[Gallbladder decompression in acute cholecystitis (author's transl)].

Decompression of the acute inflamed gallbladder via laparoscopy and inserting a small catheter not only allow to reduce the pressure on the wall but to instill antibiotic solutions into the gallbladder as well. In this way the inflammatory process will fade away and the operation (cholecystectomy) can be performed with much lesser risk. In old aged people the authors recommend so called "minioperations" like laproscopic cholecystotomy and -cholecystostomy.

[Treatment of acute cholecystitis. Clinico-statistical findings].

A series of 354 patients suffering from gall bladder and/or bile way conditions and who had undergone operation in the acute phase has been reviewed. The usefulness of early operation over more wait-and-see tactics is confirmed. Shortening of mean hospitalization is particularly significant. The difference in hospital mortality is insignificant. The only complication reported is the higher frequency of suppurative accidents in the surgical wound.

[Surgical treatment of acute cholecystitis (authors' transl)].

On the basis of 15 resp. 25 years of experience with surgical treatment of acute cholecystitis the authors evaluate their results of urgent and acute operations in 1.080 patients in whom the intensive conservative treatment of acute attacks did not sufficiently influence the symptoms of acute cholecystitis. From this group of 1.080 patients operated on urgently or acutely 21 died, the operative mortality being thus 1.9%. The mortality rate was highest after urgent operations where from 58 patients 10 died, the mortality being 17.2%. From the group of 889 acute cases operated on during 12, 24 o…

[Acute emphysematous cholecystitis. Report of 3 cases].

Three cases of acute emphysematous cholecystitis are reported. Our experience has been that this disease, compared with the occurrence of common acute cholecystitis, is rare. The clinical picture is discussed and emphasis is made concerning the roentgenographic diagnosis and surgical findings, such as obstruction of the cistic duct, which is probable the cause of the histopathologic changes found in the gall bladder. The negative results from the culture of material obtained from the gall bladder was probably due to antibiotic administration prior to surgery. A review of the recent literature …

[Acute emphysematous cholecystitis. One case (author's transl)].

Acute emphysematous cholecystitis is a rare condition. Its preoperative diagnosis is based upon radiological studies which should include a film in the dorsal decubitus position, a lateral and a film in lateral decubitus with the ray horizontal. The presence of air within the gall bladder lumen; and in a part or all of the wall. Additional findings: the presence of air in the peribiliary space; the presence of gas within the biliary tract, rarely reported, is of special interest in the case reported here. Gangrenous cholecystitis must be differentiated essentially from: 1 degree an internal bi…

Fatal Clostridium welchii septicaemia following acute cholecystitis.

A case of Clostridium welchii septicaemia following acute cholecystitis is described. The onset was acute and a rapidly fatal outcome ensued. Radiological findings were negative. An approach to the antibiotic treatment and general management is discussed.
