[Analysis of risk factors for the development of pediatric appendicitis].

BACKGROUND: Appendicitis is the most common form of abdominal pain requiring surgery among children. This study evaluates the association of age, sex, seasonal, and familial factors with the histopathology of appendectomies. METHODS: 588 patients who underwent operations for acute appendicitis between 2009 and 2011 were included in this study. The patients were classified in one of three groups based on histopathological results: acute, perforated, and negative appendectomy. This study evaluates the association between age, sex, seasonal, and familial factors and the histopathological classif…

The iliohypogastric and ilioinguinal nerves. Distribution in the abdominal wall, danger areas in surgical incisions in the inguinal and pubic regions and reflected visceral pain in their dermatomes.

The courses of the iliohypogastric and ilioinguinal nerves were studied in 44 adult human cadavers, in order to clarify their relations to incisions in the abdominal wall in appendectomy, inguinal hernial repair, caesarean section and lumbar nephrectomy. If either of these nerves is trapped during suturing of the abdominal layers, especially after inguinal hernia repair and appendectomy typical nerve irritation in the inguinal region is observed. To avoid cutting the anterior branches of the iliohypogastric and ilioinguinal nerves in appendectomy, incisions should be placed at a distance of no…

Xanthogranulomatous appendicitis: A comprehensive literature review.

BACKGROUND: Xanthogranulomatous inflammation is characterized histologically by a collection of lipid-laden macrophages admixed with lymphocytes, plasma cells, neutrophils, and often multinucleated giant cells with or without cholesterol clefts. AIM: To review the medical literature on xanthogranulomatous appendicitis (XGA). METHODS: We present a patient with XGA and review published articles on XGA accessed via the PubMed, MEDLINE, Google Scholar, and Google databases. Keywords used were "appendix vermiformis," "appendectomy," "acute appendicitis," and "XGA." The search included articles pu…

Risk factors for ulcerative colitis in a Chinese population: an age-matched and sex-matched case-control study.

BACKGROUND AND AIMS: Cigarette smoking, alcohol use, appendectomy, and family history of inflammatory bowel disease (IBD) have all been shown to be associated with IBD, but there were no reports of risk factors for IBD in a Chinese population in which the incidence of IBD is increasing during the past decade. We conducted a case-control study to examine associations between previously reported environmental risk factors and development of ulcerative colitis (UC) in Wuhan city, central China. METHODS: A total of 177 patients with UC and 177 age-matched and sex-matched controls were prospective…

Benign Surgery, Malignant Rhythm: Post-laparoscopic Sick Sinus Syndrome in a Child.

Laparoscopic appendectomy is one of the most frequently performed surgical procedures in pediatric surgery and is considered highly safe. Hemodynamic changes produced by pneumoperitoneum are usually transient and clinically insignificant in otherwise healthy children. We report the case of a nine-year-old boy who developed severe postoperative bradycardia and hypotension following an uncomplicated laparoscopic appendectomy. Electrocardiography and echocardiography demonstrated findings suggestive of sick sinus syndrome with transient ventricular dysfunction requiring inotropic support. The chi…

Successful preoperative identification of fish bone causing appendicitis using 3-dimensional multidetector-CT.

A 79 years male with fishbone-induced acute appendicitis was treated surgically with laparoscopic procedure. In preoperative diagnostic study, 3-dimensional multidetector-computed tomography (3D-MDCT) demonstrated the foreign body in the appendix and 3D-reconstructed images seemed fishbone with sharp-pointed dorsal fin directing to the tip of the appendix. With these findings, surgical indication of appendectomy was confirmed and laparoscopic appendectomy was performed. He made a full recovery and was discharged in a satisfactory condition following 7 days of post-operative treatment. From thi…

Primary appendiceal Burkitt's lymphoma presenting as acute appendicitis: An extremely rare case report and review of the literatture.

Primary lymphomas of appendix are extremely rare tumors. The incidence is 0.015% out of all gastrointestinal lymphomas; furthermore, limited data is available in literature. The appendiceal neoplasms are most commonly presented as acute appendicitis followed intestinal obstruction, intussusception or perforation. We present a case of a 22 year-old male patient who presented with acute appenditis and underwent emergency laparotomy. On abdominal exploration, swollen and enlarged appendix measured 3cm was present for which appendectomy were performed. The histopathological examination of appendec…

It's time to deconstruct treatment-failure: A randomized controlled trial of nonoperative management of uncomplicated pediatric appendicitis with antibiotics alone.

BACKGROUND: Published data demonstrate that management of uncomplicated pediatric appendicitis with antibiotics-alone is safe and frequently successful. Randomized controlled trials (RCT) comparing antibiotics-alone to appendectomy are lacking, alongside insight into drivers of failure. We sought to validate the antibiotics-alone approach and identify barriers to success using an RCT design. METHODS: Patients aged 6-17 years with uncomplicated appendicitis were randomized to appendectomy or intravenous piperacillin/tazobactam for 24-48 h followed by 10 days of oral ciprofloxacin/metronidazole…

Schistosomal appendicitis: a case series.

Schistosomiasis is a neglected tropical disease with significant morbidity, yet appendicitis secondary to schistosomal infestation remains rare, even in endemic regions. We report a series of five cases of acute appendicitis in adult patients, where histopathological examination revealed schistosomal appendicitis. All patients presented with classical features of acute appendicitis, underwent appendectomy, and recovered uneventfully. One case demonstrated mural calcifications on preoperative imaging, raising suspicion of schistosomiasis preoperatively. Histology confirmed the diagnosis in all …

Synchronous presentation of acute acalculous cholecystitis and appendicitis: a case report.

INTRODUCTION: Acute acalculous cholecystitis is traditionally associated with elderly or critically ill patients. CASE PRESENTATION: We present the case of an otherwise healthy 23-year-old Caucasian man who presented with acute right-sided abdominal pain. An ultrasound examination revealed evidence of acute acalculous cholecystitis. A laparoscopy was undertaken and the dual pathologies of acute acalculous cholecystitis and acute appendicitis were discovered and a laparoscopic cholecystectomy and appendectomy were performed. CONCLUSION: Acute acalculous cholecystitis is a rare clinical entity…

[Principles of diagnostics and treatment of chronic appendicitis].

Treatment results of 101 patients, operated on with the diagnosis of the chronic appendicitis, were analyzed. Of them, 55 had periodic right iliac pain syndrome, the rest 46 had a history of appendicular abscess or infiltrate. 58 patients were operated on laparoscopically, the rest had traditional open appendectomy. The use of ultrasound and roentgen diagnostics proved to be non-effective. The reliable laparoscopic symptoms of chronic appendicitis were singled out. The laparoscopy provided the correct diagnosis in 93.3% of patients and allowed avoiding the groundless appendectomy in 31.2%. The…

Measles and appendicitis.

Two patients who were admitted in the prodromal stage of measles with right lower quadrant pain are described. One patient underwent appendectomy. Histologic examination of the appendix showed the characteristic Warthin-Finkeldey giant cells in the subepithelial layer and allowed the pathologist to predict a measles rash before it appeared. The second patient's pain resolved spontaneously and the measles rash appeared just prior to discharge from the hospital. A discussion of the association between measles and appendicitis is presented. It is concluded that although the association between me…

Incidentally discovered, intentionally managed: a narrative review of the last decade's updates on the management of neuroendocrine tumor of the appendix.

Appendiceal neuroendocrine neoplasms (aNENs) represent an increasingly recognized and heterogeneous subset of gastroenteropancreatic neuroendocrine tumors (NETs), most often identified incidentally on appendectomy specimens. Postoperative management relies on accurate risk stratification grounded in histopathological assessment and aligned with major international guidelines (WHO, ENETS), which distinguish between well-differentiated NETs (NET grades 1 to 3) and poorly differentiated neuroendocrine carcinomas. Critical prognostic determinants shaping therapeutic decision-making include tumor s…

Misses and Near Misses in Paediatric Appendicitis: An Eight-Year, Single-Centre Retrospective Review.

AIM: Diagnostic error can result in the appendectomy of a normal appendix, commonly known as negative appendectomy (NA). Missed appendicitis (MA) is related to a poor outcome. The aim of this study was to determine whether there are factors in presentation associated with NA or MA. METHODS: This is a single-centre, retrospective chart review of histology-confirmed appendicitis over an eight-year period. We defined MA as appendicitis diagnosed within the week of a paediatric emergency department discharge for a similar presentation. Negative appendectomy was defined as a histologic examination…

No association between hospital volume and short-term outcomes of some common surgeries: a retrospective cohort study based on a Japanese nationwide database.

PURPOSE: Centralization of high-risk surgeries has become a widespread strategy. However, whether or not the hospital volume affects the outcomes of common surgeries remains unclear. This study explored the association between hospital volume and short-term outcomes of common surgeries, as represented by appendectomy, cholecystectomy, and pneumothorax surgery, by analyzing data from a Japanese nationwide database. METHODS: All hospitals were categorized into four groups (very low-, low-, high-, and very high-volume) according to the annual hospital volume of all gastrointestinal surgeries or …

Appendix-preserving elective herniorrhaphy for de Garengeot hernia: two case reports.

BACKGROUND: Emergency appendectomy is often performed for de Garengeot hernia. However, in some cases, there may be a chance to perform an appendix-preserving elective surgery. CASE DESCRIPTION: A 76-year-old woman presented to our hospital with complaints of a right inguinal swelling, which we diagnosed as a de Garengeot hernia using computed tomography (CT). B-mode ultrasonography (US) of the mass showed an appendix 4-6 mm in diameter with a clear wall structure; color Doppler US showed pulsatile blood flow signal in the appendiceal wall. Twenty-eight days later, herniorrhaphy with transabd…
