Interfacing the body's own sensing receptors into neural prosthesis devices.

Functional Electric Stimulation (FES) is today available as a tool in muscle activation used in picking up objects, in standing and walking, in controlling bladder emptying, and for breathing. Despite substantial progress over nearly three decades of development, many challenges remain to provide a more efficient functionality of FES systems. The most important of these is an improved control of the activated muscles. Instead of artificial sensors for feedback, new developments in electrodes to do long-term and reliable recordings from peripheral nerves emphasize the use of the body's own sens

The effects of biofeedback assisted breathing retraining on lung functions in patients with cystic fibrosis.

This study examines the effects of respiratory muscle feedback and breathing retraining (BRT) on lung function in patients with cystic fibrosis (CF). Twenty-six patients with CF were matched for age and severity of disease. Standard respiratory spirometry was performed on all subjects before and after biofeedback training. Thirteen experimental subjects underwent eight sessions of pneumographic or strain-gauge feedback from the abdominal muscles and electromyogram feedback from accessory respiratory muscles to assist in learning diaphragmatic and pursed-lips breathing maneuvers. Control subjec

Female Sexual Function and Pelvic Floor Muscle Training: A Narrative Review.

Female sexual dysfunction (FSD) is a multifactorial condition affecting desire, arousal, orgasm, and satisfaction, with wide-ranging implications for women's physical and emotional well-being. Although prevalent, especially among postmenopausal and postpartum populations, FSD remains under-recognized and undertreated. Pelvic floor muscle training (PFMT) has emerged as a promising, non-invasive therapeutic approach for managing FSD, particularly when associated with pelvic floor disorders such as urinary incontinence, pelvic organ prolapse, and overactive bladder. This narrative review synthesi

The use of biofeedback intervention in the improvement of depression levels: a randomised trial.

OBJECTIVE: To evaluate the use of biofeedback intervention in the levels of depression. The main hypothesis tested if the use of biofeedback improves depression levels compared to the control group. METHODS: A randomised clinical trial. The final sample was composed of 36 participants (18 in the experimental group, receiving 6 training, once a week, with biofeedback; and 18 in the control group, who received conventional treatment in the service).Outcome measures were assessed in two stages: pre-test and post-test. The research used the following instruments: demographic survey data, Mini Int

Further Exploration of Treatment Response in Latinos with Comorbid Asthma and Panic Disorder: A Brief Report of HRV and ETCO2 as Potential Mediators of Treatment Response.

Heart rate variability (HRV) and end tidal CO2 (ETCO2) in relation to treatment response have not been studied in Latino populations or in comorbid asthma and panic disorder (PD). An extension of previously published research, the current study explored psychophysiological variables as possible mediators of treatment response. Latino treatment completers (N = 32) in the Bronx with asthma-PD received either Cognitive-Behavioral Psychophysiological Therapy (CBPT) or Music Relaxation Therapy (MRT). CBPT included HRV-biofeedback (HRVB); in-the-moment heart rate data to help an individual learn to

Gait biofeedback and impairment-based rehabilitation for chronic ankle instability.

Our purpose was to analyze the effects of 4 weeks of visual gait biofeedback (GBF) and impairment-based rehabilitation on gait biomechanics and patient-reported outcomes (PROs) in individuals with chronic ankle instability (CAI). Twenty-seven individuals with CAI participated in this randomized controlled trial (14 received no biofeedback (NBF), 13 received GBF). Both groups received 8 sessions of impairment-based rehabilitation. The GBF group received visual biofeedback to reduce ankle frontal plane angle at initial contact (IC) during treadmill walking. The NBF group walked for equal time du

Effect of pelvic floor muscle training (PFMT) during pregnancy on bladder neck descend and delivery.

BACKGROUND: Pelvic floor muscle exercises (PFME) are commonly recommended during pregnancy and after birth for both prevention and treatment of lower urinary tract symptoms (LUTS). Stress urinary incontinence has long been presumed to be associated with urethral hypermobility. Pregnancy and delivery are known cause of increasing bladder neck descent (BND). OBJECTIVE: The present study aimed to determine the effect of antenatal PFME on bladder neck descent in nulliparous pregnancy. MATERIAL AND METHOD: 219 nulliparous women pregnant between 8-12 weeks were interviewed and then underwent trans

Three-dimensional endoanal ultrasound and anorectal manometry in children with anorectal malformations: new discoveries.

BACKGROUND/PURPOSE: Fecal incontinence and constipation are common problems in follow-up of anorectal malformations (ARMs). We evaluated the anal sphincters using the 3-dimensional endoanal ultrasonography (3D-EAUS) and the anorectal manometry after ARMs repair. METHODS: Seventeen patients, divided into 3 groups according to Wingspread classification, underwent anorectal manometry and 3D-EAUS. Clinical, manometric, and endosonographic scoring systems were used. RESULTS: The average anal resting pressure (aARP) was significantly higher in low ARMs than in intermediate and high ARMs. The anal

Diagnosing vocal cord dysfunction in young athletes.

PURPOSE: To provide an overview of the pathophysiology, steps in making a diagnosis, differential diagnosis, and treatment methods for vocal cord dysfunction (VCD) in young athletes. DATA SOURCES: Review of published literature about VCD and exercise-induced asthma (EIA) and a case study. CONCLUSIONS: The clinical presentation of VCD is often confusing. A young athlete who is having difficulty "catching his breath" may have more than EIA. Young athletes who have been previously diagnosed with EIA may actually have VCD. IMPLICATIONS FOR PRACTICE: The ability to correctly differentiate VCD fr

[A computerized endurance test: efficacy of pelvic floor rehabilitation treatment in patients with stress incontinence].

In the present study the authors wanted to transfer rigorous methods of study, already in use in other sectors (sports, medicine, isokinetic work, etc.) of the validity of interventions made and their effectiveness, into an "emerging" field, that of perineal rehabilitation. 15 female patients, aged between 35 and 45, affected by stress incontinence underwent a baseline clinico-instrumental evaluation of the perineal floor including a computerized test of endurance. The patients then embarked upon a standardized rehabilitative perineal training lasting a month and at the end underwent an identi

Standard gastroenterologist versus multidisciplinary treatment for functional gastrointestinal disorders (MANTRA): an open-label, single-centre, randomised controlled trial.

BACKGROUND: Functional gastrointestinal disorders are common and costly to the health-care system. Most specialist care is provided by a gastroenterologist, but only a minority of patients have improvement in symptoms. Although they have proven to be effective, psychological, behavioural, and dietary therapies are not provided routinely. We aimed to compare the outcome of gastroenterologist-only standard care with multidisciplinary care. METHODS: In an open-label, single-centre, pragmatic trial, consecutive new referrals of eligible patients aged 18-80 years with Rome IV criteria-defined func

Longitudinal development of frontoparietal activity during feedback learning: Contributions of age, performance, working memory and cortical thickness.

Feedback learning is a crucial skill for cognitive flexibility that continues to develop into adolescence, and is linked to neural activity within a frontoparietal network. Although it is well conceptualized that activity in the frontoparietal network changes during development, there is surprisingly little consensus about the direction of change. Using a longitudinal design (N=208, 8-27 years, two measurements in two years), we investigated developmental trajectories in frontoparietal activity during feedback learning. Our first aim was to test for linear and nonlinear developmental trajector

Gait retraining and incidence of medial tibial stress syndrome in army recruits.

PURPOSE: Gait retraining, comprising biofeedback and/or an exercise intervention, might reduce the risk of musculoskeletal conditions. The purpose was to examine the effect of a gait-retraining program on medial tibial stress syndrome incidence during a 26-wk basic military training regimen. METHODS: A total of 450 British Army recruits volunteered. On the basis of a baseline plantar pressure variable (mean foot balance during the first 10% of stance), participants classified as at risk of developing medial tibial stress syndrome (n = 166) were randomly allocated to an intervention (n = 83) o

Tactile intervention as a novel technique in improving body stability in healthy elderly and elderly with diabetes.

BACKGROUND: Body sway increases in the elderly because of normal aging and high incidence of disease such as diabetes. Prevalence of sway is greater in the elderly with diabetes because of damage to the central and peripheral nervous systems. Increase in body sway is associated with an elevated risk of falling. Falling is one of the major causes of morbidity and mortality in the elderly. The purpose of this study was to develop a new technique to improve body stability and decrease body sway in the elderly people with or without diabetes. SUBJECTS AND METHODS: Twenty-two subjects--12 elderly

Perineal retraining improves conservative treatment for faecal incontinence: a multicentre randomized study.

BACKGROUND: Anal incontinence is a frequent complaint that profoundly affects quality of life. Our aim was to determine whether perineal retraining gives additional benefits to standard medical treatment. METHODS: Patients with anal incontinence and a Wexner score >4 were randomly assigned to standard conservative treatment (control) or perineal retraining, including biofeedback, in addition to standard treatments (biofeedback). Diaries, self-administered questionnaires and satisfaction scores quantified the benefits. Self-evaluated improvement was the primary outcome measure. A score ≥3 (in

Temporomandibular disorders and facial pain: a psychophysiological perspective.

This article presents a psychophysiological perspective on temporomandibular muscle and joint disorders (TMJD) and facial pain. After a brief introduction to TMJD, the article presents data, largely derived from work carried out in my laboratory, that address four questions: (1) What are the consequences of parafunctional activities? (2) Do TMJD patients engage in parafunctional activities? (3) Why are TMJD patients unaware of these activities? and (4) What are the implications of these findings for treatment? The findings suggest that low-level parafunctions increase pain in otherwise pain-fr
