Candidemia is one of the most frequent opportunistic mycoses worldwide. Limited epidemiological studies in Latin America indicate that incidence rates are higher in this region than in the Northern Hemisphere. Diagnosis is often made late in the infection, affecting the initiation of antifungal therapy. A more scientific approach, based on specific parameters, for diagnosis and management of candidemia in Latin America is warranted. 'Recommendations for the diagnosis and management of candidemia' are a series of manuscripts that have been developed by members of the Latin America Invasive Myco

The immunosuppressive individuals are highly prone to get afflicted with invasive opportunistic fungal infections such as Candidiasis, Aspergillosis, Histoplasmosis, Coccidioidomycosis, Blastomycosis, Penicilliosis, Cryptococcosis and Zygomycosis which are becoming a cause of concern to the mankind due to their high morbidity and mortality rates. The existing antifungal agents are not completely effective due to their severe side-effects and recurrent drug resistance in fungi. Hence, there is an urgent need to develop newer and better antifungal drugs. The enzyme Glucosamine-6-phosphate (G-6-P

HIV infection results in profound alterations of immunologic function that render the patient severely immunocompromised, and susceptible to malignancies and opportunistic infections. Three AIDS-defining malignancies include Kaposi's sarcoma (KS), non-Hodgkin's lymphoma (NHL) and invasive cervical cancer. In AIDS patients, KS is often aggressive and multifocal, with visceral involvement and widespread cutaneous and nodal spread; NHL is always high grade and often widely disseminated at the time of diagnosis with frequent involvement of extranodal sites; cervical cancer is invasive and has grea

Invasive fungal infections are major medical complications in immunocompromised patients. The recent rise in the incidence of cancer and the increased use of newer medical treatment modalities, including organ transplantations, have resulted in growing numbers of highly immunosuppressed individuals. Although aspergillosis and candidiasis are among the most common invasive mycoses in such patients, there is evidence that the incidence of infectious diseases caused by Zygomycetes has risen significantly over the past decade. Patients with diabetes, malignancies, solid organ or bone marrow transp

With the increased number of immunocompromised patients there has been a concomitant increase in patient morbidity and mortality due to fungi. The etiologic microorganisms vary depending upon the type of immune dysfunction. Patients with malignancies and chemotherapy-induced neutropenia commonly are infected with Candida and Aspergillus. Other ubiquitous fungi such as Rhizopus, Fusarium, and Trichosporon are more frequently implicated as agents of disease in these patients. Patients with cell-mediated immune dysfunction such as acquired immune deficiency syndrome (AIDS) are susceptible to muco

INTRODUCTION: Acute leukemia accounts for more than 30% of all pediatric cancer cases, and of these, 15-20% are acute myeloid leukemia (AML). Children who super from AML are more likely to develop infections due to the humoral and cellular immune deficits generated by the disease and its treatment. The incidence of fungal infections is underestimated; reports show that up to 75% of fungal infections go undiagnosed until autopsy. In only 30 years, the incidence of invasive candidiasis has increased by 40-fold. Thus, the high morbidity and mortality associated with fungal infections in hematolog

C. albicans is a commensal organism present in the human microbiome of more than 60% of the healthy population. Transition from commensalism to invasive candidiasis may occur after a local or a general failure of host's immune system. This transition to a more virulent phenotype may reside either on the capacity to form hyphae or on an acquired resistance to antifungal drugs. Indeed, overexpression of genes coding drug efflux pumps or adhesins, cell wall proteins facilitating the contact between the fungus and the host, usually marks the virulence profile of invasive Candida spp. In this paper

In patients with hematologic malignancies due to immune system disorders, especially persistent febrile neutropenia, invasive fungal infections (IFI) occur with high mortality. Aspergillosis, candidiasis, fusariosis, mucormycosis, cryptococcosis and trichosporonosis are the most important infections reported in patients with hematologic malignancies that undergo hematopoietic stem cell transplantation. These infections are caused by opportunistic fungal pathogens that do not cause severe issues in healthy individuals, but in patients with hematologic malignancies lead to disseminated infection

Candida glabrata is an opportunistic fungal pathogen and a leading cause of invasive candidiasis, particularly in immunocompromised patients, where treatment is increasingly compromised by intrinsic and acquired antifungal resistance. Despite lacking morphological plasticity, C. glabrata employs distinct virulence strategies, including adhesin-mediated host colonization, intracellular survival within phagocytes, stress tolerance, iron acquisition and biofilm formation. This review synthesizes current knowledge of C. glabrata virulence and antifungal resistance mechanisms, with a particular foc

The escalating threat of drug-resistant fungal infections, particularly invasive candidiasis caused by Candida albicans, demands the development of novel antifungal agents. Herein, we report the synthesis and evaluation of a series of half-sandwich arene ruthenium(II) complexes bearing polypyridyl ligands. Among them, a lead complex Ru-6 [(η6-p-cymene)Ru(Br-Me-dppz)Cl]PF6, with Br-Me-dppz = 11-bromo-12- methyldipyrido [3,2-a:2',3'-c] phenazine, exhibited potent and broad-spectrum antifungal activity against C. albicans and other drug-resistant fungal pathogens. Ru-6 showed superior in vitro fu

Heterozygous gain-of-function (GOF) variants in STAT1 are the most frequent genetic cause of chronic mucocutaneous candidiasis (CMC). Data from Africa remain scarce. We conducted a cross-sectional study at Ibn Rochd University Hospital, Casablanca (2016-2021), enrolling patients with persistent or recurrent mucocutaneous and/or invasive fungal disease. Whole-exome and/or Sanger sequencing were performed, with pathogenicity assessed using ACMG/AMP criteria. We identified 12 patients from 11 kindreds carrying 10 distinct heterozygous STAT1 GOF variants. CMC was the dominant manifestation (11/12,

The alarming rise in fungal infections poses a serious health concern worldwide. Candida albicans is a highly virulent, ubiquitous opportunistic fungal pathogen associated with high mortality, particularly in immunocompromised individuals. Its infections range from superficial mucosal to invasive candidiasis. The pathogenicity of C. albicans is largely attributed to its ability to undergo phenotypic switching between different morphological forms ranging from unicellular to hyphae. A key factor in its pathogenesis is candidalysin, a fungal cytolytic peptide toxin. Candidalysin is generated thr

Invasive Candidiasis infections are a clinical challenge, with limited effective therapeutic agents and increasing resistance. The discovery of new antifungal agents is urgently required. Here, we developed a new series of 2-methyl-1,4-naphthoquinone (Menadione) Tethered to 1H-1,2,3-triazolyl-selenoester in good yields, which exhibit antifungal potential activity against Candida species. Antifungal and antibiofilm activities for the twenty-one compounds were performed in vitro against Candida spp and exhibited inhibitory effects at different concentrations for several Candida species. Two comp

BACKGROUND: Invasive candidiasis and persistent fungal colonization of the airways are increasingly recognized as potential barriers to successful weaning from mechanical ventilation in critically ill patients. Among non-albicans Candida species, Candida glabrata, recently reclassified as Nakaseomyces glabrata, is particularly challenging due to its unique mechanisms of resistance and immune evasion. The use of aerosolized liposomal amphotericin B (Ambisome®) offers a promising local therapeutic approach, combining targeted delivery with a favorable safety profile.

Invasive candidiasis affects 1.6 million people annually, with high mortality among immunocompromised and hospitalized patients. Echinocandins are frontline antifungals, but rising resistance limits their efficacy. Here, we show that Candida albicans and multidrug-resistant Candida auris share a conserved cell wall architecture yet differ markedly in their adaptive responses to echinocandins. Solid-state NMR reveals that both species possess a rigid inner layer of tightly associated chitin microfibrils and β-1,3-glucans, supported by a flexible matrix of β-1,6-glucans and additional β-1,3-gluc

Invasive C. albicans infections result in high mortality rates. While IL-1 is important to combat C. albicans infections, the underlying mechanisms remain unclear. Using global and conditional Il1r1 knockouts in mice, here we show that IL-1R signaling in non-hematopoietic cells in the kidney and brain is crucial for a protective response. In the kidney, endothelial IL-1R contributes to fungal clearance independent of neutrophil recruitment, while IL-1R in hematopoietic cells is dispensable. IL-1R signaling indirectly recruits neutrophils and monocytes in the brain by regulating chemokines and

Candida albicans is a common commensal fungus and fourth most frequent causative agent of nosocomial infections including life-threatening invasive candidiasis in humans. The effectiveness of present antifungal therapies using azoles, polyenes, flucytosine and echinocandins has plateaued in managing fungal infections. The limitations of these antifungal drugs are related to polymorphic morphology, biofilm formation, emergence of drug-resistant strains and production of several virulence factors. Development of new antifungal agents, which can particularly afflict multiple cellular targets and

INTRODUCTION: Febrile neutropenia is a common complication experienced by children with cancer or those undergoing hematopoietic stem cell transplantation. Repeated episodes of febrile neutropenia result in cumulative exposure to broad-spectrum antibiotics with potential for a range of serious adverse effects. Short-course antibiotics, even in patients with high-risk febrile neutropenia, may offer a solution.
AREAS COVERED: This review addresses the known broad effects of antibiotics, highlights developments in understanding the relationship between cancer, antibiotics, and the gut microbiome
