Genetic counselling for hypertrophic cardiomyopathy: are we ready for it?

Hypertrophic cardiomyopathy (HCM) is a dominant genetic disorder of the myocardium associated with dysfunctional contractile proteins. The m … er identification or - theoretically - preclinical risk stratification. A scheme according to which cardiologists and clinical and molecular geneticists could cooperate in counselling patients and managing HCM clinically is proposed.

[Ehlers-Danlos disease revealed during pregnancy through the diagnosis of aortic dissection].

Dissection of the aorta is a serious condition but rare in young women, and occurring during the 3rd trimester of pregnancy. The main risk factors are hypertension and diseases of the connective tissue or of collagen (Marfan's syndrome and Ehlers-Danlos disease). The authors report a case of dissection of the aorta managed in a pluridisciplinary manner by the anaesthetists, cardiologists, obstetricians and cardiothoracic surgeons, which resulted in a favourable outcome for both mother and baby. The diagnosis of Ehlers-Danlos disease was made from the onset and, over a period of 10 years with C…

Fatal esophageal-arterial fistula 17 years following tetralogy of Fallot repair.

Blalock-Taussig shunts (subclavian to pulmonary anastomoses) have remained the most effective palliation in cyanotic heart disease. Late com … The circumstances leading to this dramatic outcome are outlined and discussed. It is important for cardiologists caring for congenital heart disease patients to be aware of the late complications of congenital heart surgery, and carry out the proper follow-up investigations.

Diagnostic reliability of M-mode echocardiography for detecting mitral valve prolapse in 50 consecutive panic patients.

Fifty consecutive panic patients had M-mode echocardiographs read independently by two cardiologists with expertise in echocardiography. In this prospective study, there was poor interrater reliability (22 of 50; K = 0.11) for diagnosis of mitral valve prolapse (MVP). On repeat evaluation 10 months later there was also unacceptable intrarater reliability for each reader: 22 of 35 (K = 0.41) and 22 of 35 (K = 0.45). We conclude that M-mode echocardiography is clinically unreliable for establishing the diagnosis of mitral valve prolapse. These findings suggest that the variable reporting of M-mo…

Comparing guideline adherence and readability: Artificial intelligence with deep learning versus specialized physicians in peripheral artery disease management.

BACKGROUND: Peripheral artery disease (PAD) is a global health challenge. Advances in artificial intelligence (AI), such as large language models (LLMs) and chain-of-thought (CoT) reasoning, offer novel approaches for clinical recommendations. This study compared the readability and guideline adherence of responses from physicians and AI for a standardized PAD case. METHODS: This cross-sectional study gathered responses from 30 specialized physicians (11 cardiologists, 19 vascular surgeons) across seven Latin American countries and 13 LLM systems (10 standard, three CoT). Both groups addresse…

The Role of Mechanical Circulatory Support Devices in Pulmonary Embolism: A Comprehensive Review.

Pulmonary embolism (PE) remains a leading cause of cardiovascular mortality, particularly in its high-risk and intermediate-high risk presentations where acute right ventricular (RV) failure is the primary driver of hemodynamic collapse. This review comprehensively examines the pathophysiology of RV failure in PE, their indications based on current evidence and guidelines, practical considerations for implementation, management strategies, complications, and integration with other therapeutic modalities. We also highlight the crucial role of a multidisciplinary team and discuss future directio

Subclavian Venous Spasm During Pacemaker Implantation: A Case Report.

We present the case of an 88-year-old female who presented to the cardiologist's clinic with symptoms of palpitations, dizziness, and fatigue. After cardiac monitoring, the patient was diagnosed with symptomatic bradycardia and required a pacemaker. During the pacemaker implantation procedure, attempts to access the subclavian vein were unsuccessful, and venography revealed severe venous spasm. The spasm did not relieve with time or nitroglycerin. Eventually, a leadless pacemaker was successfully implanted. We propose leadless pacemaker implantation as a viable management strategy for refracto

Two types of abdominal pain as the initial presentation of Takayasu arteritis: A case report.

BACKGROUND: Takayasu arteritis (TA) is a chronic inflammatory arteritis affecting large vessels, predominantly the aorta and its main branches. Diagnosis is usually delayed in most patients owing to the absence of typical clinical manifestations in the acute phase until the development of substantial arterial symptoms. The purpose of this case report is to describe abdominal pain being an uncommon symptom in TA. LEARNING OBJECTIVE: The clinical presentation of TA can be non-specific and can have serious consequences if the diagnosis is delayed, helping the clinicians who can treat with such p

Non-specific intraventricular conduction delay and ventricular pre-excitation: "Pseudo-P wave", warning for it!

Atrial fibrillation (AF) is the most common cardiac arrhythmia in adults with substantial morbidity and mortality. The diagnosis of AF is established by electrocardiogram showing heart rhythm without clear P waves with irregular RR intervals. The electrocardiographic diagnosis of AF is not always easy and can still hide pitfalls even for the most experienced cardiologist. We present the case of an 86 years old patient admitted to our hospital for Covid-19 infection affected by dilated cardiomyopathy with an electrocardiogram showing a non-specific intraventricular conduction delay mimicking si

Emergency department diagnosis of infective endocarditis using bedside emergency ultrasound.

Infective endocarditis is a challenging diagnosis that is rarely made in the emergency department. As the use of focused emergency ultrasound expands into more applications, including advanced echocardiography, the diagnosis of infective endocarditis may be made earlier, potentially leading to more timely treatment. We report a case of an ill-appearing patient presenting to the emergency department with an indwelling central venous catheter, a cardiac murmur, and necrotic toes, who was diagnosed with a large tricuspid vegetation and prominent tricuspid regurgitation on bedside emergency ultras

Point of care cardiac ultrasound applications in the emergency department and intensive care unit--a review.

The use of point of care echocardiography by non-cardiologist in acute care settings such as the emergency department (ED) or the intensive care unit (ICU) is very common. Unlike diagnostic echocardiography, the scope of such point of care exams is often restricted to address the clinical questions raised by the patient's differential diagnosis or chief complaint in order to inform immediate management decisions. In this article, an overview of the most common applications of this focused echocardiography in the ED and ICU is provided. This includes but is not limited to the evaluation of pati

The medical management and dental implications of long QT syndrome.

Long QT is a disorder of the heart's conduction system. It can be congenital or acquired. The acquired type is caused mainly by medications. The diagnosis of LQT depends on the clinical features, the family history, and the ECG findings. The clinical presentations range from dizziness to syncope and sudden death. The dentist should consult the cardiologist to avoid any complications that may put the LQTS patient at risk. Medications such as erythromycin and antifungals are contra-indicated and should not be prescribed. Local anaesthetics containing adrenaline and bupivacaine should be avoided.

[Congenital atrioventricular block--intrauterine diagnosis and neonatal pacemaker treatment].

A complete fetal heart block was diagnosed in 29 weeks gestation by means of 2-dimensional and M-mode echocardiograms and by analyzing the combined Doppler effect of atrial and ventricular movements. Because of increasing fetal ascites and edema, Digoxin treatment is given from 31 weeks gestation until birth. 30 hours postnatal the child develops congestive heart failure which disappeared completely after pacemaker treatment. We stress the importance of early diagnosis with possibility of prenatal treatment and optimal planning of delivery in cooperation between obstetrician and pediatric card

[Pregnancy in women wearing a heart valve prosthesis: 5 case histories (author's transl)].

The study of a personal series of five pregnancies and the review of 130 cases published enabled us to recall the physiological, clinical and therapeutic sides of the "pregnancy-heart valve prothesis" combination. Maternal mortality remains at 3%; morbidity, essentially in the form of haemorragic or thrombo-embolic accidents, comes to 45%. We insist on the importance of a pregnancy planned and accepted both by the cardiologist and the obstetrician as contraception is a difficult problem with these patients, classical oestroprogestative methods and intra-uterine devices being too much of a risk

[Update on functional mitral regurgitation: a comprehensive review].

In recent decades, the advances in the management of functional mitral regurgitation (MR), makes knowledge of this disease extremely important to the clinical cardiologist, as it has an increasing prevalence in patients older than 50 years. The objective of this article is to review the pathophysiological bases of MR, in order to differentiate its mechanism, and to correlate them with the clinical, hemodynamic, and imaging findings that assess the severity of the disease. These data are relevant for the adequate selection of the patient and the opportune moment of the therapeutic interventions

The History of Noonan Syndrome.

Early in her career, Jacqueline Noonan, a pediatric cardiologist, recognized that a number of children with valvular pulmonary stenosis had similar facial features. Dr. Noonan reported the clinical characteristics of this condition including short stature, hypertelorism, ptosis, mild mental retardation, undescended testes, and skeletal malformations. Further characterization of Noonan Syndrome led to the development of clinical criteria for the diagnosis of the condition. Identification of the first genetic cause of Noonan Syndrome, mutation of ptpn11 was reported in 2001. Multiple subsequent

Three patients with signs of acute flail mitral leaflet seen on emergency department echo: a critical constellation within the focused cardiac exam.

Acute flail mitral leaflet is a time-sensitive, reversible cause of cardiogenic shock. Transthoracic echocardiography (echo) is increasingly becoming a vital tool for non-cardiologist physicians who treat patients with undifferentiated chest pain and dyspnoea. The sonographic abnormalities seen in acute flail mitral leaflet are within the boundaries of a focused echo. Individually, these findings are non-specific. As a constellation, however, they are highly suggestive of this disease process. We present a case series of three patients with acute flail mitral leaflet seen on emergency departme
