Experimental Laboratory Modeling of Choroidal Vasculature: A Study of the Dynamics of Intraoperative Choroidal Hemorrhage during Pars Plana Vitrectomy.

OBJECTIVES: Choroidal hemorrhages (CH) result from rupture of choroidal vessels leading to extravasation of blood into the suprachoroidal space. In this study, we aimed to understand the hemodynamics of CH by developing a purpose-built scale model of the choroidal vasculature and calculating stress levels in the model under different conditions. MATERIALS AND METHODS: We modeled the choroidal vasculature using a rubber tube 10 cm in length and 1 cm in diameter that was wrapped with conductive thread to enable the measurement of stress at the walls of the tube. Stress levels across the tube we…

Dynamic changes in choroidal conditions during anti-vascular endothelial growth factor therapy in polypoidal choroidal vasculopathy.

We defined the relationships between initial choroidal conditions and their dynamics and exudative changes during anti-vascular endothelial growth factor (anti-VEGF) therapy in polypoidal choroidal vasculopathy (PCV). One hundred treatment-naïve eyes of 100 patients with PCV treated for 24 months at Keio University Hospital with intravitreal ranibizumab or aflibercept monotherapy (three injections and PRN thereafter) were retrospectively analyzed. Wet macula risk after three induction injections, which affected visual prognosis, was predicted by initial pachyvessels in the choroid (foveal grea…

Choroid plexus remodeling associated with inflammatory CSF exposure in preterm acquired hydrocephalus.

Acquired hydrocephalus in preterm infants follows intraventricular hemorrhage (IVH) or central nervous system infection and is associated with inflammatory cerebrospinal fluid (CSF) profiles. We integrated clinical magnetic resonance imaging (MRI), CSF cytokine profiling, primary human choroid plexus (ChP) epithelial and endothelial cell assays, and independent transcriptomic and proteomic datasets to investigate ChP remodeling. Infants requiring CSF diversion exhibited greater ChP thickness independent of gestational age, ventricular size, and IVH severity, supporting its potential as a bioma…

Interactions of choroid and sclera in the myopia model of the chicken.

PURPOSE: Changes in choroidal thickness are currently used to predict future refractive error development but there is incomplete knowledge about the communication between choroid and sclera. We studied how choroidal thickness changes interact with scleral thickness changes and how the abundance of dopamine (DA)- and all-trans retinoic acid (atRA)-synthetizing choroidal cells varies when choroidal thickness is altered by drugs. METHODS: Changes in choroidal thickness were induced by a single intravitreal injection in the morning of the muscarinic antagonist atropine, the DA agonist apomorphin…

Developmentally Regulated Expression and Activity of Sulphotransferases in the Rat Choroid Plexuses.

During development, entry of any substances from the circulation into the brain is tightly regulated by a series of blood-brain interfaces. Notably, the choroid plexuses, which form the blood-cerebrospinal fluid barrier, serve as a key interface for molecular exchange in early life. Control mechanisms within the choroid plexuses include efflux transporters and conjugating enzymes, such as glutathione S-transferases and UDP-glucuronosyltransferases, which have been shown to play key roles in safeguarding the developing brain. Sulphotransferases are another family of conjugating enzymes reported…

Choroid plexus alterations in long COVID and their associations with IL-6.

SARS-CoV-2 disrupts the choroid plexus (ChP) epithelium by binding to the ACE-2 receptor, causing blood cerebrospinal fluid barrier leakage and permitting interleukin (IL)-6 and pathogens into the brain, subsequently leading to demyelination, white matter (WM) damage in long COVID, and clinical worsening. The role of the ChP in long COVID and its relationships to WM integrity, IL-6, clinical symptoms, and ACEIs/ARBs medications remains unclear. Fifty-two long COVID individuals, 21 COVID-19 survivors, and 26 healthy controls (HCs) completed Montgomery-Asberg Depression Rating Scale (MADRS), Mon…

TTN+ macrophages are enriched in the human choroid plexus in Alzheimer's disease.

The choroid plexus (ChP) regulates cerebrospinal fluid homeostasis and immune surveillance, but its cellular organization and response to Alzheimer's disease (AD) remain incompletely defined. Here, we integrate single-nucleus RNA sequencing with spatial transcriptomics to generate a high-resolution atlas of the adult human ChP. We identify all major epithelial, stromal, and immune populations and resolve heterogeneity within macrophages, including a distinct subset expressing the giant protein TTN. These TTN+ macrophages exhibit coordinated cytoskeletal, stress-response, and phagocytic gene pr…

Genetic and adenoviral ablation of the choroid plexus reduces postnatal hippocampal neurogenesis.

BACKGROUND: Choroid plexus (ChP) produces cerebrospinal fluid (CSF), and regulates brain development and adult subventricular zone (SVZ) neurogenesis, but its role in hippocampal subgranular zone (SGZ) neurogenesis in adulthood and early postnatal stages is not well understood. Current tools to directly manipulate neonatal ChP/CSF volume are very limited, representing an urgent need in the field. METHODS: We first discovered the specific "leaky" expression of DTR gene in the ChP of adult ROSA26-iDTR mice which can be used to specifically ablate ChP in adult brain that generated robust and lon…

Longitudinal changes of choroid plexus volumes and MRI ratios in multiple sclerosis.

This longitudinal study aimed to identify the predictive role of choroid plexus enlargement for disability progression and its association with tissue damage in early multiple sclerosis patients. Forty patients with clinically isolated syndrome or early relapsing-remitting multiple sclerosis underwent brain MRI scanning at baseline, after 2 and after 6-8 years. FreeSurfer segmentation was used for brain volume estimation, and the choroid plexus was segmented by use of an independent automated segmentation tool. MRI ratios (T1 weighted through fluid attenuated inversion recovery image values) w…

Aberrant choroid plexus formation drives the development of treatment-related brain toxicity.

Brain tumors are commonly treated with radiotherapy, but the efficacy of the treatment is limited by its toxicity to the normal tissue inclu … em markers, altered neuroepithelial stem cell differentiation, and an increase of ZO1+/AQP1+/CLDN3+-choroid plexus (CP)-like structures accompanied by an upregulation of IGF2 mRNA, known to be expressed in CP and cerebrospinal fluid. The altered differentiation is attributed to changes in the WNT/BMP signaling pathways. We conclude that aberrant CP format …

Spatial and Temporal Changes in Choroid Morphology Associated With Long-Duration Spaceflight.

PURPOSE: Amid efforts to understand spaceflight associated neuro-ocular syndrome (SANS), uncovering the role of the choroid in its etiology is challenged by the accuracy of image segmentation. The present study extended deep learning-based choroid quantification from optical coherence tomography (OCT) to the characterization of pulsatile and topological changes in the macular plane and investigated changes in response to prolonged microgravity exposure. METHODS: We analyzed OCT macular videos and volumes acquired from astronauts before, during, and after long-duration spaceflight. Deep learni…

Choroid plexus enlargement correlates with subcortical tau deposition in progressive supranuclear palsy.

OBJECTIVE: The choroid plexus (CP) has gained renewed attention for its role in waste clearance in neurodegenerative disorders. However, its involvement in progressive supranuclear palsy (PSP) remains unclear. This study aimed to investigate CP volume changes in patients with PSP compared to Parkinson's disease (PD), and explore its relationship with tau deposition in PSP patients. METHODS: A total of 204 participants (92 PSP, 78 PD, 34 healthy controls (HC)) underwent structural MRI, with 63 PSP patients receiving 18F-Florzolotau positron emission tomography. CP volume was compared across th…

Altered epithelial development of the lateral ventricle choroid plexus in Mllt11 mutants.

The choroid plexuses (ChPs) are modified epithelial structures that penetrate all four cerebral ventricles and secrete cerebrospinal fluid. They consist of a central stroma that is vascularized with fenestrated blood vessels and connective tissue. The ChPs are of dual embryonic origin, with forebrain neuroepithelial cells contributing to the epithelial component and mesenchymal cells contributing to the stromal cells. The growth of the ChPs into the ventricular spaces is fueled by the migration and proliferation of neuroepithelial progenitor cells originating from the cortical hem. However, th…

Region-informed machine learning model for choroid plexus segmentation in Alzheimer's disease.

INTRODUCTION: The choroid plexus (CP), a critical structure for cerebrospinal fluid (CSF) production, has been increasingly recognized for its involvement in Alzheimer's disease (AD). Accurate segmentation of CP from magnetic resonance imaging (MRI) remains challenging due to its irregular shape, variable MR signal, and proximity to the lateral ventricles. This study aimed to develop and evaluate a region-informed Gaussian Mixture Model (One-GMM) for automatic CP segmentation using anatomical priors derived from FreeSurfer (FS) software and compare it with manual, FS, and one previous GMM-base…

Sialoglycoconjugate profiling of human choroid, retinal pigment epithelium, and basal laminar deposits.

Age-related macular degeneration is a leading cause of central vision loss in the elderly. Early hallmarks of the disease include basal laminar deposit beneath the retinal pigment epithelium (RPE) and choriocapillaris degeneration. We utilized sialic acid binding lectins Sambucus nigra/Elderberry Bark Lectin (EBL) and Maackia amurensis lectin II (MAL-II), to assess the localization of ɑ-2,6 and ɑ-2,3 sialic acids, respectively, in human macular retina, RPE, basal laminar deposits, and choroid. Photoreceptor carbohydrate epitopes differ based on retinal topography, with MAL-II recognizing fovea…
