Giant lymph node hyperplasia is a lesion of lymphoid tissue, which may be accompanied by systemic manifestations like fever, anaemia and hyperglobulinaemia. A case is described with temporal arteritis, an association hitherto not described in the literature.

An elderly man developed temporal arteritis and polymyalgia rheumatica with coexisting biochemical abnormalities of liver function. Biopsy revealed hepatic changes which have not been previously reported. There was hepatocellular necrosis and inflammation together with a prominent hyperplasia of perisinusoidal lipocytes of Ito. Temporal artery biopsy confirmed the presence of granulomatous panarteritis. Corticosteroid therapy produced rapid resolution of symptoms and reversion of liver function tests to normal.

The association of an inflammatory colitis or enterocolitis with Takayasu's disease is very rare. The case presented here in a female patient treated for four years is compared to the eight previously described patients. These subjects are young women with digestive disease, ulcerative colitis, Crohn's disease or unclassified colitis which is usually discovered before the arterial disorder. Clinical symptoms and evolution of each disease are not modified by their association. Extra-intestinal involvement of eyes, joints, skin or pericardium may be found. These manifestations, the association o

Sera from 74 patients with polymyalgia rheumatica or giant cell arteritis or both were tested for immune complexes by using the Raji cell radioimmunoassay. Levels in patients with active disease were higher than in patients whose disease had become inactive. There was no difference in levels of immune complex-like materials between patients with polymyalgia rheumatica alone and those with giant cell arteritis. Density gradient analysis of one serum showed immune complex-like materials mainly in the 19S region. Immune complexes may be important in the pathogenesis of these conditions.

A rare case of giant cell arteritis inducing an aneurysmal dilatation in the peripheral branch of the middle cerebral artery was presented. The lesion was solitary and no extra-cerebral vasculature was involved. The literature of giant cell arteritis and of similar disorders involving the intracranial arteries was reviewed.

Temporal (cranial, giant cell) arteritis and polymyalgia rheumatica are probably different expressions of the same inflammatory disease. The disease occurs mainly in the elderly and is characterized in the early stage by vague, nonspecific symptoms. Tenderness of the temporal arteries is diagnostic but rarely is present early. Unfortunagely, loss of vision usually occurs before the diagnosis is made. A high index of suspicion and frequent testing of the ESR in elderly patients with vague complaints may lead to earlier diagnosis and appropriate therapy and thus may prevent blindness and even de…

A 17-year-old boy with partial hypoxanthine-guanine phosphoribosyl transferase deficiency developed a hypersensitivity reaction to allopurinol. The reaction was manifested by the development of bizarre, atypical seizures. The patient had been neurologically normal prior to the reaction. Seizures disappeared following discontinuation of allopurinol therapy. Allopurinol apparently can cause a diffuse vasculitis involving cerebral vessels after many year of therapy, resulting in atypical seizures.

Arterial injuries attributed to irradiation are most frequent about the head and neck and are invariably triggered by infection, infarction, … n be superimposed almost exactly. Neither author is entirely happy with the diagnosis of 'radiation arteritis' for the reasons discussed, but the coincidence of radiation fields and areas of arterial damage makes the role of irradiation indicating.

The seven-year results of all cases (84) diagnosed as polymyalgia rheumatica or giant cell arteritis are reported. The diagnosis proved to be incorrect in seven, of which six had a polyarthritis. Most cases were treated with prednisolone, starting with 20 mg daily for those with evidence of cranial arteritis, and 10 mg for those without. Fourteen patients were withdrawn from treatment (after three months to 31/2 years--mean 21 months), but three relapsed and treatment has been restarted. There was no correlation between the presence or absence of arteritis, the starting dose of prednisolone an…

Giant cell arteritis (GCA) is a systemic granulomatous vasculitis of medium and large-sized arteries, which represents an ophthalmic emergency because of the risk of sudden, severe and often irreversible loss of vision. Its most important ocular manifestation is arteritic anterior ischemic optic neuropathy; less frequent manifestations include central retinal artery occlusion, cilioretinal artery occlusion, posterior ischemic optic neuropathy, choroidal ischemia and ocular motor nerve palsies. Giant cell arteritis should be actively considered in every patient over 50 years of age presenting …

Giant cell arteritis (GCA) represents one of the most diagnostically challenging systemic vasculitides, characterized by its heterogeneous clinical presentation, lack of pathognomonic features, and potential for devastating complications, with a special concern for irreversible vision loss. This comprehensive review synthesizes current evidence regarding the multifaceted diagnostic challenges in GCA, incorporating recent advances in classification criteria, imaging technologies, biomarker research, and emerging therapeutic strategies.

BACKGROUND: Rheumatoid arthritis (RA) and giant cell arteritis (GCA) are systemic autoimmune diseases with distinct clinical manifestations that are driven by dysregulation of multiple cell populations of the innate and adaptive immune systems.
OBJECTIVE: The aim of this work is to describe and compare the role of central immune cell populations in RA and GCA.
MATERIAL AND METHODS: This review is based on a comprehensive literature search in PubMed. Recent publications on immune cell populations in RA and GCA were assessed for their pathogenic significance and therapeutic implications.

BACKGROUND: Coronary involvement in Takayasu arteritis is uncommon but associated with high rates of in-stent restenosis and challenging revascularization.
CASE SUMMARY: A 22-year-old woman with Takayasu arteritis and congenital antithrombin III deficiency presented with non-ST-segment elevation myocardial infarction due to severe ostial right coronary artery in-stent restenosis after multiple prior percutaneous interventions. Intravascular ultrasound confirmed significant neointimal hyperplasia. A stent-sparing strategy using excimer laser coronary atherectomy, cutting balloon angioplasty, a…

Refractory hypothyroidism is not uncommon in patients treated with oral levothyroxine, and it represents a challenge for clinicians as regards a physiopathological diagnosis and the most appropriate long-term therapeutic management in order to restore normal TSH concentrations.

Takayasu arteritis (TAK) is often associated with other inflammatory diseases. Here, we describe two Japanese patients with TAK complicated by synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome. Both patients presented with anterior chest pain as their chief complaint and were diagnosed with TAK following SAPHO syndrome. Treatment with glucocorticoids and biological agents led to a rapid improvement in symptoms. A review of the literature identified 11 additional cases of TAK complicated by SAPHO syndrome. SAPHO syndrome frequently precedes TAK, with the diagnostic interv…
