporal lobe epilepsy.

OBJECTIVE: Laser interstitial thermal therapy (LITT) is an alternative to anterior temporal lobectomy (ATL) for the treatment of temporal lo … TT will have higher average patient cost than ATL, and this is indeed the case in our sample. After accounting for subsequent surgeries, the average patient charge in the LITT sample ($103,700) was significantly higher than for the ATL sample ($88,548). A second statistical comparison derived from our model adjusts for the difference in effectiveness by calc …

Do motor coordination and sleep difficulties predict peer functioning in children and adolescents with attention-deficit and hyperactivity disorder after accounting for existing ADHD symptomology?

Children with attention-deficit and hyperactivity disorder (ADHD) are more likely to experience peer problems compared to their non-ADHD peers, though ADHD-specific symptoms only partially explain this association. This study examined whether sleep difficulties and motor coordination problems are additional predictors of peer problems in an ADHD population. An ADHD sample of 72 participants aged 6-14 years (M = 9.86 years, SD = 1.79 years) was evaluated for an association of peer problems with measures of motor coordination, sleep difficulties as well as ADHD and comorbidity symptoms. Hierarch

Vascular flow on doppler sonography may not be a valid characteristic to distinguish colloid nodules from papillary thyroid carcinoma even when accounting for nodular size.

BACKGROUND: The purpose of this study was to test the hypothesis that there is no significant difference in vascular flow patterns between cytopathologically-proven colloid nodules and papillary thyroid carcinoma (PTC) even when adjusting for nodule size. METHODS: Doppler vascular flow patterns in 200 colloid nodules and 166 nodules with PTC were retrospective reviewed independently by 2 neuroradiologists blinded to the cytopathological results. Absence of vascular flow, perinodular and/or intranodular flow, and diffuse vascular flow were recorded. The vascular flow patterns were compared wit

[Optimization of accounting for radiation exposure of patients as a basis for assessing the risk of stochastic effects due to medical X-ray diagnostic irradiation].

The work is devoted to optimization of the control and accounting for radiation exposure of patients in the X-ray examinations by means of improving existing forms of state statistical survey (form #3-DOZ.). The analysis of the radiation exposure of patients in medical institutions of the Rostov region for the period of 2005-2011 was carried out. It is proposed to exclude the information about the individual patient doses received by calculation; differentiate a group of "adult patients' on age and gender, selecting the reproductive age, discriminate as a separate category of dose-generating, …

Do tasks make a difference? Accounting for heterogeneity of performance of children with reading difficulties on tasks of executive function: findings from a meta-analysis.

Research studies have implicated executive functions in reading difficulties (RD). But while some studies have found children with RD to be impaired on tasks of executive function other studies report unimpaired performance. A meta-analysis was carried out to determine whether these discrepant findings can be accounted for by differences in the tasks of executive function that are utilized. A total of 48 studies comparing the performance on tasks of executive function of children with RD with their typically developing peers were included in the meta-analysis, yielding 180 effect sizes. An ove

Microsatellite instability and mutation analysis among southern Italian patients with colorectal carcinoma: detection of different alterations accounting for MLH1 and MSH2 inactivation in familial cases.

BACKGROUND: Microsatellite instability (MSI) is due to defective DNA mismatch repair (MMR) and has been detected at various rates in colorectal carcinoma (CRC). The role of MSI in colorectal tumorigenesis was assessed further in this study by both microsatellite analysis of two CRC subsets [unselected patients (n = 215) and patients <50 years of age (n = 95)], and mutation screening of the two major MMR genes MLH1 and MSH2 among familial CRC cases. PATIENTS AND METHODS: PCR-based microsatellite analysis was performed on paraffin-embedded tissues. In CRC families, MLH1/MSH2 mutation analysis a

Accountability and equity in primary health care: co-creating user-centered evaluation systems for public-private partnerships.

BACKGROUND: Brazil's Unified Health System (SUS) is one of the world's largest universal health systems and operates through a plural governance arrangement involving public, philanthropic, and community actors under public coordination. In Primary Health Care (PHC), non-profit Social Health Organizations (OSS) are increasingly responsible for managing public healthcare facilities in large urban municipalities. However, evaluation systems used by public managers remain largely focused on service delivery indicators, with limited consideration of equity and user experience. AIM: To examine how

FA-MFMR: a multivariable functional Mendelian randomization method that accounts for correlated longitudinal exposures via factor augmentation.

MOTIVATION: Mendelian randomization (MR) is widely used for causal inference using genetic data, yet most existing MR methods treat exposures as static and have limited capacity to analyze longitudinal measurements. In many biomedical studies, multiple exposures evolve over time, are strongly correlated, and share unobserved temporal structure. Ignoring these features can lead to unstable estimation and reduced power. RESULTS: We propose factor-augmented multivariable functional Mendelian randomization (FA-MFMR) for estimating time-varying causal effects of multiple longitudinal exposures. FA

Qualitative accounts of community health workers providing and shaping mental health support in climate-vulnerable communities in Bangladesh.

Despite a clear need for mental health services in Bangladesh, there is limited access and provision, especially in areas vulnerable to the effects of climate change. Community health workers (CHWs) are commonplace primary care givers in rural communities and promise to deliver more culturally sensitive and pragmatic services. This study seeks to understand the experiences and perceptions of CHWs who have received basic training in mental health and are providing initial mental health support as the only providers of such care in underserved communities in Bangladesh. We conducted an explorato

Increasing Minimally Processed Food Intake in Depression via Commercial Meal Delivery: Qualitative Accounts of Participant Experiences.

Background/Objectives: Several randomized controlled trials have found that dietary interventions promoting self-guided shifts away from ultra-processed foods (UPFs) and toward minimally processed, nutrient-dense foods may alleviate depressive symptoms. However, translating these interventions into scalable and sustainable real-world approaches remains a key challenge. Adopting a minimally processed dietary pattern requires sustained effort (e.g., meal planning, shopping, and preparation) within environments where UPFs are pervasive and convenient. These demands may be especially burdensome fo

Rethinking content delivery for learning reinforcement: a reflective account of podcast summaries and Universal Design for Learning in Pharmacy Education.

PURPOSE: Maintaining student engagement with lecture content beyond the classroom remains a persistent challenge in higher education. While lecture recordings are widely used, they often require substantial time to revisit and may not align with students' study habits. This reflective article examines the introduction of concise AI-assisted podcast summaries as a supplementary learning resource within a second-year module in the MPharm programme. The purpose of this reflection is to explore how this intervention influenced student engagement with lecture material and how the experience reshape

Glocalizing medical education through community clinical experiences: a model for socially accountable preclinical, community-based training.

Calls for "glocalization" in health professions education reflect growing expectations that global educational standards are meaningfully aligned with local priorities, in keeping with social accountability principles and shared values of relevance, equity, quality and cost-effectiveness. At the same time, increasing attention to the role of social determinants in shaping health outcomes challenges medical institutions to prepare practice-ready physicians who are culturally responsive and well-equipped to deliver holistic, patient-centered care. Scalable models that translate these priorities

Reduced linguistic coherence in psychosis defies semantic similarity accounts and relates to altered large-scale cortical hierarchy.

Coherence in speech is clinically significant in mental disorders but remains difficult to quantify. We tested the widely-held assumption that semantic similarity metrics derived from large language models capture human-rated coherence. Across three large neurotypical datasets in different languages, semantic similarity failed to correlate with human ratings, while six other metrics, especially the probability-based metrics, showed significant but weak correlations. In an additional English dataset of 94 individuals, including healthy controls and patients with schizophrenia spectrum disorders

LC3B Mediated SETDB1-Accounted Alcoholic Steatohepatitis via Lipidation-Dependent LAP and Lipidation-Independent Nuclear Stabilization.

Alcohol-associated liver disease (ALD) progresses from steatosis to steatohepatitis, but the underlying mechanisms remain unclear. Here, we investigated SETDB1's role in ALD progression involving LC3B-mediated LC3-associated phagocytosis (LAP). SETDB1 expression was progressively downregulated in livers of alcohol-fed mice and ethanol-treated hepatocytes, correlating with disease severity. Setdb1 HKO mice exhibited accelerated ALD progression, developing severe steatosis, inflammation, and fibrosis even under pair-fed conditions, indicating SETDB1 deficiency enhances disease susceptibility to

FcRn function alone cannot account for elevated IgG clearance in the Lewis lung carcinoma model of cancer cachexia.

PURPOSE: The neonatal Fc receptor (FcRn) protects IgG-based monoclonal antibodies (mAbs) from catabolism by direct binding within endosomes and facilitates their recycling to extracellular spaces. Elevated clearance of immune checkpoint inhibitors (ICIs) and other IgG-based mAbs is often observed in patients with cachexia phenotypes and is associated with worse outcomes. We sought to understand if FcRn's function is altered in cancer cachexia. EXPERIMENTAL DESIGN: Clearance of IgGs with different FcRn-binding properties were evaluated in cachectic LLC tumor-bearing (TB) and non-cachectic tumo
