Three cases with injury of the axillary artery and brachial plexus complicating a displaced proximal fracture of the humerus are presented. In two patients the arterial injury was not recognized on admission. Vascular repair had to be carried out in all three cases. Two patients showed a persistent neurological deficit. In two of the cases, the arterial injury may have been caused by an attempt at closed reduction of the fracture. The possibility of axillary arterial injury should be considered in proximal fractures of the humerus with severe medial displacement of the shaft of the humerus.

Some issues related to management of fractures of the condylar process remain unsettled. In certain areas, however, there is consensus. In the child, with few exceptions, closed treatment is preferable, fixation periods should be short, a vigorous postfixation exercise program is essential, and patients should have long-term follow-up. In the adult, it is important to restore ramus height by an open reduction when (1) there are bilateral fractures without contact of the segments, (2) there is a unilateral fracture in an edentulous patient, or (3) there is a unilateral fracture in a dentulous p

INTRODUCTION: Closed repair of peritrochanteric fractures with cephalomedullary nail fixation with the patient in the lateral decubitus position on a flat table with manual traction may allow improved fracture reduction and fixation in comparison with what is possible in a supine setup.
STEP 1 OPERATING ROOM PREPARATION: Perform sterile preparation, have the patient brought to the operating room, induce anesthesia, and place the patient in the lateral decubitus position before sterile draping.
STEP 2 FRACTURE REDUCTION: Reduce the fracture using manual traction and slight internal or externa

Fractures of the base of the first metacarpal are particularly common. The present examination was carried out in order to find out a correlation between the clinical outcome and the type of the fracture, the quality of reduction, the surgical procedure, and the extent of osteoarthrosis. From February 1992 to August 1997, 24 patients with Bennett fracture-dislocation, Rolando fracture und extraarticular fracture--eight patients in each group--were treated operatively. After a median interval of 33 months, 21 patients were reviewed. The evaluation was done using a new score with subjective (pai

The incidence of diacapsular fracture of the mandibular condyle (DFMC) has increased in recent years. However, the specific modality of treatment which would provide maximum benefit and good results for adult patients of DFMC has been controversial. To evaluate and compare morphological changes of temporomandibular joint (TMJ) after open and closed treatments for DFMC, a sheep model of a type B DFMC was created by osteotomy. Eight sheep models of type B DFMC were randomly divided into 2 groups to receive closed treatment (Group 1) and open reduction and internal fixation (ORIF, Group 2), respe

INTRODUCTION: Serious complications can occur in the management of displaced femoral neck fractures in children. Are there ways to anticipate this risk and better define eventual contributing factors?
PATIENTS AND METHODS: Twenty-one children (14 boys and 7 girls) were treated between 1999 and 2006 for displaced femoral neck fractures. Nine type II, 10 type III and two type IV fractures were observed according to the Delbet's classification. Anatomical open reduction was performed in 13 patients and closed reduction in eight patients. At a mean follow-up of 26.4 months, the outcome was assess

PURPOSE: This study aimed to evaluate whether adding a median nerve and a superficial branch of the radial nerve block to hematoma block improves analgesia during distal radius fracture reduction and to identify factors associated with increased pain perception.
METHODS: A prospective, randomized, controlled study was conducted in two tertiary hospitals, including 180 adult patients with displaced distal radius fractures requiring closed reduction. Patients were allocated either to an isolated hematoma block group or to a combined block group receiving hematoma block with median nerve and sup

BACKGROUND: Distal radius fractures (DRFs) are some of the most common injuries suffered by the elderly; however, there are varied opinions on management. As there is limited research on patient factors affecting functional outcomes in conservatively managed patients with DRFs, this study aimed to find predictive factors for functional outcomes in such patients.
METHODS: Patients with DRFs satisfying the inclusion criteria were managed with closed reduction and slab followed by cast application. Radiological parameters (radioulnar angulation, dorsal tilt, radial height, and ulnar variance) an

This article describes a case of closed reduction of a dislocated total hip arthroplasty (THA) out of a constrained acetabular liner using a novel technique with a peg board secured to the operating table.A 79-year-old woman sustained multiple dislocations after a primary THA. She was converted to a constrained liner and sustained no further dislocations for 5 years. She presented after dislocating her hip while getting out of a chair. The patient was taken to the operating room, placed under general anesthesia, and laid supine on the operating table, to which a peg board had been previously s

Tibial plateau fractures, common injuries in the middle-aged, often result from falls or vehicular accidents. The wide spectrum of fracture patterns can be theoretically related to the sequential application of bending (valgus/varus) and compression forces. End-results of 599 fractures in 13 different studies were compared by fracture pattern using a clear-cut fracture classification. Minimally displaced (less than 5-10 mm displacement) fractures average 85-90 per cent acceptable end-results. Associated soft tissue lesions include frequent meniscal (50% of cases) and ligamentous (10-30%) injur

Closed pantalar dislocations are a rare variant of an uncommon injury. Pantalar dislocations are typically caused by high-energy trauma resulting in an open injury with associated fracture of the articulating bones. Given its obscurity, the literature on closed pantalar dislocations is scarce, and no standard treatment protocol has been accepted. This case report chronicles the treatment and outcome of a 29-year-old man who presented with a checkrein deformity of all digits after a closed pantalar dislocation with 6-month follow-up. A comprehensive literature review found 28 articles represent

BACKGROUND: Simultaneous ipsilateral fractures of the proximal and distal tibial articular ends are uncommon. Although this pattern has sometimes been described as a 'floating tibia', the term is not standardised. Management is dictated by fracture morphology, soft-tissue status, patient physiology and the ability to obtain stable articular reduction without additional soft-tissue injury.
CASE PRESENTATION: A 45-year-old man sustained a closed Schatzker VI tibial plateau fracture, an ipsilateral partial-articular distal tibial plafond fracture documented as AO/OTA 43-B2 and a comminuted fibul

INTRODUCTION: Patella fracture can occur due to direct injury to the knee or indirect eccentric contraction of the quadriceps tendon. These injuries can present in different configurations which require acceptable reduction and good fixation. Patients are at risk of not only losing their knee extensor mechanism but also having a defective patellofemoral articulation. Hence, the main aim in treating such fractures is to restore the knee extensor mechanism. Surgical options for treating patella fracture include tension band wiring, wiring through cannulated screws, fixation with plate, and sutur

INTRODUCTION: We hypothesized that treating distal radius fractures with cast only or closed reduction and cast is associated with high malunion risk and that the risk is higher in patients with low bone mineral density (BMD).
MATERIALS AND METHODS: We prospectively studied 130 patients aged 40 years or older with distal radius fractures treated with cast only (87 patients) or closed reduction and cast (43 patients). Radiographs were obtained before treatment, after reduction (in the closed reduction group), and at 1 year. We measured calcaneal BMD with DXA scanner and calculated T scores. We

Application of humeral nail in the treatment of humeral shaft fractures is a relatively novel method of fracture fixation. Its application at Clinical Department of Traumatology, Sestre milosrdnice University Hospital Center began in 2001. The operative procedure should be performed using a minimally invasive technique without fracture opening in order to maintain optimal conditions for fracture healing including hematoma management. However, in everyday practice it is often impossible to obtain satisfactory fracture reduction using the closed procedure, so open reduction and additional fixati
