Women with a history of surgical correction for adolescent idiopathic scoliosis are often denied neuraxial anesthesia during labor and delivery due to concerns of technical difficulties, anesthetic failures, inadvertent dural punctures, and hardware infections. We report four successful neuraxial procedures by four different anesthesiologists in three patients with previous posterior spinal instrumentation and fusion who delivered at our institution. The procedures include spinal anesthesia for cerclage, epidural analgesia for labor followed by successful conversion to epidural anesthesia for

BACKGROUND AND OBJECTIVES: Microcatheters for continuous spinal anesthesia were withdrawn from the market after an apparent increase in the incidence of cauda equina syndrome (CES) associated with spinal anesthesia after introduction of these catheters. The objective of this review is to evaluate the historical data on CES after spinal anesthesia and to compare the result to the recent data.
METHODS: The literature on the use of statistics and on complications associated with spinal anesthesia was reviewed. Poisson probabilities were calculated to assess the probability of seeing the recent c

STUDY OBJECTIVE: Spinal anesthesia often causes hypotension, with consequent risk to the fetus. The use of vasopressor agents has been highly recommended for the prevention of spinal anesthesia-induced hypotension during caesarean delivery. Many studies have shown that norepinephrine can provide more stable maternal hemodynamics than phenylephrine. We therefore tested the hypothesis that norepinephrine preserves fetal circulation better than phenylephrine when used to treat maternal hypotension consequent to spinal anesthesia.
DESIGN: Prospective, randomized, double-blinded study.

Ankylosing spondylitis (AS) complicates anesthetic management due to altered spinal anatomy and potential airway difficulties. Neuraxial anesthesia is feasible but technically challenging, with potentially variable drug distribution. High spinal block reports in AS remain limited. This report describes the case of a 58-year-old male patient with AS who underwent elective total hip arthroplasty under spinal anesthesia (3.5 mL 2% isobaric mepivacaine). An initial T11 sensory block rapidly progressed cephalad to C6 within minutes, causing upper extremity weakness, bradycardia, and hypotension. Co

BACKGROUND AND OBJECTIVES: Decrease in body temperature is common during general and regional anesthesia. Forced-air warming intraoperative during cesarean section under spinal anesthesia seems not able to prevent it. The hypothesis considers that active warming before the intraoperative period avoids temperature loss during cesarean.
METHODS: Forty healthy pregnant patients undergoing elective cesarean section with spinal anesthesia received active warming from a thermal gown in the preoperative care unit 30min before spinal anesthesia and during surgery (Go, n=20), or no active warming at a

Direct vision internal urethrotomy (DVIU) is the treatment of choice for short anterior urethral strictures. It is performed under spinal or general anesthesia although it can also be done under local anesthesia. We describe a novel method of local anesthesia for internal urethrotomy. The technique described is feasible in morbid patients who are deemed unfit for spinal or general anesthesia and achieves good analgesia.

OBJECTIVE: Both preeclampsia and neuraxial anesthesia can alter placental perfusion, potentially affecting the neonatal status. The objective of our study is to quantify the association between type of neuraxial anesthetic and short-term neonatal morbidity among preeclamptic patients undergoing cesarean delivery.
METHODS: We performed a secondary analysis of a prospective observational cohort study. Women with singleton gestations and a diagnosis of preeclampsia who underwent cesarean delivery with neuraxial anesthesia were included in the analysis. Short-term neonatal morbidities, defined as

Intrapartum trauma with spinal instability may require delivery by cesarean section (CS). While regional anesthesia is typically preferred in CS, general anesthesia may be necessary in emergent presentations that require rapid induction of anesthesia. We report a case wherein a patient presented with uterine rupture and spinal instability following a motor vehicle accident and received regional anesthesia prior to CS. A 27-year-old multiparous female at 37 + 6 weeks of gestation presented to the Emergency Department after a high-speed motor vehicle accident with significant abdominal pain and

Autonomic dysreflexia (ADR) is a syndrome of imbalanced reflex sympathetic discharge occurring in patients with spinal cord injury (SCI) at or above the level of splanchnic sympathetic outflow (T6). We present the case of a 29 year-old, gravida 3, para 1 Caucasian female at 28 weeks gestation, with a history of T3 SCI secondary to a gunshot wound 9 years prior, who developed ADR during preterm labor and received epidural anesthesia during 6 days of labor followed by spinal anesthesia for cesarean section. Spinal anesthesia may be superior to epidural anesthesia for providing hemodynamic protec

Neuraxial anesthesia provides effective anesthesia and analgesia for surgery, but may cause neurological complications. The rate of neurological complications in China remains unclear. This study conducted a retrospective epidemiological investigation of neurological complications following neuraxial anesthesia in Guangxi, southwest China. This survey used the "Golden Data" platform to distribute questionnaires to anesthesiology departments across hospitals of varying levels in Guangxi, gathering data on neuraxial anesthesia methods and associated neurological complications from 2013 to 2022.

BACKGROUND: Spinal anesthesia is considered to be the safest method of anesthesia for cesarean sections in patients with preeclampsia. Patients with preeclampsia are at an increased risk of experiencing severe hypotension following spinal anesthesia, which could have more profound and deleterious effects on both the fetus and the mother. However, bupivacaine, the most commonly used drug, can induce severe hypotension even at low doses. The purpose of this study is to minimize post-spinal hypotension in both the mother and the fetus.
OBJECTIVES: To determine and compare the reduction in hypote

Neuraxial anesthetic techniques are commonly used during the peripartum period to provide effective pain relief for labor and anesthesia during cesarean delivery. Major neurologic complications are rare after neuraxial anesthesia; however, spinal hematoma is associated with catastrophic neurologic outcomes (including lower-limb paralysis). Anticoagulant and antithrombotic drugs can increase the risk of spinal hematoma after neuraxial anesthesia, and better understanding of the pharmacokinetics and pharmacodynamics of anticoagulants has led to greater appreciation for withholding anticoagulatio

BACKGROUND: Cesarean delivery (CD) is one of the most common surgeries performed worldwide, with increasing yearly rates. Although neuraxial techniques remain the preferred anesthesia method for CD, maternal thrombocytopenia remains a prominent contraindication. Formation of spinal\epidural hematomas are extremely rare, however the minimal thrombocyte count required for safe neuraxial anesthesia is still under debate. Although transfusion of thrombocytes for the purpose of neuraxial anesthesia is still not recommended, patients with severe thrombocytopenia (less than 50 × 103/uL) are given thr

In the study of neuropathic pain, the reduction of spinal neuronal activity by an analgesic drug can inform about site and mechanistic aspects of action. Animal experiments such as in vivo electrophysiological recordings from spinal neurons, however, largely require anesthesia. The impact of the anesthesia on the interpretation of the experimental result has been mostly disregarded. Here we report major differences in basal neuronal activity and the effectiveness of morphine and gabapentin under different anesthetics in the rat neuropathic pain model of chronic constriction injury (CCI). INTRODUCTION: Proximal femur fractures have high rates of morbidity, mortality, and perioperative complications. Limiting anesthesia, especially in the elderly population, is a priority from a medical perspective. The goal of the current study is to present a technique of using local anesthetic with monitored anesthesia care (MAC) for the fixation of intertrochanteric (IT) femur fractures with cephalomedullary nailing (CMN), provide early clinical results in a small series of patients, and evaluate the safety, efficiency, and anesthetic efficacy of our technique.
