Progressive microbial alterations in the human gut and lung across early life: implications for translational medicine.

Early life represents a critical window for the establishment and maturation of the human microbiome. The sterile womb paradigm remains dominant in the field, but in utero microbial colonization is highly controversial due to challenges related to low-biomass contamination. After birth, the intestinal and pulmonary microbial communities undergo a programmed succession shaped by factors such as delivery mode, feeding practices, and environmental exposures, and they bidirectionally regulate host immune and metabolic homeostasis via the gut-lung axis. Dysbiosis during early life is closely associ

Human milk microbiome as a modulator of the early-life gut-brain axis: mechanisms and translational opportunities for neurodevelopment.

The first 1000 days of life represent a critical developmental window during which early microbial colonization contributes to immune, metabolic, and neurodevelopmental programming. Human milk is increasingly recognized as a biologically active fluid that shapes infant gut microbiome assembly through microorganisms, human milk oligosaccharides (HMOs), immune factors, and microbial metabolites. Within this framework, the human milk microbiome has emerged as a potential contributor to the milk-gut-brain axis (MGBA), a bidirectional communication network linking the gut microbiota with neural, im

From patient advocacy to patient-driven research: Building active partnerships beginning at the bench to reach the bedside.

Patient advocates offer lived experience that can shape research priorities, methods, and outcomes. Patient engagement contributes valuable resources to research, including meaningful additions to research design, definition of research priorities, and dissemination of outcomes. It is therefore critical that patients and patient advocates be part of the biomedical research process; which can be achieved by changing the perspective of patients as the recipients of research outcomes, to being partners in the process. Patient, clinician, and researcher interactions are particularly important in r…

Harnessing ovarian cancer ascites for translational science: models, biomarkers, and therapeutics.

Ovarian cancer is one of the most lethal gynecological malignancies and is often associated with fluid build-up in the peritoneal cavity, known as ascites. Nearly one-third of patients with ovarian cancer present with ascites at the time of initial diagnosis, and more frequently with recurrent ovarian cancer. Ascites is a uniquely valuable tool for research, as it is representative of both the tumor and its microenvironment. Ascites is composed of cells (single cells and multicellular aggregates) and acellular components that contribute to the development of peritoneal metastasis and chemoresi

Future of Team-based Basic and Translational Science in Radiation Oncology.

To further optimize radiotherapy, a more personalized treatment towards individual patient's risk profiles, dissecting both patient-specific tumor and normal tissue response to multimodality treatments is needed. Novel developments in radiobiology, using in vitro patient-specific complex tissue resembling 3D models and multiomics approaches at a spatial single-cell level, may provide unprecedented insight into the radiation responses of tumors and normal tissue. Here, we describe the necessary team effort, including all disciplines in radiation oncology, to integrate such data into clinical pr

Spinocerebellar Ataxia Type 3 Pathophysiology-Implications for Translational Research and Clinical Studies.

The spinocerebellar ataxias (SCA) comprise a group of inherited neurodegenerative diseases. Machado-Joseph Disease (MJD) or spinocerebellar ataxia 3 (SCA3) is the most common autosomal dominant form, caused by the expansion of CAG repeats within the ataxin-3 (ATXN3) gene. This mutation results in the expression of an abnormal protein containing long polyglutamine (polyQ) stretches that confers a toxic gain of function and leads to misfolding and aggregation of ATXN3 in neurons. As a result of the neurodegenerative process, SCA3 patients are severely disabled and die prematurely. Several screen

How research can help control tuberculosis.

Tuberculosis (TB) has played a central role in the history of biomedical science from Koch onwards. Research in the nineteenth and twentieth centuries yielded extremely valuable diagnostic, therapeutic and preventive tools for the control of TB. Following the development of shortcourse chemotherapy in the 1970s and 1980s, research into TB virtually evaporated. Despite the availability of an array of tools, TB control faltered, and the disease remains a major killer. The failure of the fruits of scientific research to control TB is a result of the shortcomings of the tools themselves as well th

Academic nightmares: Predatory publishing.

Academic researchers who seek to publish their work are confronted daily with a barrage of e-mails from aggressive marketing campaigns that solicit them to publish their research with a specialized, often newly launched, journal. Known as predatory journals, they often promise high editorial and publishing standards, yet their exploitive business models, poor quality control, and minimal overall transparency victimize those researchers with limited academic experience and pave the way for low-quality articles that threaten the foundation of evidence-based research. Understanding how to identif

Transportability Analyses in Comparative Effectiveness Research: A Conceptual Framework and Methodological Principles Endorsed by the International Society for Pharmacoepidemiology.

THE PROBLEM: Transportability considerations are increasingly important to answer research questions in comparative effectiveness research (CER) to support the transfer of evidence on medicinal products across countries or settings. As drug development costs rise and healthcare systems and professionals face economic and resource pressures, leveraging existing data and evidence generated across borders or settings can improve efficiency and inform decisions in product development and decision-making (regulatory, health technology assessment [HTA] and clinical care). Differences in population c

Ethical considerations for research involving pregnant women with HIV and their children.

Pregnant and postpartum women with HIV (PPWH) and their children have historically been excluded from certain research in the name of protection. This may, however, inadvertently exacerbate health disparities. While calls for their inclusion have increased, additional practical guidance to achieve this goal is needed. Within this editorial, we provide practical recommendations for enabling PPWH and their children to participate ethically in research by identifying and addressing issues that potentially put them at risk. We ground this discussion in a framework that considers the various vulner

A Fresh Look at Problem Areas in Research Methodology in Nutrition.

This paper makes a critical evaluation of several of the research methods used to investigate the relationship between diet, health, and disease. The two widely used methods are randomized controlled trials (RCTs) and prospective cohort studies. RCTs are widely viewed as being more reliable than cohort studies and for that reason are placed higher in the research hierarchy. However, RCTs have inherent flaws and, consequently, they may generate findings that are less reliable than those from cohort studies. The text presents a discussion of the errors that may occur as a result of confounding.

Transparency and ongoing communication with participants in brain organoid research: Consensus of an interdisciplinary working group.

Stem cell-based models of the human brain benefit from biospecimens that can be used for a broad range of future research. But current regulations do not address the desire of research participants to remain engaged beyond initial biospecimen donation. We present practicable strategies for engaging participants while preserving scientific potential.

Methods matter: Current and future practices for middle ear mechanics laboratories.

Accurate experimental measurement of middle ear mechanics is critical for both basic auditory research and clinical applications. Although numerous experimental studies have characterized middle ear function, structured guidance for selecting appropriate measurement techniques is limited, which can result in suboptimal experimental designs. In this article, we present a systematic, three-phase framework for method selection in middle ear research. Phase 1 defines project-specific parameters based on the research question, Phase 2 maps these parameters to relevant physical quantities, and Phase
