Immunotherapy in Anal Cancer.

PURPOSE OF REVIEW: Standard treatment for early-stage squamous cell cancer of the anal canal (SCCA) includes concurrent chemotherapy and radiation to achieve curative intent. Treatment options are limited, however, especially with locoregional disease relapse occurring in 20-30% of patients and about 10-30% of patients presenting with metastatic disease. With more than 90% of SCCAs occurring in the setting of HPV, immune-based therapies are now the target of possible new treatments for this rare disease. This review highlights the role of immunotherapy in HPV-associated SCCA. RECENT FINDINGS:

Treatment of squamous cell carcinoma of the anal canal: A new strategies with anti-EGFR therapy and immunotherapy.

The incidence of squamous cell carcinoma of the anal canal (SCAC) is increasing in both sexes but the standard treatment remains that of 20 years ago. However, interesting data have recently emerged on the use of anti-epidermal growth factor receptor (EGFR) agents and immunotherapy in advanced disease. Thus, new avenues of research are opening up that will hopefully lead to more effective therapeutic strategies. We provide an overview of the latest studies published on this tumor and discuss the possible future therapeutic options for combination therapy, anti-EGFR treatment and radiotherapy.

Perianal Paget disease treated with wide excision and thigh skin flap reconstruction: a case report and review of literature.

RATIONALE: Extramammary Peget disease (EMPD) is a rare tumor, which typically occurs in the perianal regions. Perianal Paget disease (PPD) was first reported in 1893, and which has only 180 cases that have been reported in literature. The rarity of the disease means that no large studies have been made, and so the optimal treatment for this disease still remains controversial. PATIENT CONCERNS: In this case, we reported a 65-years-old female patient with PPD. The patient suffered intermittent pruritus in the perianal region for 1 year. She had neither genitourinary nor gastrointestinal sympto

Performance of the marginal structural cox model for estimating individual and joined effects of treatments given in combination.

BACKGROUND: The Marginal Structural Cox Model (Cox-MSM), an alternative approach to handle time-dependent confounder, was introduced for survival analysis and applied to estimate the joint causal effect of two time-dependent nonrandomized treatments on survival among HIV-positive subjects. Nevertheless, Cox-MSM performance in the case of multiple treatments has not been fully explored under different degree of time-dependent confounding for treatments or in case of interaction between treatments. We aimed to evaluate and compare the performance of the marginal structural Cox model (Cox-MSM) to

Faecal incontinence after chemoradiotherapy in anal cancer survivors: long-term results of a national cohort.

PURPOSE: To examine the prevalence and severity of faecal incontinence amongst anal cancer survivors after chemoradiotherapy. MATERIAL AND METHODS: Anal cancer survivors from a complete, unselected, national cohort, minimum 2-years follow-up, were invited to a cross-sectional study. The St. Mark's incontinence score was used to evaluate occurrence and degree of faecal incontinence the last four weeks. The results were compared to age- and sex-matched volunteers from the general population. RESULTS: Of 199 invited survivors and 1211 volunteers, 66% and 21%, respectively, signed informed conse

Risk of anal cancer in HIV-infected and HIV-uninfected individuals in North America.

BACKGROUND: Anal cancer is one of the most common cancers affecting individuals infected with human immunodeficiency virus (HIV), although few have evaluated rates separately for men who have sex with men (MSM), other men, and women. There are also conflicting data regarding calendar trends. METHODS: In a study involving 13 cohorts from North America with follow-up between 1996 and 2007, we compared anal cancer incidence rates among 34 189 HIV-infected (55% MSM, 19% other men, 26% women) and 114 260 HIV-uninfected individuals (90% men). RESULTS: Among men, the unadjusted anal cancer incidenc

Management of internal genital warts: do we all agree? A postal survey.

A postal survey was undertaken to determine whether there was any consensus of opinion regarding the management of internal genital warts in genitourinary medicine clinics in the UK. Overall, 110 forms were returned (57% response). The majority of clinics would refer patients with cervical warts for colposcopy especially if the patient was over the age of 25 or HIV-positive. Proctoscopy or anoscopy was performed in 60% of clinics for patients with perianal warts to determine the presence of warts within the anal canal or rectum. Only 24% of patients with intra-anal warts are referred directly

HIV-related and Epstein-Barr virus-associated anal Burkitt's lymphoma: report of a case.

PURPOSE: This article describes and discusses primary Burkitt's lymphoma of the anus which is an extremely rare site of origin. METHODS AND RESULTS: A 38-year-old HIV+ Rwandan farmer had an 8-cm x 13-cm anal tumor. Histopathology and immunohistology provided evidence of an Epstein-Barr virus-associated Burkitt's lymphoma. Chemotherapy in combination of virostatic therapy is the gold standard for treatment, but because of economic constraints surgical treatment was the only practicable intervention and an abdominoperineal resection of the anorectum was performed. CONCLUSIONS: Because of the A

Increased risk of high-grade anal neoplasia associated with a human papillomavirus type 16 E6 sequence variant.

Expression of the E6 and E7 genes of human papillomavirus (HPV) type 16 have been implicated in the etiology of anogenital premalignant and malignant lesions. To evaluate whether variations in the HPV-16 E6 sequence were related to the incidence of high-grade anal neoplasia, 628 HPV-16-positive anal specimens from 193 human immunodeficiency virus (HIV)-positive and 59 HIV-negative participants were typed for variations in 15 E6 nucleotide positions. Although most participants were infected with a prototype strain, 15 (6%) carried the G131 variant, and 12 (5%) were infected with the Af1a varian

Anorectal sepsis as a presentation of occult rectal and systemic disease.

Between 1980 and 1982, 233 patients were treated for anorectal sepsis in three hospitals. The incidence of underlying disease associated with perianal sepsis and the results of surgical treatment were assessed retrospectively. Of the 233 patients who had perianal sepsis, 136 (58.4 percent) had perianal abscesses, while a further 12 (5.1 percent) had associated fistulas. Ischiorectal abscesses were found in 79 (33.9 percent) and a further two (0.9 percent) had fistulas. Four (1.8 percent) patients were found to have intersphincteric abscesses. One hundred and nine (46.8 percent) had examination

Abdomino-anal pull-through resection of the rectum.

Abdomino-anal pull-through resection of the rectum is one method of achieving resection of carcinomas involving the middle third of the rectum while allowing sphincter conservation. A number of different techniques have been described and are now reviewed along with the reported results with special reference to operative mortality, cancer curing potential, post-operative morbidity, and the quality of post operative function. Considerable variability in the published results and in the experience of different authors has prevented this procedure becoming widely adopted in the past. Its current

Colloid carcinoma arising in chronic anal fistula.

Mucinous carcinoma arising in chronic anal fistula is a rare but grave complication. Some sixty-five cases are reported in the literature. One additional case in a fistula of 47 years' duration is herein reported. The neoplastic glandular epithelium may arise from the rudimentary anal ducts or the bowel mucosa. Direct invasion occurs along the fistulous tract or adjacent tissues. Numerous cystic cavities filled with mucus are formed in the center of the process while an inflammatory reaction of the host tissue is provoked at the periphery. Hence biopsy material from the center of the process i

High prevalence and incidence of HPV-related anal cancer precursor lesions in HIV-positive women in the late HAART era.

INTRODUCTION: Anal cancer is one of the most common non-AIDS defining malignancies, especially in men who have sex with men and women living with HIV (WLHIV). OBJECTIVES: To evaluate the prevalence and incidence of precursor lesions (high-grade squamous intraepithelial lesions [HSIL]) and anal cancer in our cohort of women and to compare them to cervical lesions; to calculate the percentage of patients that acquire and clear oncogenic genotypes (HR-HPV) in the anal canal; and to determine predictive factors for anal HPV infection. PATIENTS AND METHODS: Prospective-longitudinal study (May 201

Management of idiopathic ulcer of the anal canal by excision in HIV-positive patients.

PURPOSE: The aim of this article is to show our results from the surgical treatment of idiopathic anal ulcers. METHODS: We present 33 patients with acquired immunodeficiency syndrome and idiopathic perianal ulcers. They presented with anal pain that failed to improve with medical treatment. Evaluation under anesthesia typically showed an irregular ulcer in the anal canal and an intersphincteric abscess. Surgical treatment consisted of an incision around the ulcer, its excision including margins but not its base, and abscess drainage. RESULTS: All patients experienced significant immediate po

[Human papillomavirus as a risk factor in carcinogenesis].

BACKGROUND: The causal link between Human papillomavirus (HPV) and cervical neoplasia has been established through clinical, epidemiological and molecular studies during the last two decades. In 1995, HPV 16 and 18 were classified as human carcinogens. MATERIALS AND METHODS: This paper discusses the aetiological role of HPV infection in carcinomas of the anogenital region, the respiratory and the upper digestive tract on the basis of a literature search and the author's own research. RESULTS: Almost all patients with cervical carcinomas and precursor lesions are HPV-positive. HPV is a necess
